IN TWO MINDS ABOUT COGNITIVE-BEHAVIOURAL THERAPY: TALKING TO PATIENTS ABOUT WHY THEY DO NOT DO THEIR HOMEWORK

ABSTRACT In this paper I use three case studies to describe my attempts to understand and discuss with patients the difficulties that they often experience when trying to carry out tasks that have arisen from a collaborative relationship between us in cognitive-behavioural therapy. The difficulties seem to appear because of a conflict between two quite different views of the predicted outcome of collaboration. The conflict can occur both within patient and therapist as well as between them. I suggest that it can be understood by use of the concept of internal cohabitation (Sinason 1993; Richards 1993, 1994, 1995), the premise of which is that each human body contains two autonomous minds. One mind has the capacity to collaborate and develop, while the other believes only ill can come from such a venture.     

'IT IS TORAH AND I HAVE TO LEARN': TALKING TURKEY ABOUT THE BODY

ABSTRACT Both rabbis and therapists are on the receiving end of powerful projections, particularly the availability of their bodies as an object of phantasy. Both sets of professionals are seen as a potential source of healing and judgement, and both are viewed as having access to some hard-to-name yet soughtafter power (God/the unconscious) – yet both are essentially storytellers. The analyst's stories about countertransference can illuminate the emotional communication between two people, but may perhaps need to retain some humility in the face of the mystery that we are so much more than our means to know give us to know.     

TALKING TO YOUNG PEOPLE ABOUT SEX IN SOUTH AFRICA: NEIGHBORHOOD AND SOCIAL INFLUENCES

Our study seeks to untangle systemic influences on specific family dynamics understudied in a non‐Western context. Specifically, we examine the associations between caregivers’ perceptions of neighborhood cohesion, neighborhood safety, and social support, and one aspect of parenting relevant to the South African community from which data were drawn, caregiver–youth communication about sex. Multivariate regression models demonstrate interactive relationships among our variables of interest (p ≤ .05). For caregiver–youth dyads (N = 99), living in a neighborhood caregivers perceived to be relatively cohesive or safe was associated with differential relationships (i.e., conditional effects; p ≤ .07) between caregiver social support and youth report of communication about sex. Our findings indicate that neighborhood and social‐level influences on parenting are not universal. Moreover, cross‐cultural adaptations of family‐based behavioral interventions to prevent HIV should consider the role of caregiver social support within diverse neighborhood contexts.     

EXPLORING THE CONTOURS OF RELIGION AND SPIRITUALITY IN CREATING COMMUNITY: A FOCUS ON PERSONS WITH PSYCHOSIS

For persons living with psychosis, religious and spiritual spaces are a readily available resource that can support recovery and community inclusion. However, these resources are often underutilized in treatment planning. This in‐depth qualitative analysis explored the construction of community through faith spaces, religion, and spirituality for persons with psychosis. An ethnically and racially diverse group of 31 people with psychosis were interviewed at 3 time points over 8 months. Key supports and community members also were interviewed. A grounded theory analytic strategy revealed 3 themes: (a) creating community and increasing resilience through religion and spirituality, (b) barriers to engaging in religion and spirituality, and (c) facilitating inclusion in faith spaces. Faith spaces provided an environment in which the whole person was embraced, enabling a more coherent sense of self. These findings have implications for enhancing social capital and spiritual capital among persons with psychosis.     

TEACHING AND LEARNING ABOUT PSYCHOANALYSIS: FILM AS A TEACHING TOOL, WITH REFERENCE TO A PARTICULAR FILM, MORVERN CALLAR

Using the film, Morvern Callar , directed by Lynne Ramsay (2002a ) in order to reflect on the impact of the countertransference, the author suggests that film is a powerful medium which can be used to communicate intense emotional and preverbal states, and to make sense of certain key psychoanalytic concepts via a direct emotional experience. Describing the film's narrative, with particular reference to the central character, she links this with predominantly Kleinian psychoanalytic thinking about schizoid states, and puts forward the idea that, by watching the film in a particular way, non-clinical students may have the proto-experience of aspects of clinical work, especially those linked with the countertransference. This, she suggests, offers a bridge towards clinical experience possibly obtained at a later date, where emotional states may be projected into the clinician.

HEALTHY LEADERS: MULTILEVEL HEALTH PROMOTION CONSIDERATIONS FOR DIVERSE UNITED METHODIST CHURCH PASTORS

Community psychologists often work with institutions and leaders, such as clergy, to bring about social change. Studies finding high rates of chronic disease among clergy have called for the design of clergy health interventions. However, among clergy there is substantial diversity. We conducted four focus groups with a cross‐section of United Methodist clergy and one focus group each with female, local, young, and large‐sized church pastors. We compared themes from the specific versus broader focus groups. Findings are as follows: female pastors felt guilty for taking personal time and experienced pressure to prove themselves; local pastors reported financial strain and utilized a variety of interpersonal relationships; young pastors indicated child‐related stress but also greater interest in nutrition, exercise, and church‐based health promotion; and large‐sized church pastors expressed increased confidence in negotiating personal time and reported more sharing of pastoral duties. We organized themes by levels of the socioecological framework to guide intervention design.
