目的:研究早期怀疑脑干腔梗(或小片状梗死)患者的脑干听觉诱发电位(BAEP)变化。方法:对20例临床以椎一基底动脉缺血为主患者进行BAEP检查并以20例正常人做对照。结果:早期怀疑脑干腔梗(或小片状梗死)患者中,13例BAEP异常(占65%),其中6例经CT证实为脑干小片状梗死,6例经MRI证实为脑干腔梗,这些腔梗和小片状梗死多涉及桥脑被盖侧方,1例MRI未见异常,7例BAEP均属正常范围(占35  相似文献

目的 探讨脑干听觉诱发电位(BAEP)在血管性痴呆患者应用价值.方法 选择200例血管性痴呆患者进行脑干听觉诱发电位(BAEP)检测分析.结果 脑干听觉诱发电位显示研究组III、V波潜伏期(PL)和I~III、III~V、I~V波峰间潜伏期(IPL)及(III~V)/(I~III)比值均高于对照组,差异有统计学意义(P<0.01).结论 脑干听觉诱发电位可客观的反映听觉系统和脑干功能状况,预防疾病进一步发展,对血管性痴呆的临床诊疗及预后均有价值.  相似文献

脑干听觉诱发电位在小脑卒中应用的价值探讨   总被引:1,自引:0,他引:1  
对28例小脑卒中患者进行脑干听觉诱发电位(Brainsternauditoryevokedpotential,BAEP)检测研究,其中小脑出血19例,小脑梗塞9例,结果发现:BAEP在小脑卒中的异常为85.71%,且主要发生在病灶侧,部分病例可累及双侧。主要以Ⅲ,Ⅴ波PL及I~Ⅲ,I~ⅤIPL延长,BAEP在出血及梗塞的定性上无参考价值,随着病情的好转,BEAP亦逐渐恢复正常。  相似文献

目的探讨脑干听觉诱发电位(BAEP)在不典型脑干梗死患者中的特点及临床价值。方法使用肌电图诱发电位仪对86例不典型脑干梗死患者进行BAEP检查且与其MRI、CT结果比较。结果 86例不典型脑干梗死患者的BA-EP42例异常,异常率48.8%。MRI、CT无1例异常(P<0.01)。结论 BAEP能够间接评估不典型脑干梗死患者的供血情况,给临床诊断与治疗提供客观依据。  相似文献

目的探讨痉挛性斜颈患者脑干听觉诱发电位的功能变化。方法比较分析30例痉挛性斜颈患者及30例正常对照患者(颈部向右侧扭转)脑干听觉诱发电位双侧Ⅰ/Ⅴ波高比及Ⅰ~Ⅲ、Ⅲ~Ⅴ波间期的差异。结果病例组脑干听觉诱发电位双侧的Ⅰ~Ⅲ、Ⅲ~Ⅴ波间期正常,斜颈同侧的Ⅰ~Ⅲ波间期较对侧延长,差异有统计学意义(P<0.05);双侧Ⅲ~Ⅴ波间期及Ⅰ/Ⅴ波高比比较差异无统计学意义(P>0.05)。正常对照组颈部向右侧扭转时双侧比较Ⅰ~Ⅲ波间期、Ⅲ~Ⅴ波间期、Ⅰ/Ⅴ波高比比较差异无统计学意义(P>0.05)。结论脑干听觉诱发电位双侧Ⅰ~Ⅲ、Ⅲ~Ⅴ波间期正常提示脑干上部传导通路结构正常,斜颈扭转方向的同侧Ⅰ~Ⅲ波间期较对侧延长提示同侧的传导通路功能异常。  相似文献

目的使用诱发电位仪检测后循环短暂性脑缺血发作(transient ischemic attack,TIA)患者脑干听觉诱发电位(Brainstem auditory evoked potential,BAEP)的变化,探讨合并糖尿病(DM)的后循环TIA患者受损部位的特点。方法入组后循环TIA病例共58例,其中合并糖尿病组20例,无糖尿病组38例,使用诱发电位仪分别检测其BAEP的变化。结果2组后循环TIA患者的Ⅲ波、Ⅴ波波峰潜伏期(Peaklatency,PL),Ⅰ~Ⅲ波、Ⅲ~Ⅴ波峰间潜伏期(Interpeak latency,IPL)均较正常值延长,其中合并DM组Ⅲ波PL、Ⅰ~Ⅲ波IPL与无DM组的Ⅲ波PL、Ⅰ~Ⅲ波IPL比较有显著性差异(P〈0.05),而无DM组的Ⅲ-Ⅴ波IPL与合并DM组的Ⅲ~Ⅴ波IPL比较有显著性差异(P〈0.05)。结论合并DM的后循环TIA患者听神经及脑桥下段较无DM的TIA患者更容易受到缺血性损伤。  相似文献

脑干听觉诱发电位监护在脑干肿瘤手术中的应用研究   总被引:4,自引:1,他引:3  
目的研究脑干及相应颅神经功能损伤时较敏感的电生理指标,为脑干肿瘤手术提供精确、准确和安全的术中监测手段。方法对18例脑干肿瘤病人,用同一进口监护仪于手术前、术中及术后分别进行脑干听觉诱发电位(BAEP)连续实时监护,测定手术操作对这些指标的影响。结果脑干肿瘤手术操作均可以引起BAEP改变,BAEP的Ⅰ、Ⅲ、Ⅴ波潜伏期(PL)及Ⅰ~Ⅴ、Ⅲ~Ⅴ峰间潜伏期(IPL)明显延长(P<0.01),Ⅴ波波幅明显降低(P<0.01),其中BAEP的Ⅴ波潜伏期及波幅改变最为显著。结论BAEP的Ⅴ波潜伏期延长和波幅下降是术中敏感的电生理指标,对其进行监护可为手术中及手术后避免神经功能损伤提供客观指标,降低手术伤残率,减少或避免病人手术后神经功能损伤。  相似文献

建立犬脑干局灶性缺血模型,观察脑干局灶性缺血后脑干听觉诱发电位(BAEP)的变化。结果发现犬脑干缺血0.5、1、1.5、3、6和12h缺血组(IS)BAEP的Ⅰ、Ⅱ、Ⅱ、Ⅴ各波潜伏期(PL)及Ⅰ-Ⅲ、Ⅲ-Ⅴ、Ⅰ-Ⅴ波的峰间期(IPL)与假手术组(SH)相比较均非常明显地延长(P〈0.01),以1-1.5h延长最显著(P〈0.01),3h后各植逐渐缩,提示BAEP可反映犬脑干缺血后的脑干损害,可客评  相似文献

目的 探讨脑干听觉诱发电位(BAEP)在偏头痛中的应用.方法 使用脑干反应测听仪(NGT-Ⅱ型)对180例偏头痛患者进行BAEP测定,其中发作期49例,间歇期131例,并与256例正常人的BAEP对照.结果 偏头痛患者中BAEP的异常者为95例,占52.8%,异常率明显高于对照组(P=0.001).发作期检查者49例,其中40例异常,占81.6%,异常率明显高于间歇期.结论 偏头痛的BAEP的异常率明显增高,尤其在发作期,BAEP可作为偏头痛的辅助检查之一.  相似文献

目的:探讨脑干听觉诱发电位(BAEP)对前庭系统性眩晕的定位价值。方法:对138例临床确诊的前庭系统性眩晕患者进行了BAEP的检测。结果:138例前庭系统性眩晕患者行BAEP检查有98例异常,异常率为71%,其中内耳型13/98例(13%),表现为Ⅰ、Ⅱ波各项指标的改变;脑干型48/98例(49%),表现为Ⅰ、Ⅱ波正常,Ⅲ、Ⅳ、Ⅴ波各项指标存在异常;混合型37/98例(38%),各波指标均出现改变。结论:BAEP有助于鉴别前庭周围性眩晕与前庭中枢性眩晕,在前庭系统性眩晕的临床诊断中有定位价值。  相似文献

Brainstem auditory evoked potentials (BAEPs) have been investigated in 34 patients affected by Multiple Sclerosis. Abnormalities were found in 68% of the patients. Silent lesions of the brainstem were detected in 60% of the clinically definite and in 44% of the probable cases. The diagnostic value of these findings is discussed.
Sommario I potenziali evocati uditivi sono stati investigati in 34 pazienti affetti da sclerosi multipla. Anormalità sono state evidenziate nel 68% dei pazienti affetti da sclerosi multipla clinicamente definita. Lesioni silenti del brainstem sono state trovate nel 60% delle forme clinicamente definite e nel 44% di quelle probabili. Il valore diagnostico di questi reperti è discusso.

Abstract: Brainstem auditory evoked potentials (BAEPs) and somatosensory evoked potentials after median nerve stimulation (MN-SEPs) and after posterior tibial nerve stimulation (PTN-SEPs) were studied in 17 patients with neurolehget's syndrome (NB). Eleven patients (64.7%) showed an absence of wave I, III or V or a prolongation of the interpeak latency 1–111, or 111-V in BAEPs. Six patients (37.4%) showed a prolongation in the latency of cortical P37 of PTN-SEPs and/, or the interpeak latency EP-N13 or N13–N18 of MN-SEPs. The BAEP and SEP abnormalities indicated a conduction failure of the acoustic lateral lemniscus pathway and the medial lemniscus pathway in the brainstem of the patients with NB. Abnormal EPs can provide sensitive information which shows the presence of subclinical lesions in the central nervous system.  相似文献

BACKGROUND: Brainstem auditory evoked potentials (BAEP) reflect the activation of brainstem nuclei in the first milliseconds after presentation of an auditory stimulus. These electrophysiological correlates of neural processing are highly automatic and not influenced by cognitive factors or task demands; however, data from patients with anxiety disorders suggest deviations in the BAEP. It has been hypothesized that these differences reflect heightened activation of structures involved in defensive states, such as the amygdala and locus coeruleus, projecting to the inferior colliculus, one of the brainstem generators of wave V of the BAEP. The present study investigated this possibility by testing BAEP during experimentally induced anxiety in healthy volunteers. METHODS: In this study, BAEP were recorded from healthy normal volunteers under threat of shock, compared with safe conditions. RESULTS: The first experiment (n = 12) showed that shock anticipation increased the amplitude of wave V. A replication experiment (n = 13) confirmed this finding. CONCLUSIONS: Although BAEP are highly robust with respect to attentional manipulations, they are affected by transient activation of the fear system due to threat of shock. This finding indicates that some of the electrophysiological brainstem abnormalities observed in anxiety disorders can be replicated in healthy control subjects by inducing a transient state of anxiety.  相似文献

目的:探讨瞬目反射及脑干听觉诱发电位(BAEP)对糖尿病患者颅神经亚临床损害的诊断价值。方法:对150例糖尿病患者及50名健康志愿者进行双侧瞬目反射和BAEP检查。结果:瞬目反射和BAEP均检查200人、400侧次,两项检查在糖尿病患者中总的阳性率为31.3%,远高于临床症状、体征的阳性率(6%)。瞬目反射中R2、R2′潜伏期和R1-R2波间期延长最为明显,BAEP的异常则以V波潜伏期延长最常见。结论:瞬目反射与BAEP检查能够可靠的检测糖尿病患者颅神经和脑干受损的早期及亚临床损害。  相似文献

Brainstem function in patients with Chiari-II malformation was evaluated by brainstem auditory-evoked potentials (BAEPs). All cases had associated hydrocephalus, some of which were shunted. The effect of the shunting operation on the wave forms was examined as well as the age-wave relationships. Although the shunting operation resulted in shortened brainstem conduction time, the overall postshunt responses were still far from normal. Another significant finding was that whereas there were remarkable abnormalities in recorded wave-forms in cases younger than 8 years, normal or almost normal responses were obtained in all cases older than 8. BAEP abnormalities could not be correlated with the severity of meningomyelocele, nor was the predictive value of response in assessing potential risk of symptomatic Chiari malformation established.This paper was presented at the 15th Annual Meeting of the International Society for Pediatric Neurosurgery, New York, July 12–16, 1987  相似文献

目的 利用视觉(VEP)和听觉诱发电位(AEP)技术初步检出焦虑症和找出评价森田疗法绝对卧床期客观疗效的指标.方法 应用视觉和听觉技术.观察焦虑症新森田疗法组绝对卧床期前后VEP和AEP指标的变化特点.结果 (1)VEP潜伏期N1、P2上,绝对卧床期结束后较卧床前分别为前移和延迟,分别为18 ms和19 ms.(2)AEP波幅P2绝对卧床期结束后较卧床前升高3.5 μV.结论 焦虑症新森田疗法组绝对卧床期前后VEP和AEP变化值得进一步观测.  相似文献

Summary The results are reported of serial brainstem auditory evoked potentials recordings in 51 patients with decerebration and bulbar syndrome. In contrast to the stability of latencies of single components of the potential in healthy subjects, patients with decerebration syndromes show considerable instability and an increase in the latency of all the components of the potential. In 34 decerebrate patients the P-I latency and the interpeak latencies for the medullo-pontine and ponto-mesencephalic segments as well as the central conduction time were significantly increased. There was marked reduction of the amplitude of P-V and P-III and deformation of the single components of the potential with widening and smoothing. The amplitude ratios A-V to A-I and A-III to A-I were significantly decreased. The findings are interpreted as due to mesencephalic and pontine functional disturbance during decerebration. The brainstem auditory evoked potential can be used to estimate the time of brain death.
