Iatrogenic aneurysms are usually postcatheterisation pseudoaneurysms of the femoral artery. Until recently, the treatment of choice was ultrasound guided compression repair. A case of pseudoaneurysm of the axillary artery, arising as a complication of pacemaker insertion in an 83 year old man is reported. Compression repair was not possible in this case, and so the aneurysm was occluded by percutaneous ultrasound guided thrombin injection directly into the aneurysm sac. Percutaneous ultrasound guided thrombin injection is a promising new minimally invasive technique for the treatment of iatrogenic pseudoaneurysms.

Pseudoaneurysm formation of the femoral artery is a well-known complication following catheter-based vascular procedures. Ultrasound-guided compression or surgical correction are commonly used for its repair. We describe a new method of treatment for femoral pseudoaneurysm. The pseudoaneurysm is visualized by contralateral angiography and thrombosed with a percutaneous thrombin injection while the distal vessel is isolated with a brief balloon inflation. Fluoroscopically guided percutaneous thrombin injection is a promising, minimally invasive technique for the treatment of iatrogenic pseudoaneurysm, especially in patients with compromised distal circulation.  相似文献

The increasing number of coronary interventions and more aggressive anticoagulation is associated with a larger number of iatrogenic femoral pseudoaneurysms (PAs). The majority of PAs clot spontaneously or can be repaired by ultrasound-guided compression or ultrasound-guided thrombin injection. The therapeutic armamentarium for the remaining PAs ranges from different interventional procedures to surgical occlusion. In a small case series, we describe our initial experiences with a new, less invasive approach after unsuccessful compression therapy. Ultrasound-guided injection of 52 +/- 33 ml saline beneath the communication tract of the PA yields to rapid occlusion in six consecutive patients. During 4 weeks of follow-up, the PAs remained occluded in all patients and surgical intervention could be avoided.  相似文献

目的探讨超声引导下凝血酶注射(UGTD治疗医源性股动脉假性动脉瘤(PSA)的安全性和可行性。方法2000年1月至2007年2月,对36例经皮股动脉路径行冠状动脉介入诊疗术后发生的股动脉PSA进行了UGTI,其中男21例,女15例,年龄34482(63.5±10.8)岁。造影术后发生11例,支架置入术后发生25例。凝血酶注射成功后平卧4~6h,所有病例均在治疗后1~3d复查超声,30d临床随访。结果36例患者,单囊腔PSA32个,复合囊腔PSA4个(≥2个腔),瘤腔平均为(2.98±1.30)cm×(1.84±0.75)cm,凝血酶注射剂量为250~1000(644.29±239.10)U,34例患者1次UGTI即刻闭合瘤腔,2例注射凝血酶500U后动脉与瘤腔通道血流明显减弱,在超声引导压迫下5min闭合。UGTI治疗PSA成功率为94.4%0(34/36)。1例注射凝血酶1000U后虽然瘤腔闭合,但股浅动脉内血栓形成,行外科手术治疗。1例注射凝血酶500U后瘤腔闭合,但2min后出现寒颤、高热过敏反应,对症处理后好转。术后1d复发2例,1例超声引导压迫后瘤腔闭合,另1例再次注射凝血酶1000U成功闭合,30d临床随访无复发,UGTI治疗PSA复发率为5.6%(2/36)。结论UGTI治疗股动脉PSA是一简单、安全、快速、耐受好的方法,可作为临床治疗PSA的首选方法。  相似文献

目的 对经皮冠状动脉介入治疗 (PCI)后出现的股动脉假性动脉瘤 (FAP)的患者进行回顾性分析 ,了解PCI后FAP的发生及临床处理情况。方法 入选 10 5例PCI后FAP的患者 ,并与同期无血管并发症的患者进行对照 ,分析FAP的临床特点和处理方法。结果 本组病例冠状动脉造影的FAP发生率为 0 8% ,PCI的FAP发生率为 1 6 % ,logistic回归分析显示女性 (P =0 0 4 7)、动脉鞘管型号≥ 7F(P =0 0 0 7)、介入术中平均压水平 (P <0 0 0 1)和围术期应用低分子肝素 (P <0 0 0 1)与FAP的发生显著相关。弹力绷带加压包扎修复的成功率为 6 3% ,超声指导下压迫修复的成功率为 85 %。结论 女性、鞘管直径、介入术中血压水平以及围术期应用低分子肝素是发生FAP的危险因素。FAP超声指导下压迫修复成功率高。  相似文献

Thrombin injection is frequently used to occlude iatrogenic pseudoaneurysms in larger vessels, but has never successfully been used in the radial artery location. Here we report the use of this treatment in a patient with radial artery pseudoaneurysm following coronary intervention. After Doppler sonographic visualization of the pseudoaneurysm cavity and its neck, an ultrasound-guided transcutaneous injection of thrombin was carried out. Immediately after the injection, the pseudoaneurysm was completely clotted and Doppler measurement confirmed the stop of blood flow. The result suggests that ultrasound-guided injection of thrombin into a radial artery pseudoaneurysm following coronary intervention is a feasible alternative to surgical intervention.  相似文献

BACKGROUND: Pseudoaneurysm is a common complication of cardiac catheterization and coronary intervention with an incidence of 2% even in experienced centers. PATIENTS AND METHODS: In a feasibility study conducted between December 2004 and February 2006 we enrolled 76 patients consecutively to receive local thrombin injection (mean 329 IU; range 100-800 IU) into the aneurysma sac. RESULTS: Ultrasound guided thrombotic occlusion of pseudoaneurysms was successful after one injection in 83% of the patients, 17% of the patients required more than one injection. The overall success rate of the procedure was 98,9%. No peripheral embolisation of thrombin was noted during any injection and we registered no other complication that needed any further intervention. CONCLUSIONS: We conclude that ultrasound guided occlusion of pseudoaneurysms using thrombin injection with a success rate of the procedure of 98,9% is feasible and safe.  相似文献

目的评价老年女性患者经桡动脉和股动脉途径行PCI的安全性和临床效果。方法选择行PCI、年龄≥75岁的老年女性患者332例,按手术入径方式分为桡动脉组184例和股动脉组148例,回顾性分析患者的临床资料。冠状动脉造影检查显示,冠状动脉狭窄≥75%者行冠状动脉支架置入术。比较桡动脉和股动脉途径介入治疗临床疗效。结果 332例患者中有96例接受PCI,桡动脉组和股动脉组患者3支病变比率分别为39.7%和47.4%,分叉病变比率分别为60.3%和52.6%。与桡动脉组比较,股动脉组患者手术时间长,外周血管损伤明显增多(P0.05),术后观察时间及卧床时间明显延长,假性动脉瘤、下肢静脉血栓例数明显增多,穿刺动脉闭塞明显减少(P0.05)。结论老年女性患者自身病情复杂,选择桡动脉途径行PCI可以减少局部血管并发症,近期治疗风险小,病死率低,可取得较理想疗效。  相似文献

This study examined the relationship between the femoral arteriotomy location and the risk of femoral access site complications after diagnostic and interventional cardiac catheterization procedures. One of the most common complication of cardiac catheterization and percutaneous coronary intervention (PCI) involves the vascular access site. The femoral approach is the most frequent site of vascular access during invasive cardiac procedures. This approach is associated with vascular complications, such as retroperitoneal bleeding, which can be life-threatening. If angiographic predictors of retroperitoneal bleeding can be identified, this complication could be avoided. A prospective cohort of 33 patients with femoral access site complications was subgrouped based on the angiographic arteriotomy site. Concurrent patients without complications were randomly selected to form a control group. Study and control patients were compared on presenting risk factors and outcomes. Logistic regression analysis was used to identify independent predictors for femoral access site complications. Arteriotomy location above the most inferior border of the inferior epigastric artery in patients undergoing PCI was associated with 100% of all retroperitoneal bleeds (P < 0.001). Low, high middle, and high femoral arteriotomy sites were associated with 71% of all vascular access complications. The combination of these locations for the femoral arteriotomy was an independent predictor of adverse vascular access site complications beyond traditional risk factors (odds ratio = 28.7; CI = 6.73-122.40; P < 0.0001). Vascular complications occurred more frequently in patients who were of older age (72 vs. 66 years; P < 0.001). The location of the femoral arteriotomy site assessed by a femoral angiogram is predictive of life-threatening complications. Patients undergoing PCI with an arteriotomy above the most inferior border of the inferior epigastric artery are at an increased risk for retroperitoneal bleeding. This complication may be avoided by risk-stratifying patients prior to intervention with a femoral angiogram.  相似文献

Objectives

The objective was to assess the effect of ultrasound (US)‐guidance compared to the anatomical landmark (AL) approach in patients requiring femoral artery (FA) access for coronary angiography/percutaneous coronary interventions (PCI).

Background

US‐guidance has been proposed as a strategy to optimize FA access, potentially leading to decreased vascular complications.

Methods

Patients requiring FA access for coronary angiography/PCI were randomized to the US‐guided or AL approaches. The primary endpoint was a composite of immediate procedural vascular outcomes, and access‐site outcomes at day one. Results were subsequently pooled in a study‐level meta‐analysis of randomized trials comparing US‐guided FA access to another strategy.

Results

A total of 129 patients were randomized (64 US‐guided group; 65 AL group). The primary endpoint occurred in 30 patients (47%) with US, and in 39 patients (62%) with AL (P = 0.09). Four additional studies met the inclusion criteria and were included in the meta‐analysis (1553 patients). Following data pooling, bleeding events (OR = 0.41; 95%CI 0.20‐0.83; P = 0.01), venipunctures (OR = 0.18; 95%CI: 0.11‐0.29; P < 0.0001), and multiple puncture attempts (OR = 0.24; 95%CI: 0.19‐0.31; P < 0.0001) were significantly improved with US‐guidance, but not successful common FA cannulation (OR = 0.84; 95%CI: 0.60‐1.17; P = 0.29).

Conclusion

Our study did not show significant benefits for the use of US to guide arterial femoral access compared to the anatomical landmark approach, but pooled analysis of five randomized trials showed decreased rates of bleeding events and venipunctures, and improved first‐pass success. The clinical impact of these findings is uncertain, and do not warrant a systematic use of US‐guidance in this clinical setting.

Rationale:Pseudoaneurysm (PSA) is a common complication related to vascular intervention, and surgical therapy is the primary method. However, a giant brachial artery PSA over 2 weeks is rarely observed. Due to the adhesion of surrounding tissue, thrombus organization, the extensive injury, and the high expense of transluminal stent-graft placement, a single ultrasound-guided local high-dose thrombin injection can be a therapy option. Such cases are rarely reported.Patient concerns:A 71-year-old man with a history of left elbow fossa interventional puncture presented to our hospital with a pulsatile mass in the left elbow fossa. He had a history of cerebral infarction 32 years prior without sequelae, emphysema for more than 2 years, hyperlipidemia for 3 months, and prostatic hyperplasia for 8 months. After conservative therapy, the lumbar compression fracture produced by trauma 24 years ago healed, and the intracranial hematoma induced by trauma ten years ago was absorbed.Diagnosis:Ultrasound examination showed giant mixed echoes on the posterior medial side of the left brachial artery.Interventions:The patient underwent a single ultrasound-guided local high-dose thrombin injection to treat giant brachial artery PSA.Outcomes:Following therapy, the ultrasonography revealed that extensive thrombosis immediately formed in the cavity, and the internal blood flow signals had completely vanished. A week later, a physical examination showed that the PSA had shrunk with no apparent tenderness and that the texture had hardened. Pulsation and vascular murmurs disappeared. Ultrasound showed that the PSA was reduced, and no blood flow signals were found.Lessons:A single ultrasound-guided local high-dose thrombin injection had a considerable effect in curing large iatrogenic PSA. However, when deciding on the best therapy, specificity must be taken into account.  相似文献

OBJECTIVE—To determine the effect of ultrasound guided injection in recalcitrant idiopathic plantar fasciitis.
METHODS—Four patients with a clinical diagnosis of idiopathic plantar fasciitis, who were unresponsive to palpation guided injection with triamcinolone acetonide and local anaesthetic, underwent ultrasonographic examination of the heel.
RESULTS—The following ultrasonographic features were noted:- (a) increased thickness of plantar fascia in symptomatic heels compared with asymptomatic heels, (b) loss of distinction of the distal plantar fascia borders, (c) reduced echogenicity of the plantar fascia. Ultrasound guided injection of the enlarged, hypoechoic plantar fascia resulted in complete relief in four of five heels(mean duration of follow up=24 months) in three cases. One patient developed a recurrence of symptoms after six months.
CONCLUSION—Ultrasound allows for confirmation of the clinical diagnosis and ultrasound guided injection produces a good clinical response when unguided injection is unsuccessful. The technique is quick, inexpensive, and entails no radiation exposure.

To study the possibility of using thrombin generation tests in platelet-rich and platelet-poor plasma for evaluation of dual antiplatelet therapy efficacy in patients with coronary artery disease (CAD), following percutaneous coronary intervention. Venous blood was analyzed from CAD patients aged 53–75 years who had undergone percutaneous coronary intervention with stenting within one year and had been receiving standard doses of clopidogrel and aspirin (75 and 75–100 mg per day, respectively). The control group comprised age- and sex-matched subjects without clinical signs of CAD who were not receiving these drugs. Thrombin generation tests were performed in platelet-rich and platelet-poor plasma. Intravascular platelet activation, induced platelet aggregation, and routine coagulation were evaluated. Antiplatelet treatment did not influence results of routine coagulation tests or intravascular platelet activation. The dual antiplatelet therapy affects collagen-induced platelet aggregation (44 ± 2.5 vs. 7.9 ± 2.6%, р = 10?7) and leads to decreases in endogenous thrombin potential (1900 ± 85 vs. 1740 ± 95 nM?min, p = 0.0045), maximum thrombin concentration (134 ± 9.5 vs. 106 ± 6.5 nM, p = 4?10?6), and increases in time to peak thrombin (27 ± 1.5 vs. 31 ± 2 min, p = 0.0012). Decreases in thrombin generation rate showed the highest statistical significance (13 ± 2 vs. 7.9 ± 0.8 nM/min, p = 10?8). Antiplatelet treatment did not alter thrombogram parameters for platelet-poor plasma.  相似文献

Spontaneous coronary artery dissection is a rare cause of myocardial ischemia and sudden death. Coronary aneurysms and pseudoaneurysms, which may occur after percutaneous coronary interventions, rarely occur spontaneously. 目的:探讨股动脉、腹主动脉及颈动脉粥样硬化与冠状动脉粥样硬化的相关性。方法: 采用高频超声测量109例行冠状动脉造影术后1周的患者股动脉、腹主动脉及颈动脉的内-中膜厚度(IMT)、斑块积分及斑块数目。结果: 股动脉、腹主动脉及颈动脉粥样硬化与冠状动脉粥样硬化呈正相关(P<0.01、P<0.05);股动脉斑块预测冠状动脉粥样硬化的灵敏度为89%,特异度为77%,准确度84%;腹主动脉斑块预测冠状动脉粥样硬化的灵敏度为73%,特异度为72%,准确度72%;颈动脉斑块预测冠状动脉粥样硬化的灵敏度为83%,特异度为79%,准确度82%。结论: 超声检查股动脉、腹主动脉及颈动脉IMT及斑块可间接预测不同程度的冠状动脉粥样硬化;股动脉的灵敏度与准确度较腹主动脉更好。  相似文献
