We describe nine patients with underlying hematologic cancer in whom primary cutaneous aspergillosis developed at the sites of Hickman intravenous catheters. Our patients, 17 to 74 years of age, were all immunocompromised either from their primary disease or from chemotherapy, and the Hickman catheters had been placed to provide venous access for chemotherapy or hyperalimentation or both. Clinical signs of infection included erythema, induration, and cutaneous or subcutaneous necrosis at the point of entry into the subclavian vein, in the subcutaneous tunnel, or at the exit site from the skin. Diagnosis was confirmed by positive wound culture for Aspergillus flavus in all but one patient. Treatment consisted of intravenous amphotericin B, oral flucytosine, and local wound care. Three patients recovered completely without operative débridement; three others recovered after operative débridement and delayed grafting. Two patients died of disseminated aspergillosis, and one died of unrelated causes while recovering from primary cutaneous aspergillosis. Successful treatment required resolution of aplasia or leukopenia, catheter removal, systemic treatment with amphotericin B, and local wound care. We conclude that primary cutaneous aspergillosis, a rare infection, may occur at the sites of Hickman catheters in immunocompromised patients, and that it is a serious complication requiring prompt diagnosis and treatment.  相似文献

Calciphylaxis is an uncommon complication of end stage renal disease (ESRD) and secondary hyperparathyroidism. It characterized by cutaneous necrosis with mural calcifications and thrombosis in the small vessels of dermis. It is important to diagnose and treat, because of mortality rate from calciphylaxis is very high. We present the case of a patient with ESRD and type II diabetes mellitus developing calciphylaxis of the both upper and lower extremities had normal corrected calcium-phosphate product level. After amputation, necrosis was showed rapid progression resulting in death in one month.  相似文献

A hundred and eight subjects were studied for the frequent occurrence innervation anomalies in upper and lower extremities. Martin-Gruber Anomalies (MGA) were found in 19 (17.5%) subjects. The anomaly was bilateral in 14 subjects (73.6%). No case of motor ulnar to median nerve anastomosis in the forearm could be found. The 73.1, 20.8 and 14.3% incidences of the neural communication between the ulnar and median nerves in the hand were detected, in the Abductor Pollicis Brevis (APB), in the First Dorsal Interosseus (FDI) and in the Abductor Digiti Minimi (ADM) muscles, respectively. The existence of an accessory deep peroneal nerve was found in 23 (21.3%) subjects and 40 (18.5%) of legs. The anomaly was bilateral in 17 subjects (74%). We observed both MGA and accessory deep peroneal nerve in 9 cases.  相似文献

Prenatal analysis of the fetal structures gives us information about fetal growth and gestational age. The aim of this study was to investigate the morphologic structure of the upper and lower extremities of Turkish fetuses during the fetal period, to measure the morphometric values, and to determine the relationship between CRL (crown-rump length) and growth of the foot. In this study, 106 human fetuses (56 males, 50 females) without external anomalies and aged between 10–40 post-menstrual weeks (PMW) were studied. In the upper extremity, the width of shoulder and the length of the arms, forearms and hands were measured. In the lower extremity, the width of the iliac crest, knee condyles, feet, and heels and the length of the thighs, legs and feet were measured. A significant correlation was found between all parameters taken within the fetal period and PMW (p < 0.001). Statistically significant correlations found between foot-growth measurements and fetal parameters indicate that foot length may be a good predictor of age. The measurements of the upper and lower extremities during fetal period are a reliable parameter for use in the assessment of gestational age. These measurements are particularly useful when other parameters do not accurately predict gestational age in some cases such as hydrocephalus, anencephaly, short-limb dysplasia. Our opinion is that the measurements can be useful to assess gestational age in several fields such as anatomy, pathologic anatomy (fetopathology), forensic medicine, medical imaging, obstetrics and pediatrics.  相似文献

Task-oriented repetitive movements can improve muscle strength and movement co-ordination in patients with impairments due to neurological lesions. The application of robotics and automation technology can serve to assist, enhance, evaluate and document the rehabilitation of movements. The paper provides an overview of existing devices that can support movement therapy of the upper extremities in subjects with neurological pathologies. The devices are critically compared with respect to technical function, clinical applicability, and, if they exist, clinical outcomes.  相似文献

During the dissection of the right and left upper limbs of a 70-year-old female cadaver, we encountered combined vascular anomalies. On the left side, we observed the presence of a lateral inferior superficial brachial artery, large anterior interosseous artery and an absence of a radial artery, which is quite a rare anomaly. The lateral inferior superficial brachial artery that arose 39 mm distal to the brachial artery descended to the wrist. The anterior interosseous artery originated from the brachial artery at the level of 12 mm distal to the head of the radius and descended on the anterior aspect of the interosseous membrane with the anterior interosseous branch of the median nerve. On the right side, a trifurcation of the brachial artery was observed. It gave off three terminal branches: the radial, ulnar and a muscular artery at the proximal one-third of the humerus. It is obvious that accurate information concerning unusual patterns of the arteries in the upper limbs is relevant clinically, especially in order to avoid accidental injury or intra-arterial injection.  相似文献

Arteries of 98 legs amputated for diabetic and arteriosclerotic gangrene were studied, as well as arteries of diabetic patients whose death was not associated with the lower limb pathology; 10 legs of subjects killed in an accident were examined for control. The authors applied histological, histochemical, angiographical, and morphometrical methods in line with multifactorial statistical analysis of the revealed changes. Morphological manifestations of the arterial changes found in the lower limbs of patients with diabetic and arteriosclerotic gangrene appeared to vary significantly in frequency and predominant localization of lipid/fibrous plaques and organized thrombi causing the occlusion of major arteries, in the patterns of plaque and muscular layer calcinosis, as well as in the features of the development of collateral circulation. Therefore, the authors consider diabetic macroangiopathy not as early acute arteriosclerosis, but as a separate pathologic entity.  相似文献

Four patients with giant cell arteritis (GCA) involving the vessels of lower extremities (3 males and 1 female, the age range from 57 to 67 yr) are described. The maximal changes were revealed in the femoropopliteal segment of the arterial bed. The role of morphologic study for the differential diagnosis of GCA with obliterating atherosclerosis is stressed.  相似文献
