人附睾上皮分泌蛋白4在卵巢癌诊治中的应用价值   总被引:6,自引:0,他引:6  
目的 评估人附睾上皮分泌蛋白4(HF4)在卵巢癌诊断与疗效监测中的应用价值.方法 用ELISA检测82例卵巢癌患者、26例卵巢良性疾病患者及40名健康对照者血清HF4水平,同时测定糖类抗原125(CA125),对结果进行比较分析.结果 在未绝经组中,卵巢癌患者血清中HE4水平(中位数为46.6 pmol/L)明显高于卵巢良性疾病患者(中位数为31.5 pmol/L)和健康对照(中位数为24.0 pmol/L,U值分别为95.5和100.0,P均<0.01),同样在绝经组,卵巢癌患者HE4水平(中位数为139.0 pmol/L)也明显高于良性疾病患者(中位数为35.4 pmol/L)和健康埘照(中位数为27.7 pmol/L,U值分别为25.0和49.0,P均<0.01).根据受试者操作特性曲线(ROC曲线)分析,未绝经者HE4榆测卵巢癌的临界值为35.7 pmol/L,敏感度为71.4%,特异度为80.O%;绝经者HE4检测卵巢癌的临界值为54.5 pmol/L,敏感度为87.O%,特异度为92.9%;HE4诊断价值稍高于CA125.对于卵巢疾病患者,未绝经者和绝经者卵巢癌发病风险计算(ROMA)值分别大于3.5%和12.5%时,可能存在卵巢癌发病的高风险.此外,卵巢癌患者血清HFA水平及阳性率随着肿瘤分期的升高而升高;淋巴结转移患者HFA水平显著高于未转移者(U值分别为34.0和77.0,P值均<0.01).HE4水平与疗效也密切相关,临床治疗有效时HE4下降,而病情恶化或出现转移时则升高.结论 HE4测定有助于卵巢癌的诊断和疗效临测.  相似文献

目的 建立本实验室电化学发光法检测血清骨钙素(osteocalein,OCN)、Ⅰ型胶原羧基端肽β特殊序列(β-crossLaps,β-CTX)和总Ⅰ型前胶原氨基端肽(total procollagen type 1 amino-terminal propeptide,tP1NP)的参考区间.方法 根据临床和实验室标准化协会(CLSI)C48-A文件要求筛选出合适人群,按性别和绝经期前后分为男性、绝经前女性、绝经后女性3组.收集窄腹血清,使用Roche Modular E170电化学全自动免疫分析仪检测OCN、β-CTX、tP1NP.结果 393例合适人群中男性112名(年龄29~69岁)、绝经前女性148名(年龄28~54岁)、绝经后女性133名(年龄44~68岁),各组血清OCN、β-CTx、tP1NP的检测结果为男性[(15.33±4.76)μg/L、(413±189)ng/L、(42.15±17.14)μg/L];绝经前女性[(12.99±4.53)μg/L、265(30~820)ng/L、(36.43±14.23)μg/L];绝经后女性[(18.96±5.15)μg/L、(513±195)ns/L、51.40(8.98~118.6)μg/L].除血清β-CTx(绝经前女性)和tP1NP(绝经后女性)各组数据经Log转换均为正态分布.各组的95%参考区间分别为:男性(6.00~24.66μg/L、43~783 ns/L、9.06~76.24μg/L);绝经前女性(4.11~21.87μg/L、68~680 ns/L、8.53~64.32μg/L);绝经后女性(8.87~29.05μg/L、131~900 ns/L、21.32~112.8μg/L).结论 本实验室建立的3项血清骨转换标志物的参考区间与厂商提供的有差异,各实验室在引用时应加以注意.  相似文献

Objective To establish the reference intervals of serum osteocalcin (OCN), C-terminal cross-linking telopeptide of type Ⅰ collagen (β-CTx) and total type Ⅰ procollagen N-terminal peptide (P1NP) by electrochemiluminescence assay. Methods According to the Clinical and Laboratory Standards Institute (CLSI) "CA8-A" document the appropriately healthy people, who were divided into three groups (men, premenopausal women, and postmenopausal women) by sex and pre- or postmenopausal status were screened. The levels of fasting serum of OCN,β-CTx, tPINP were detected by Roche Modular E170 electrochemical immunoassay. Results 393 appropriately healthy people consists of 112 men between the ages of 29 and 69 years, 148 premenopausal women between the ages of 29 and 69 years, 133 postmenopausal women between the ages of 29 and 69 years. The levels of serum OCN, β-CTx, tP1NP in men group were (15.33±4.76) μg/L, (413±189) ng/L, (42.15±17.14) μg/L, respectively. The levels of serum OCN, β-CTx, tP1NP in premenopausal women group were (12.99±4.53) μg/L, 265(30-820) ng/L, (36.43±14.23) μg/L, respectively. The levels of serum OCN, β-CTx, tP1NP in postmenopausal women group were (18.96±5.15) μg/L, (513±195) ng/L, 51.40 (8.98 -118.6)μg/L, respectively. Logarithmic transformation produced normal distributions for all markers but serum β-CTx of premenopausal women group and serum tPINP of postmenopausal women group. The 95% of the distribution intervals for serum OCN, β-CTx, tP1NP in men group was 6.00-24.66 μg/L, 43-783 ng/L, 9.06-76.24 μg/L, respectively. The 95% of the distribution intervals for serum OCN, β-CTx, tP1NP in premenopausal women group was 4.11-21.87 μg/L, 68-680 ng/L, 8.53-64.32 μg/L respectively. The 95% of the distribution intervals for serum OCN, β-CTx, tPl NP in postmenopausal women group were 8.87-29.05 βg/L, 131-900 ng/L, 21.32-112. 80 μg/L, respectively. Conclusions Compared with the reference intervals provided by manufacture, the reference intervals of three serum bone turnover markers established by our laboratory have great difference. Laboratory should pay attention to the reference intervals was cited.  相似文献

目的 建立北京和上海地区表面健康人群血清三碘甲状腺原氨酸(TT3)、甲状腺素(TT4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)的参考区间.方法 按照美国国家临床生化学院检验医学应用准则中关于甲状腺疾病诊断与监测的实验室支持中的筛选要求,在北京、上海地区筛选体格检查和相关实验室检查均正常的表面健康人群390名(男221名,女169名),年龄18 ~ 65岁.采集受检者空腹血清,使用美国UniCel DXI 800全自动化学发光分析仪进行TT3、TT4、FT3、FT4、TSH的检测.不同性别、地区间上述各种甲状腺激素水平的比较采用两独立样本t检验,不同年龄组间的比较采用方差分析,两两比较采用LSD法.参考区间采用P2.5~P97.5表示.结果 对各种激素水平进行性别间比较,男性组TT3、TT4、FT3、FT4、TSH水平分别为(1.90±0.32) nmol/L、( 116.77±18.02) nmol/L、(5.28±0.67) pmoL/L、(11.54±1.97) pmol/L、(1.92±1.12) mIU/L,女性组分别为(1.82±0.32) nmo]/L、( 115.73±14.39) nmol/L、( 5.04±0.59) pmol/L、( 10.94±1.45) pmol/L、(2.37±1.86) mIU/L,TT3、FT3、FT4、TSH差异均有统计学意义(t分别为2.377、3.642、3.471、2.520,P均<0.05).不同地区间结果比较,只有FT3在地区间差异有统计学意义,北京和上海分别为(5.01±0.63)、(5.41±0.61) pmol/L(t =6.410,P<0.01),其他指标差异均无统计学意义.相关分析结果表明,TT4与年龄呈正相关(r=0.22,P<0.01);TSH与年龄呈负相关(r=-0.12,P<0.05).TT3、TT4、FT3、FT4、TSH参考区间分别为(1.22 ~2.50) nmol/L、(83.37~149.37) nmol/L、(3.88 ~6.48) pmoL/L、(7.70~14.86) pmol/L、(0.38 ~5.58) mIU/L.结论 北京和上海地区人群血清甲状腺激素水平与进口试剂盒给定的参考区间有差异,建立我国甲状腺激素参考区间具有重要意义.  相似文献

目的探讨血清胃蛋白酶原PGⅠ、PGⅡ、PGR(PGⅠ/PGⅡ)值的健康人群参考范围。方法用酶联免疫吸附试验(ELISA)检测58例健康人血清PGⅠ、PGⅡ含量,并计算PGR值。结果健康人群血清PGⅠ、PGⅡ含量呈正态分布,PGⅠ95%置信区间为82.675~115.339μg/L,PGⅡ为8.043~13.582μg/L,PGⅠ/PGⅡ为10.135~13.244。结论得出的健康人群血清PGⅠ、PGⅡ及PGR值参考范围在实验室及临床工作中具有指导意义。  相似文献

目的探讨人附睾蛋白4(HE4)在肺癌患者血清中表达的临床意义。方法选取连云港市第一人民医院2012年7月~2013年3月间住院的有明确病理诊断的肺癌患者222例,其中肺鳞癌63例、肺腺癌113例、小细胞肺癌46例。正常对照组为同期在该院体检的132例健康成人,在Cobase601电化学发光免疫检测系统上检测其血清HE4水平,分析患者与正常对照组间水平差异、不同病理类型间差异,比较男、女性患者问HE4异常率差异。结果男性正常对照组血清HE4水平为77.9士23.6pmol/L,显著高于女性正常对照组的64.8±16.9pmol/L(t=3.671,P=0.000);男性患者中肺鳞癌、肺腺癌和小细胞肺癌组血清HE4水平分别为125.1±65.5pmol/L,172.8±151.1pmol/L和162.9±120.0pmol/L,显著高于正常对照组(x2=47.30,P=0.000);女性患者中肺鳞癌、肺腺癌和小细胞肺癌组血清HE4水平分别为124.5±98.2pmol/L,221.1土433.2pmol/L和116.4士40.2pmol/L,显著高于正常对照组(x2=43.8,P=0.000),不同病理类型异常率间差异无统计学意义(P〉0.05)。结论HE4在肺癌的临床诊断中是有意义的标志物,在男女性问的表达水平有差异。  相似文献

【目的】探讨子宫内膜癌患者血清人附睾分泌蛋白4(H E4)水平及其临床意义。【方法】采用电化学发光法法对子宫内膜癌患者(A组)、子宫内膜增生患者(B组)及正常健康妇女(C组)各60例进行血清中HE4的含量测定,分析子宫内膜癌临床FIGO分期与血清HE4水平的关系,并对比A组治疗有效者治疗前后H E4水平。【结果】A组血清H E4显著高于B组和C组,且差异有显著性( P<0.01)。A组患者术前血清HE4浓度随子宫内膜癌的临床FIGO分期提高而提高,各分期之间比较差异有统计学意义( P <0.01)。A组 H E4水平明显低于治疗前,且差异有显著性( P <0.01)。【结论】血清 H E4水平可作为子宫内膜癌辅助诊断、判断恶性程度及是否转移、观察疗效的有效指标。  相似文献

[目的]分析经阴道彩色多普勒超声(TV-CDS)联合血清人附睾蛋白(HE4)检测对子宫内膜癌的诊断价值.[方法]回顾性分析2014年3月至2017年3月期间在本院因阴道不规则出血进行检查的206例患者的临床资料,将患者按病理检查结果分为子宫内膜癌组和良性病变组.所有患者采用TV-CDS观察全子宫形态,测量子宫内膜厚度、子宫内膜血流,计算子宫内膜体积;采用酶联免疫吸附试验(Elisa法)检测血清HE4水平.[结果]子宫内膜癌组子宫最大径、内膜厚度及内膜体积均大于子宫良性病变组,差异具有统计学意义(P<0.05);子宫内膜癌组血清hE4水平高于良性病变组,差异具有统计学意义(P<0.05).TV-CDS联合血清HE4检查对子宫内膜癌及子宫良性病变的诊断正确率分别为94.02%和92.13%,高于单用两种方法,漏诊率及误诊率分别为5.98%和7.87%,低于单用两种方法,之间差异均具有统计学意义(P<0.05).TV-CDS联合血清HE4诊断子宫内膜癌的敏感度为94.02%、特异度为94.38%、阳性预测值为95.65%、阴性预测值为92.31%,约登指数为0.884,均远超TV-CDS及血清HE4检测,差异具有统计学意义(P=0.006).[讨论]TV-CDS联合血清hE4检测较单用两种检测手段检测能够显著提高对子宫内膜癌的诊断效能,对指导临床有重要意义.  相似文献

目的探讨8828名健康人群血清甲胎蛋白(α-fetoprotein,AFP)和癌胚抗原(Carcinoembryonic antigen,CEA)水平分析,以期为AFP、CEA的生物参考区间的确定提供重要参数。方法选择2011年3月至2011年9月,来北京同仁医院体检中心健康体检且体检结果正常人群,共8828例。分别对男性组及女性组AFP和CEA指标进行对比及统计学参数分析。结果男性组血清AFP和CEA中位浓度均明显高于女性组(均P<0.001),且AFP和CEA含量与年龄有明显的相关性。本研究总体AFP参考区间为0-5.9ng/ml,总体CEA参考区间为0-3.4ng/ml。结论 AFP和CEA在不同性别和不同年龄组间有明显差异,依据临床需求,建立合理的AFP和CEA参考区间有重要临床意义。  相似文献

目的:探讨重庆地区不同年龄段健康体检女性血清人附睾蛋白4(H E4)的水平分布,为验证本地区的女性血清H E4水平参考区间提供参考。方法选取2013年1~12月在重庆某三甲医院进行健康体检的健康女性853名,分为6组:20~29岁组、30~39岁组、40~49岁组、50~59岁组、60~69岁组、≥70岁组,检测、比较各年龄组H E4水平。结果20~29岁组、30~39岁组、40~49岁组、50~59岁组间H E4水平比较,差异无统计学意义( P>0.05);将她们合并为<60岁组后,与60~69岁组及≥70岁组比较,差异均有统计学意义( P<0.05);以 x ±2s为参考上限,参考区间分别为:60岁以下 HE4<60.83 pmol/L ,60~69岁 HE4<84.51 pmol/L ,70岁及以上HE4<93.79 pmol/L。结论重庆地区女性血清 HE4水平受年龄因素影响,建议按照60岁以下、60~69岁、70岁及以上分别建立女性血清 H E4水平的参考区间。  相似文献

Objective To establish the reference range of venous blood cells of healthy aged people by means of investigation on the reference range of venous blood cells of healthy adults and the aged.Methods The parameters of 3302 venous blood samples from the healthy adults and the aged were analyzed by applying SymexKX-21N hematology analyzer,and the test results were statistically analyzed with SPSS10.0 software.Results The reference range of the healthy aged people(males and females):WBC(3.57-9.00)×109/L and(3.52-8.39)×109/L;RBC(3.78-5.42)×1012/L and (3.44-4.83)×1012/L;hemoglobin(Hb)121.40-168.10 and 107.90-147.90 g/L;hematocrit(HCT)36.62%-49.18% and 32.82%-44.03%;mean corpuscular volume(MCV)85.10-100.60 and 84.70-100.60 fL;mean corpuscular hemoglobin(MCH)28.54-34.48 and 28.20-34.10 pg;mean corpuscular hemoglobin concentration(MCHC)322.00-357.00 and 320.00-352.00 pg/L;coefficient of variation of red blood cell distribution width(RDW-CV)12.13%-15.10% and 10.20%-17.12%;platelet(PLT)(70.00-240.00)×109/L and(77.00-262.00)×109/L;mean platelet volume(MPV)9.77-15.37 and 9.51-15.55 fL;platelet distribution width(PDW)11.44%-27.77%;platelet hematocrit(PCT)1.10-2.73 and 1.13-2.99 mL/L;large platelet ratio(P-LCR)24.35%-70.14%.The differences of the parameters(WBC,RBC,Hb,HCT,MCHC,PLT and PCT)were statistically significant between senile males and senile females(P<0.01),while the other parameters weren't(P>0.05);the differences of parameters(WBC,MPV,PDW and P-LCR)were not significant between adults and the ageds(P>0.05),while the other parameters were(P<0.05).There were also significant differences(P<0.05)between the test results and the reported date about the blood cell parameters of adults and the ageds.Conclusion The reference range of the healthy ageds is different from that of adults;therefore it is necessary to establish the laboratory reference range of venous blood cells of the elderly in this region scientifically and rationally.  相似文献
