Incremental Pacing for the Diagnosis of Cavotricuspid Isthmus Block.   Background: Complete conduction block of the cavotricuspid isthmus (CTI) reduces atrial flutter recurrences after ablation. Incremental rapid pacing may distinguish slow conduction from complete CTI conduction block.
Methods and Results: Fifty-two patients (67 ± 9 years) undergoing 55 CTI ablation procedures were included. With ablation, double potentials (DPs) separated by an isoelectric line of ≥30 ms were obtained. Incremental atrial pacing (600–250 ms) was performed from coronary sinus (CS) and low lateral right atrium (LLRA). A <20 ms increase in the DPs distance during incremental pacing was indexed as complete CTI block. In 8 patients, an initial <20 ms DPs distance increase was noted; direct complete isthmus block was suggested and no additional ablation performed. In the remaining, the CTI line was remapped for conduction gaps and additional radiofrequency energy pulses applied. Complete block, as indexed by incremental pacing, occurred in 46 of 55 procedures, with one flutter recurrence (follow-up 8 ± 2 months): DPs interval variation of 116 ± 20 to 123 ± 20 ms (CS), P = 0.21; and 122 ± 25 to 135 ± 35 ms (LLRA), P = 0.17. The remaining 9 patients (persistent rate-dependent DPs increase) presented 3 flutter recurrences, P = 0.01: DP distance from 127 ± 15 to 161 ± 18 ms (CS), P < 0.001; and 114 ± 24 to 142 ± 10 ms (LLRA), P = 0.007.
Conclusion: Incremental pacing distinguishes complete CTI block from persistent conduction. Such identification, accompanied by additional ablation to achieve block, should minimize flutter recurrences after ablative therapy. (J Cardiovasc Electrophysiol, Vol. 21, pp. 33–39, January 2010)  相似文献

Radiofrequency ablation is an established method for treatment of type I atrial flutter. The assessment of creation of complete bidirectional isthmus block following linear ablation of the isthmus is an integral part of ablation procedure. Conventionally, bidirectional isthmus block is tested by pacing on either side of ablation line and looking for reversal of activation sequence in the right atrium. We looked at the feasibility of recording double potentials, separated by an isoelectric interval along the ablation line as an alternative method to demonstrate bidirectional isthmus block. An attempt was made to record the double potentials following linear ablation of the cavotricuspid isthmus. Following ablation, bidirectional isthmus block was also tested by pacing from the coronary sinus os and the low-lateral right atrium. We could demonstrate double potentials in 9 of the 11 patients in whom we attempted to record them following linear ablation of flutter. The presence of bidirectional block by pacing from coronary sinus os and low lateral right atrium could be demonstrated in 10 (91%) patients. Thus, double atrial potentials, separated by an isoelectric interval can be demonstrated following ablation of atrial flutter. Double potentials, if demonstrable on coronary sinus os and low lateral right atrium pacing, could serve as an alternative marker of isthmus block.  相似文献

OBJECTIVES: The purpose of this study was to determine the characteristics of double potentials (DPs) that are helpful in guiding ablation within the cavo-tricuspid isthmus. BACKGROUND: Double potentials have been considered a reliable criterion of cavo-tricuspid isthmus block in patients undergoing radiofrequency ablation of typical atrial flutter (AFL). However, the minimal degree of separation of the two components of DPs needed to indicate complete block has not been well defined. METHODS: Radiofrequency ablation was performed in 30 patients with isthmus-dependent AFL. Bipolar electrograms were recorded along the ablation line during proximal coronary sinus pacing at sites at which radiofrequency ablation resulted in incomplete or complete isthmus block. RESULTS: Double potentials were observed at 42% of recording sites when there was incomplete isthmus block, compared with 100% of recording sites when the block was complete. The mean intervals separating the two components of DPs were 65 +/- 21 ms and 135 +/- 30 ms during incomplete and complete block, respectively (p < 0.001). An interval separating the two components of DPs (DP(1-2) interval) <90 ms was always associated with a local gap, whereas a DP(1-2) interval > or =110 ms was always associated with local block. When the DP(1-2) interval was between 90 and 110 ms, an isoelectric segment within the DP and a negative polarity in the second component of the DP were helpful in indicating local isthmus block. A DP(1-2) interval > or =90 ms with a maximal variation of 15 ms along the entire ablation line was an indicator of complete block in the cavo-tricuspid isthmus. CONCLUSIONS: Detailed analysis of DPs is helpful in identifying gaps in the ablation line and in distinguishing complete from incomplete isthmus block in patients undergoing radiofrequency ablation of typical AFL.  相似文献

评价导管射频消融治疗峡部依赖性心房扑动 (AFL)中 ,心房激动顺序的变化与双电位标测这两种消融终点的判断方法 ,以期提高成功率 ,减少复发率。选取 1997年 11月至 2 0 0 2年 9月连续收治的 4 3例典型AFL病人。按照消融终点不同 ,以及收治时间顺序分为心房特征性激动顺序变化组 (AC组 ) ,双电位组 (DP组 )。于右房三尖瓣环与下腔静脉之间的峡部进行线性消融。AC组以出现右房特征性激动顺序的变化 (即低侧位右房刺激时呈逆钟向阻滞 ,冠状静脉窦刺激时呈顺钟向阻滞 )为依据判定为峡部双向阻滞。DP组在AC组消融结果基础上 ,以消融线上标测到宽间期双电位 (>10 0ms)为依据判定为峡部双向阻滞。随访复发率。结果 :4 3例患者 ,所有患者均达到相应的射频消融终点。DP组所有患者消融成功后双电位为 116 .6± 12 .5ms。随访 14 .5± 5 .4个月 ,AC组的复发率高于DP组 (2 3.5 %vs 0 ,P <0 .0 5 )。结论 :作为峡部双向阻滞的指征 ,宽间期双电位比心房特征性激动顺序变化更准确 ,手术成功率更高。  相似文献

Background: Complete bi-directional isthmus block is the endpoint of typical atrial flutter ablation. The purpose of this study was to investigate the feasibility of the local double potential (DP) interval and the change in transisthmus conduction time for predicting complete isthmus block after ablation of the cavotricuspid isthmus. Methods: The study population consisted of 32 patients with typical atrial flutter after a procedure of radiofrequency (RF) ablation of the cavotricuspid isthmus (16 had incomplete block and 16 had complete block). The transisthmus conduction time was determined during pacing from the proximal coronary sinus and low lateral right atrium before and after RF ablation. The DP interval close to the ablation line was evaluated after final RF energy application. Results: In the counterclockwise direction, transisthmus conduction time had an increase of 37 ± 25.4% and 127.3 ± 35.5% (P < 0.001), and the DP interval was 63.3 ± 8.7 ms and 120 ± 17.4 ms (P < 0.001) after achievement of incomplete and complete block, respectively. The sensitivity, specificity, positive and negative predictive values of an increase in the transisthmus conduction time 50% were 100%, 81%, 84% and 100%, respectively; those of DP interval 100 ms were 100%. In the clockwise direction, transisthmus conduction time had an increase of 38.8 ± 28.6% and 135.7 ± 63.6% (P < 0.001), and the DP interval was 63.6 ± 13.8 ms and 127.7 ± 27.1 ms (P < 0.001) after achievement of incomplete and complete block, respectively. The sensitivity, specificity, positive and negative predictive values of an increase in the transisthmus conduction time 50% were 100%, 67%, 83% and 100%, respectively; those of the DP interval 100 ms were 100%. Conclusions: The transisthmus conduction time 50% increase or DP interval 100 ms was feasible to predict complete bi-directional isthmus block.  相似文献

Background: Radiofrequency ablation (RFA) of the cavotricuspid isthmus (CTI) is an established therapy for typical atrial flutter. Previous studies have demonstrated that the CTI is often composed of discrete muscle bundles, and evidence has suggested that these bundles correlate with high-voltage local electrograms in the tricuspid isthmus. This randomized, multicenter clinical trial was designed to prospectively compare the hypothesis that a maximum voltage-guided (MVG) technique targets critical conducting bundles in the isthmus, as reflected by a reduction in ablation requirements compared to the anatomical approach to atrial flutter ablation.
Methods: Bidirectional block was achieved in patients undergoing ablation for typical atrial flutter using 1 of 2 randomly assigned methods. The anatomical approach produced a contiguous line of ablation lesions from the inferior aspect of the tricuspid annulus to the inferior vena cava using a standard method. The MVG technique sequentially targeted the maximum voltage local electrograms in the CTI along a similar line.
Results: Sixty-nine patients were randomized, with mean age 63 ± 10 and 58 (84%) male. Among patients in the anatomic group (n = 34), mean ablation time was 11.2 ± 7.5 minutes compared to 5.9 ± 3.3 in the MVG group (n = 35) (P = 0.0026). A mean of 14.2 ± 9.7 ablation lesions were created in the anatomic group, and 7.9 ± 4.8 in the MVG group (P = 0.0042).
Conclusions: Ablation for atrial flutter using an MVG technique results in significantly less ablation requirements than the traditional approach, potentially by concentrating ablation lesions on the muscle bundles responsible for transisthmus conduction.  相似文献

目的:探讨单导管标测法在心房扑动(房扑)射频消融中的应用方法和效果。方法:阵发性心房颤动并发房扑患者行肺静脉电隔离术时采用单导管标测法消融房扑30例。所有患者行肺静脉电隔离术后,将10极冠状静脉窦(CS)导管远端2对电极放置于CS内,余位于CS外,并使之有一定的张力,使导管贴靠于三尖瓣环和低右房。用冷盐水灌注消融导管线性消融三尖瓣峡部,房扑发作患者在房扑下消融,窦律患者在CS远端电极起搏下消融,可在术中随时把大头消融导管置于希氏束部位,用于评价是否已完全达双向阻滞,即:起搏CS远端电极,刺激信号至CS近端电极A波的距离大于至希氏束A波的距离,则CS口至低右房单向阻滞;CS近端电极起搏,刺激信号至CS远端电极A波的距离大于至希氏束A波的距离,则低右房至CS口单向阻滞,从而达双向阻滞,CS近端电极起搏所需电压较高,有的患者可达24mA。结果:所用阵发性心房颤动并发房扑患者均成功行三尖瓣峡部线性射频消融,达到双向阻滞,无手术相关并发症,随访4个月~2年,无房扑复发。结论:单导管标测法对房扑患者行三尖瓣峡部线性射频消融操作简单、快速,可完全用于评价消融结果,成功率高,并且节省手术费用。  相似文献

Background: The maximum voltage-guided (MVG) approach to ablation for atrial flutter targets high-amplitude signals along the cavotricuspid isthmus (CTI). It is based on the observation that the isthmus is often composed of bundles of conducting tissue and the hypothesis that these bundles manifest as high-amplitude electrograms, providing targets for selective ablation. We aim to identify patient and procedural factors that correlate with rapid isthmus ablation.
Methods: All patients undergoing CTI ablation at our center from January 2005 to May 2006 were included. Patients were divided into outcome groups relative to the median value for total ablation time. The two groups were compared according to patient and procedural variables, using multivariate regression methods.
Results: Seventy-six patients were assessed with mean age 60.2 ± 10.6 years; 63 (82.9%) were male. Mean ablation time to bidirectional block across the CTI was 6.85 ± 5.87 min (range 0.68–28.7); median 4.77 min. Seventy-six percent of patients required less than 5 min total ablation time until bidirectional block was achieved. Variables independently associated with a short ablation time were the presence of sinus rhythm at start of ablation (P = 0.0050, odds ratio (OR) 8.03), high mean temperature among all ablations (P = 0.019, OR 17.81), and low variability of mean power among all ablations (P = 0.0048, OR 19.26).
Conclusions: Using the MVG approach to atrial flutter ablation, shorter total ablation times are observed among patients in sinus rhythm at the onset of ablation, with higher mean temperature among ablation lesions, and less variability of power between ablations.  相似文献

BACKGROUND: Successful radiofrequency (RF) ablation of typical, isthmus-dependent atrial flutter requires establishment and confirmation of bidirectional conduction block across the cavotricuspid isthmus. Low atrial pacing usually is performed from the bipoles of the 20-pole Halo catheter, septal and lateral to the cavotricuspid isthmus ablation line. However, occasionally this is difficult because of high pacing thresholds and/or saturation of the atrial electrograms recorded near the pacing catheter. OBJECTIVES: The purpose of this study was to assess if right ventricular (RV) pacing and resulting retrograde atrial activation can be used to assess conduction block from the septum to the lateral wall in a clockwise direction. METHODS: Thirty-five consecutive male patients (mean age 64 +/- 10 years; mean ejection fraction 42 +/- 13%; mean left atrial dimension 44 +/- 6 mm) with typical isthmus-dependent atrial flutter were studied. The following electrophysiology catheters were used: 20-pole catheter along the tricuspid annulus, quadripolar catheters at the His and/or RV apex, and 8-mm ablation catheter. Following RF ablation of the cavotricuspid isthmus, bidirectional conduction block was confirmed in all 35 patients by pacing at a cycle length of 600 ms from bipoles septal and lateral to the cavotricuspid isthmus ablation line. Conduction times from pacing artifact to adjacent bipolar atrial electrograms and reversal of atrial activation pattern were analyzed. RV pacing was performed and retrograde atrial activation pattern assessed. If retrograde AV nodal conduction was absent, isoproterenol was infused intravenously at 2 microg/min, and RV pacing was repeated. The conduction time between the double potentials across the cavotricuspid isthmus ablation line was measured. RESULTS: Mean conduction times across the isthmus during septal (S), lateral (L), and RV pacing were 145 +/- 21 ms, 144 +/- 24 ms, and 129 +/- 20 ms, respectively. Retrograde AV nodal conduction was present in 34 of 35 patients (isoproterenol 8 patients). Evidence of conduction block by a clear change in activation pattern across the isthmus was seen during RV pacing in 33 of 35 patients with bidirectional conduction block. CONCLUSION: RV pacing is a simple and easy maneuver that can be performed to assess isthmus conduction in most patients.  相似文献

应用非接触标测系统实施典型心房扑动 (AFL)的右房 (RA)全心腔标测和导航射频消融。 6例典型AFL ,男 5例、女 1例 ,年龄 5 6 .2± 15 .3(35~ 76 )岁。常规放置冠状静脉窦和His束电极 ,将标测球囊置于RA中下部 ,构建RA心内膜模型 ,分别于低位RA和冠状窦口 (CSO)S1S16 0 0ms起搏观察峡部传导 ,诱发并标测AFL的激动顺序和折返路径。 1例为顺钟向AFL ,4例为逆钟向AFL ,1例未能诱发AFL。AFL周期 2 0 7± 34ms,非接触标测可显示整个折返环路、激动顺序和缓慢传导区。AFL的激动可以穿过界嵴上部并且传导相对缓慢 ,提示RA平滑部是折返环的一部分。后位峡部线性消融在导航系统指导下进行 ,无需X线透视。消融完成后重复上述起搏验证峡部双向传导阻滞。除 1例术中出现心房颤动 (AF)外 ,其余病例即刻均达到峡部双向阻滞 ,未出现其他并发症 ,随访 8.1± 6 .7(3~15 )个月未见复发。非接触标测系统可安全、有效和直观地实现典型AFL的右房全心腔标测并导航消融 ,验证峡部双向阻滞 ,减少X线曝光时间和无效放电次数。界嵴在典型AFL时具备传导功能 ,RA平滑部和粗糙部共同参与折返环的组成。  相似文献

Abstract Background: Atrial flutter is a common arrhythmia which frequently recurs after cardioversion and is relatively difficult to control with antiarrhythmic agents.
Aims: To evaluate the success rate, recurrence rate and safety of radiofrequency (RF) ablation for atrial flutter in a consecutive series of patients with drug refractory chronic or paroxysmal forms of the arrhythmia.
Methods: Electrophysiologic evaluation of atrial flutter included activation mapping with a 20 electrode halo cadieter placed around the tricuspid annulus and entrainment mapping from within the low right atrial isthmus. After confirmation of the arrhythmia mechanism with these techniques, an anatomic approach was used to create a linear lesion between the inferior tricuspid annulus and the eustachian ridge at the anterior margin of the inferior vena cava. In order to demonstrate successful ablation, mapping techniques were employed to show that bi-directional conduction block was present in the low right atrial isthmus.
Results: Successful ablation was achieved in 26/27 patients (96%). In one patient with a grossly enlarged right atrium, isthmus block could not be achieved. Of the 26 patients with successful ablation, mere has been one recurrence of typical flutter (4%) during a mean follow-up period of 5.5±2.7 months. This patient underwent a successful repeat ablation procedure. Of eight patients with documented clinical atrial fibrillation (in addition to atrial flutter) prior to the procedure, five continued to have atrial fibrillation following the ablation. There were no procedural complications and all patients had normal AV conduction at the completion of the ablation.
Conclusions: RF ablation is a highly effective and safe procedure for cure of atrial flutter. In patients with chronic or recurrent forms of atrial flutter RF ablation should be considered as a first line therapeutic option.  相似文献

AIMS: Antiarrhythmic drug treatment for atrial fibrillation can cause atrial flutter-like arrhythmias. The aim of this study was to clarify the effect of catheter ablation of the tricuspid annulus-vena cava inferior isthmus on amiodarone-induced atrial flutter and to determine the incidence of atrial fibrillation after catheter ablation of amiodarone-induced atrial flutter in comparison to regular typical flutter. METHODS AND RESULTS: Among 92 consecutive patients with typical atrial flutter who underwent isthmus ablation 28 patients had atrial flutter without a history of previous atrial fibrillation (group I), 10 patients had atrial flutter following the initiation of amiodarone therapy for paroxysmal atrial fibrillation (group II) and 54 patients had atrial flutter and atrial fibrillation (group III). Atrial cycle length during atrial flutter in amiodarone-treated patients (group II) (277+/-24 ms) was significantly longer as compared to the cycle length of atrial flutter in group I (247+/-33 ms) and group III patients (235+/-28 ms). The rate of successful transient entrainment and overdrive stimulation to sinus rhythm was not different between patients with (60%) or without amiodarone therapy (group I: 71%, group III: 53%). Successful isthmus ablation with bidirectional conduction block eliminating right atrial flutter was achieved in 90% of amiodarone-treated patients and 93% of patients without amiodarone therapy. In the amiodarone-treated patient group atrial conduction times during pacing in sinus rhythm were significantly prolonged by 20-30% before and after ablation in all regions of the reentrant circuit. During a mean follow-up of 8+/-3 months post-ablation, atrial fibrillation recurred in two of 10 patients on continued amiodarone therapy after successful isthmus ablation. Thus, successful catheter ablation of atrial flutter due to amiodarone therapy was associated with a markedly lower recurrence rate of paroxysmal atrial fibrillation (20%) as compared to patients with atrial flutter plus preexisting paroxysmal atrial fibrillation (76%) and was similar to the outcome of patients with successful atrial flutter ablation without preexisting atrial fibrillation (25%). CONCLUSION: These data suggest that isthmus ablation with bidirectional block and continuation of amiodarone therapy is an effective therapy for the treatment of atrial flutter due to amiodarone therapy for paroxysmal atrial fibrillation.  相似文献

Background: The characteristics of cavotricuspid isthmus (CTI) in patients with atrial fibrillation (AF) and flutter that may predict recurrence of flutter is not known. We aimed to investigate the CTI characteristics in patients who underwent a second ablation procedure for recurrent AF after previous combined pulmonary vein (PV) and CTI ablation.
Methods: Among 196 consecutive patients with drug-refractory symptomatic AF who underwent PV isolation and CTI ablation with bidirectional isthmus block, 49 patients (age 50 ± 12 years, 43 males) had recurrent AF and received a second procedure 291 ± 241 days after the first procedure. Right atrial angiography for the evaluation of the CTI morphology, and the biatrial contact bipolar electrograms were obtained before both procedures.
Results: In the second procedure, 11 (group 1) of the 49 patients demonstrated recovered CTI conduction. Compared with the patients without CTI conduction (group 2, n = 38), group 1 patients had a higher frequency of a pouch-type anatomy (82% vs 13%, P < 0.001), longer CTI (34.0 ± 8.6 vs 25.5 ± 7.5 mm, P = 0.01), longer ablation time, and larger number of radiofrequency applications; furthermore, the preablation bipolar voltage decreased along both the CTI and ablation line in group 2, whereas it remained similar in group 1 in the second procedure.
Conclusions: A high (22%) percentage of CTIs exhibited recurrent conduction in the long-term follow-up. The CTIs with recurrent conduction had a higher incidence of a pouch and longer length compared with those without recurrent conduction.  相似文献

对比观察射频消融右房后位峡部和间隔峡部治疗心房扑动 (简称房扑 )两种方法的疗效。 41例房扑患者随机分为后位峡部组 ( 18例 )和间隔峡部组 ( 2 3例 ) ,消融线径分别为三尖瓣环—下腔静脉和三尖瓣环—欧氏嵴。成功消融终点为房扑不能诱发和峡部呈完全性双向阻滞。结果 :40例消融成功 ( 97.6 % ) ,无并发症。后位峡部组 3例和间隔峡部组 2例在首选消融方法失败后 ,改用另一种消融方法获得成功。两组患者的放电次数和手术时间均无显著性差异。平均随访 12 .4± 6 .8个月 ,在后位峡部消融成功者中 ,有 2例房扑复发。结论 :射频消融右房后位峡部和间隔峡部治疗房扑均安全有效 ,两种方法互补可以提高消融的成功率  相似文献

Background: Left atrial (LA) linear lesions are effective in substrate modification for atrial fibrillation (AF). However, achievement of complete conduction block remains challenging and conduction recovery is commonly observed. The aim of the study was to investigate the localization of gap sites of recovered LA linear lesions.
Methods and Results: Forty-eight patients with paroxysmal (n = 26) and persistent/permanent (n = 22) AF underwent repeat ablation after pulmonary vein (PV) isolation and LA linear ablation at the LA roof and/or mitral isthmus due to recurrences of AF or flutter. In 35 patients, conduction through the mitral isthmus line (ML) had recovered whereas roof-line recovery was observed in 30 patients. The gaps within the ML were distributed to the junction between left inferior PV and left atrial appendage in 66%, the middle part of the ML in 20%, and in 8% to the endocardial aspect of the ML while only 6% of lines showed an epicardial site of recovery. The RL predominantly recovered close to the right superior PV (54%) and less frequently in the mid roof or close to the left PV (both 23%). Reablation of lines required significantly shorter RF durations (ML: 7.24 ± 5.55 minutes vs 24.08 ± 9.38 minutes, RL: 4.24 ± 2.34 minutes vs 11.54 ± 6.49 minutes; P = 0.0001). Patients with persistent/permanent AF demonstrated a significantly longer conduction delay circumventing the complete lines than patients with paroxysmal AF (228 ± 77 ms vs 164 ± 36 ms, P = 0.001).
Conclusions: Gaps in recovered LA lines were predominantly located close to the PVs where catheter stability is often difficult to achieve. Shorter RF durations are required for reablation of recovered linear lesions. Conduction times around complete LA lines are significantly longer in patients with persistent/permanent AF as compared to patients with paroxysmal AF.  相似文献

BACKGROUND: Ablation of typical atrial flutter relies on the suppression of electrical conduction along the cavo-tricuspid isthmus. Bidirectional isthmus block is a criterion of successful ablation and is associated with the presence of different activation times on each side of the ablation line. OBJECTIVE: The aim of this study was to determine whether the difference in isthmic activation times correlates with the length of the atrial flutter cycle. POPULATION AND METHODS: We studied 31 patients with typical atrial flutter (93.6% male, mean age 66 +/- 9 years) who underwent successful ablation during tachycardia. CARTO electroanatomic mapping was used to confirm diagnosis of isthmus-dependent atrial flutter, guide the ablation line creation and assess its efficacy. At the end of the procedure, a three-dimensional activation map of the right atrium was constructed, under pacing from the coronary sinus ostium (with a 500 ms cycle). Activation times on the lateral (right) and septal (left) sides of the ablation line were measured. The difference between these two activation times was termed the difference in isthmic activation times (delta IAT), and was compared to the flutter cycle length. RESULTS: Mean activation times were 173.7 +/- 34.3 ms on the lateral border of the ablation line and 19.1 +/- 12.5 ms on the septal border. Mean delta IAT was 154.6 +/- 27.8 ms and mean atrial flutter cycle length was 257.5 +/- 30.6 ms. delta IAT and flutter cycle length were significantly correlated (r = 0.503, p = 0.0039). The linear regression equation that best described this result was: delta IAT = 37 + (0.46 x flutter cycle). INTRODUCTION: Complete bidirectional cavotricuspid isthmus block is the endpoint for ablation of typical atrial flutter. The purpose of this study was to determine whether the extent of prolongation of the transisthmus interval after ablation predicts complete bidirectional block. METHODS AND RESULTS: Fifty-seven consecutive patients underwent 60 ablation procedures for isthmus-dependent atrial flutter. The clockwise and counterclockwise transisthmus intervals were determined before and after ablation during pacing from the low lateral right atrium and the coronary sinus. Bidirectional block was achieved with ablation in 55 (96%) of 57 patients. The transisthmus intervals before ablation and after complete transisthmus block were 100.3 +/- 21.1 msec and 195.8 +/- 30.1 msec, respectively, in the clockwise direction (P < 0.0001), and 98.2 +/- 24.7 msec and 185.7 +/- 33.9 msec, respectively, in the counterclockwise direction (P < 0.0001). An increase in the transisthmus interval by > or = 50% in both directions after ablation predicted complete bidirectional block with 100% sensitivity and 80% specificity. The positive and negative predictive values were 89% and 100%, respectively. The diagnostic accuracy of a > or = 50% prolongation in the transisthmus interval was 92%. CONCLUSION: Prolongation of the transisthmus interval by > or = 50% in the clockwise and counterclockwise directions is associated with a high degree of diagnostic accuracy and an excellent negative predictive value in determining complete bidirectional transisthmus block. This may be a useful and simple adjunctive criterion for assessment of complete transisthmus conduction block.  相似文献

INTRODUCTION: The atrial activation sequence around the tricuspid annulus has been used to assess whether complete block has been achieved across the cavotricuspid isthmus during radiofrequency ablation of typical atrial flutter. However, sometimes the atrial activation sequence does not clearly establish the presence or absence of complete block. The purpose of this study was to determine whether a change in the polarity of atrial electrograms recorded near the ablation line is an accurate indicator of complete isthmus block. METHODS AND RESULTS: Radiofrequency ablation was performed in 34 men and 10 women (age 60 +/- 13 years [mean +/- SD]) with isthmus-dependent, counterclockwise atrial flutter. Electrograms were recorded around the tricuspid annulus using a duodecapolar halo catheter. Electrograms recorded from two distal electrode pairs (E1 and E2) positioned just anterior to the ablation line were analyzed during atrial flutter and during coronary sinus pacing, before and after ablation. Complete isthmus block was verified by the presence of widely split double electrograms along the entire ablation line. Complete bidirectional isthmus block was achieved in 39 (89%) of 44 patients. Before ablation, the initial polarity of E1 and E2 was predominantly negative during atrial flutter and predominantly positive during coronary sinus pacing. During incomplete isthmus block, the electrogram polarity became reversed either only at E2, or at neither E1 nor E2. In every patient, the polarity of E1 and E2 became negative during coronary sinus pacing only after complete isthmus block was achieved. In 4 patients (10%), the atrial activation sequence recorded with the halo catheter was consistent with complete isthmus block, but the presence of incomplete block was accurately detected by inspection of the polarity of E1 and E2. CONCLUSION: Reversal of polarity in bipolar electrograms recorded just anterior to the line of isthmus block during coronary sinus pacing after ablation of atrial flutter is a simple, quick, and accurate indicator of complete isthmus block.  相似文献

通过比较心房扑动 (简称房扑 )成功消融前后右心房峡部时间间期 ,分析峡部时间间期的延长程度对峡部完全性双向阻滞的预测价值。选择 1996~ 2 0 0 2年在我院行射频消融治疗的典型房扑患者 30例 ,男 2 3例、女 7例 ,年龄 4 7.85± 9.35岁 ,采用解剖和影像定位法 ,在冠状静脉窦口持续起搏下消融峡部。结果 :2 9例消融成功 ,达到双向传导阻滞的标准 ,成功率 96 .6 7%。消融后起搏信号至右房下侧壁的时间间期 (SAH1 2 )和起搏信号至冠状窦口CS3 4的时间间期明显延长 (14 0 .4 7± 2 0 .4 8msvs73.82± 13.0 1ms ;138.17± 15 .5 5msvs77.6 3± 8.36ms ,P <0 .0 0 0 1)。 2 9例中有 17例在完全性传导阻滞前可以记录到不完全性传导阻滞 ,峡部不完全性传导阻滞时SAH1 2 比术前增加 4 5 .4 9%± 8.7%。消融前后右心房峡部传导时间间期增加≥ 5 0 % ,预测峡部完全性双向传导阻滞的灵敏度 10 0 % ,特异度 83.3%。结论 :右心房峡部传导时间间期的定量分析是判断峡部完全性双向传导阻滞的有价值的方法。  相似文献

Introduction: Atrial fibrillation and atrial flutter often coexist. The long-term occurrence of atrial fibrillation in patients presenting with atrial flutter alone is unknown. We report the long-term follow-up in patients who underwent cavotricuspid isthmus ablation for treatment of lone atrial flutter.
Methods and Results: Between January 1997 and June 2002, 632 patients underwent cavotricuspid isthmus ablation for the treatment of typical atrial flutter at the Cleveland Clinic Foundation. Three hundred sixty-three patients were included in this study and followed for a mean duration of 39 ± 11 months. The mean duration of atrial flutter symptoms was 12 ± 5 months. Mean left-atrial size and left-ventricular ejection fraction were 4.2 ± 0.8 cm and 47 ± 13%, respectively. After a mean follow-up time of 39 ± 11 months, 13% (48 of 363) of the patients remained in sinus rhythm. Five percent (18 of 363) of patients experienced recurrence of atrial flutter only. Sixty-eight percent (246 of 363) experienced the onset of atrial fibrillation and 14% (51 of 363) experienced recurrence of atrial flutter and the new onset of atrial fibrillation. Overall, 82% (297 of 363) of the patients experienced new onset of drug refractory atrial fibrillation. Left-atrial size was a predictor of atrial fibrillation recurrence post-atrial flutter ablation.
Conclusion: At long-term follow-up, approximately 82% of patients post-cavotricuspid isthmus ablation for atrial flutter developed drug refractory atrial fibrillation. This finding suggests that elimination of atrial flutter might delay, but does not prevent, atrial fibrillation. Evidence suggests both arrhythmias may share common triggers and such patients may derive a better long-term benefit from anatomical ablative treatment of atrial fibrillation as well.  相似文献
