A ruptured aneurysm associated with pituitary tumor (author's transl)

The aneurysm case associated with brain tumor is not so rare. Most of aneurysms are non-ruptured and found incidentally during the examination for brain tumor. We have experienced with 4 aneurysm cases associated with brain tumor among 1,280 aneurysm cases during these 17 years. One out of 4 cases was of ruptured aneurysm associated with pituitary tumor. This rare case and review of the literature were described.     

Occult temporal arteritis--a case report (author's transl)

A 57-year-old female patient with ischaemic papilloedema developed vascular pseudopapillitis of the opposite eye three months later. Temporal arteritis was diagnosed by means of a biopsy of the temporal arteries. Such cases of occult temporal arteritis are characterized by the absence of typical general symptoms and pathological laboratory findings, or, alternatively, these symptoms appear only much later than the ischaemic alteration to the fundus. Hence, one should consider occult temporal arteritis when confronted with any ischaemic process of the optic nerve, and also in the case of occlusion of the central retinal artery, expecially in cases with a simultaneous rise in the blood-sedimentation rate, since high doses of steroids, when given in time, may prevent total loss of sight.     

A report about the problem of Mayer-Rokitansky-Kuester-syndrome (author's transl)

Twenty cases of Mayer-Rokitansky-Kuester-syndrome from Iran are reported with the results of corrective surgery. One pecularity of this syndrome never recorded previously is the unilaterality of the condition and endometrium in the secretory phase. We suggest that this abnormality may have some racial or sectorial incidence.     

Malformation of the tear ducts associated with clefts in the face (author's transl)

A case of malformation of the tear duct associated with clefts in the face is described. To our knowledge this combination has not been described before. The whole symptoms complex consisted of: bilateral cleft lip upper jaw and gum, choanalatresia, mucocele of the left nasal cavity from adhesions of the lower concha with the nasal septum, cystic dilatation of the tear sac at the connection to a bony naso-lacrimal duct. Right inguinal hernia, syndactyly of the first and second left toes, iris-chroid coloboma right with congenital cataract and unilateral right convergent squint. Because of the advanced dacryocystitis on the basis of the congenital dacryostenosis, and external dacryostorhinostomy (modified after Kaleff 1937) was performed after pre-treatment by instillation of antibiotic solutions in the tear ducts. At the same time the nasal cavity mucocele was opened by partial excision of the lower concha and opening the choanal atresia. After a post-operative course free of complications and with clear patency of the tear ducts the oral surgical operation was done--closure of the face clefts. The chromosome analysis was normal the only abnormal result here was an unexplained slow rate of multiplication of lymphocytes in four cultures inoculated at various points of time. Because we could not find any hereditary factors, this symptom complex is probably due to exogenous embryo damage during the early development phase.     

Cardiac resuscitation - a seven-year report (author's transl)

Over the 7-year period 1966 to 1973, cardiac resuscitation was performed in 315 cases. The reasons were circulatory arrest (ventricular fibrillation or asytole) in 228 and other life-threatening disturbances in 87 cases. During daytime, especially from 8 a.m. to 12 noon and from 4 to 8 p.m., a higher percentage of cases was successfully resuscitated and during these periods emergency personnel seemed to be more readily available. The underlying disease is one of the determinants for the prognosis, whereas age is without relevance. Over the last few years, together with a decrease in the frequency of emergency calls, the percentage of cases of successful resuscitation increased. 14.3% of all patients in whom resuscitation was undertaken were discharged from hospital.

A report is given of an unusual case of an extensively scarred fundus following chorioditis in sarcoidosis, where depigmentation was prominent and reminding us in the differential diagnosis of possible degenerative changes.     

Purpura fulminans associated with varicella and polyvalent protective inoculations (author's transl)

We report about a 10 5/12 years old boy, who developed the rare disease of Purpura fulminans following Varicella, after having received the trivalent Poliomyelitis vaccine of Sabin and a diphtheria-tetanus booster during the incubation period of Varicella. The acute stage of the disease was overcome by symptomatic therapy; extensive skin necrosis especially in the lower extremities had to be treated surgically. The etiology of Purpura fulminans has not yet been established. Since diffuse intravascular clotting (DIC) plays an essential part in the pathogenesis of this disease. Heparintherapy is now used as the treatment of choice. At the present state of knowledge we believe that the protective inoculations given in the incubation period of Varicella possibly provoced Purpura fulminans as a rare complication of Varicella.     

Bilateral intrauterine torsion of the spermatic cord (case report) (author's transl)

A case of bilateral intrauterine torsion of the spermatic cord is reported. The possibility of an intrauterin origin is given by observations in newborn infants. The head symptoms are scrotal swelling, flush and induration. Already a suspected torsion of the spermatic cord justifies immediate surgery as the germinative and interstitial cells are irreversibly damaged after very short time. The obstetrician is obliged to diagnose these pre- or perinatally originated torsions and to arrange immediate surgery.
