内窥镜采集大隐静脉行冠状动脉旁路移植术患者的术后并发症处理策略

目的 回顾性分析内窥镜采集大隐静脉(EVH)行冠状动脉旁路移植术(CABG)患者的术后并发症及处理经验。方法 2014年1月至2015年1月我科采用内窥镜采集大隐静脉做为CABG血管材料患者352例。手术方式非体外循环下冠状动脉旁路移植手术317例,体外循环下冠状动脉旁路移植手术35例,其中单纯冠状动脉旁路移植手术3例,同期行二尖瓣成形18例,二尖瓣置换6例,主动脉瓣置换6例,室壁瘤切除、左室成形2例。结果 术后并发症包括：心房颤动18例,低心排出量综合征10例,围术期心肌梗死6例,低氧血症6例,精神异常3例,皮下隧道内血肿、下肢肿胀疼痛3例,皮肤灼伤2例。术后死亡4例 (1.13%),原因为低心排出综合征合并多脏器功能衰竭2例、心室颤动1例,大面积脑梗死l例。结论 加强术后监护及治疗,及时纠正低心输出量综合征,积极防治术后心律失常等均为提高手术成功率的关键因素。应用内窥镜采集大隐静脉行冠状动脉旁路移植术患者术后并发症发生率低,其作为一种微创取血管的方法,是安全和值得推荐的大隐静脉采集方法。     

Electrochemical Recovery and Behaviors of Rare Earth (La,Ce, Pr,Nd, Sm,Eu, Gd,Tb, Dy,Ho, Er,Tm, and Yb) Ions on Ni Sheets

The electrochemical behaviors of rare earth (RE) ions have extensively been studied because of their high potential applications to the reprocessing of used nuclear fuels and RE-containing materials. In the present study, we fully investigated the electrochemical behaviors of RE(III) (La, Ce, Pr, Nd, Sm, Eu, Gd, Tb, Dy, Ho, Er, Tm, and Yb) ions over a Ni sheet electrode in 0.1 M NaClO₄ electrolyte solution by cyclic voltammetry between +0.5 and −1.5 V (vs. Ag/AgCl). Amperometry electrodeposition experiments were performed between −1.2 and −0.9 V to recover RE elements over the Ni sheet. The successfully RE-recovered Ni sheets were fully characterized by scanning electron microscopy, energy dispersive X-ray spectroscopy, Fourier transform infrared spectroscopy, X-ray photoelectron spectroscopy, and photoluminescence spectroscopy. The newly reported recovery data for RE(III) ions over a metal electrode provide valuable information on the development of the treatment methods of RE elements.     

New antiarrhythmic agents: amiodarone, aprindine, disopyramide, ethmozin, mexiletine, tocainide, verapamil.

The present status, clinical experience, side effects, clinical pharmacology and electrophysiologic actions of seven new antiarrhythmic agents are reviewed. The drugs selected for comment are amiodarone, aprindine, disopyramide, ethmozin, mexiletine, tocainide and verapamil. Each drug has been shown to have clinical efficacy in suppressing cardiac arrhythmias.

病例:男,82岁,因"咽痛、咳嗽2周"入院。患者2周前无明显诱因出现咽痛,咳嗽,少量白痰,无发热,无呼吸困难及其他伴随症状,门诊以"上呼吸道感染"给予服用复方大青叶合剂,效果不佳。胸部CT示左上肺陈旧性结核、右中肺慢性炎症,以"支气管炎"收入院。既往有高血压、慢性胃肠炎病史,服用冷食后会出现腹胀。体格检查:体温36.2℃,脉

Pregnancy loss, polycystic ovary syndrome, thrombophilia, hypofibrinolysis, enoxaparin, metformin.

Thrombophilia, hypofibrinolysis, and polycystic ovary syndrome (PCOS) are associated with recurrent pregnancy loss (RPL) and spontaneous abortion (SAB) alone and concurrently. The efficacy and safety of combined enoxaparin-metformin was prospectively assessed in women with PCOS with one or more previous SAB, thrombophilia, and/or hypofibrinolysis. Twenty-four white women with PCOS were studied; 23 with previous pregnancies, seven with RPL of unknown etiology (>/=three consecutive pregnancy losses <20 weeks' gestation), two with two consecutive SABs, 13 with one SAB, and one with one live birth (HELLP syndrome). Prospectively, metformin (1.5 to 2.55 g/day) was administered before and throughout gestation, with concurrent enoxaparin (60 mg/day) throughout gestation. The 24 cases differed from 93 normal white female controls for the factor V Leiden mutation, 17% vs. 2%, Fisher's p [p(f)] = .016, and for the 4G4G mutation of the plasminogen activator inhibitor-1 (PAI-1) gene (46% vs. 24%, Chi-square 4.63, p =. 031). The patients also differed from 44 normal white female controls for high levels (> 21.1 U/mL) of the PAI-1 gene product, plasminogen activator inhibitor activity (PAI-Fx) (33% vs. 8%, p(f) =. 018), and for high factor VIII (>150%) (22% vs. 0%, p(f) = .037). Of the 24 women, 23 had 65 previous pregnancies without metformin or enoxaparin, with 18 live births, 46 SAB (71%), and one elective abortion. On metforminenoxaparin, the same 23 women had 26 current pregnancies (28 fetuses), with 20 live births, two normal pregnancies 13 weeks or longer, and six SAB (21%), 3.4-fold lower than previous gestations (McNemar's S = 33.6, p <. 0001). There were no adverse maternal or fetal therapy effects. Enoxaparin-metformin reduces pregnancy loss in women with PCOS with one or more previous SAB, who also have thrombophilia and/or hypofibrinolysis.
