目的比较成年早发抑郁症(EOD)和成年晚发抑郁症(LOD)患者默认网络(DMN)内部功能连接的差异,探究不同发病年龄的抑郁症患者是否有不同的发病机制。方法选取在昆明医科大学第一附属医院精神科门诊或住院的EOD患者(n=58)和LOD患者(n=62)为研究对象,同期招募年轻健康对照组(n=60)和年老健康对照组(n=52)。对受试者进行静息态功能磁共振扫描,选择左侧楔前叶为种子点,计算该种子点与全脑的功能连接,并比较各组间该种子点的功能连接差异。结果四组之间功能连接具有差异的脑区涉及双侧额叶、颞叶、基底节、枕叶、顶叶及小脑等脑区。EOD组左侧楔前叶与左侧小脑Crus1区、左侧小脑IX区、左侧颞中回、右侧楔前叶、右侧前扣带回、右侧额中回、右侧角回、右侧脑岛、右侧内侧额上回、右侧颞中回的功能连接均高于年轻健康对照组(Z=3. 752 4~5. 867 8,P均0. 05);而左侧楔前叶与左侧额中回、左侧中央旁小叶、右侧缘上回、右侧额上回、右侧颞下回、右侧中央后回、右侧中央前回、右侧枕上回的功能连接均低于年轻健康对照组(Z=-5. 007 6~-3. 797 7,P均0. 05)。LOD组左侧楔前叶与左侧小脑Crus2区、左侧尾状核、左侧颞下回、左侧小脑Crus1区、左侧角回、左侧额中回、右侧额中回、右侧角回、右侧眶额部额中回的功能连接均高于年老健康对照组(Z=4. 122 8~6. 579 4,P均0. 05);与左侧海马旁回、左侧额上回、右侧枕中回、右侧中央前回、右侧内侧额上回、右侧锯状回、右侧颞下回、右侧中央旁小叶、右侧梭状回、右侧后扣带回的功能连接均低于年老健康对照组(Z=-5. 884 0~-3. 617 2,P均0. 05)。EOD组左侧楔前叶与左侧锯状回、左侧小脑IV-VI区、左侧小脑Crus2区的功能连接比LOD组高(Z=4. 087 7、3. 937 4、3. 672 1,P均0. 05);EOD组左侧楔前叶与右侧额中回、右侧眶额部额下回、右侧额上回的功能连接比LOD组低(Z=-4. 274 8、-3. 956 8、-4. 724 3、-3. 663 2,P均0. 05)。结论 DMN内部功能连接增高及额顶网络功能连接降低可能与EOD的发病机制相关,而DMN前部功能连接增高和后部功能连接降低可能与LOD的发病机制相关,不同发病年龄的成年抑郁症患者可能有不同的发病机制。  相似文献

皮质下缺血性血管性认知损害扩散张量成像研究   总被引:1,自引:0,他引:1  
目的通过扩散张量成像(DTI)探讨皮质下缺血性血管性认知损害患者白质微结构变化及其与认知功能之间的相关性。方法采集49例皮质下缺血性脑血管病患者[轻度血管性痴呆(VaD)10例、非痴呆型血管性认知损害(VCIND)20例、认知功能正常19例]DTI数据并观察皮质下白质微结构改变,分析VaD组患者DTI参数与认知功能间的相关性。结果与对照组相比,VaD组内侧前额叶、前扣带回、胼胝体干、双侧顶叶、右侧颞叶、双侧眶额叶,以及VCIND组右侧额下回、右侧海马、双侧楔前叶FA值减低(均P=0.000);与VCIND组比较,VaD组内侧前额叶、前扣带回、胼胝体、双侧顶叶、右侧颞叶FA值减低(P=0.000)。与对照组相比,VaD组内侧前额叶、胼胝体、双侧顶叶、双侧颞叶、前扣带回,以及VCIND组双侧楔前叶、右侧海马MD值升高(均P=0.000);与VCIND组相比,VaD组右侧内侧前额叶、前扣带回、胼胝体干、双侧顶叶、双侧颞叶MD值升高(均P=0.000)。VaD组内侧前额叶FA值与数字连线测验A时呈显著负相关(r=-0.782,P=0.007),双侧额下回MD值与数字连线试验A时程呈显著正相关(r=0.877,P=0.001)。结论 DTI对皮质下缺血性认知损害患者白质微结构改变更敏感,能够反映患者认知功能早期异常改变;内侧前额叶白质微结构的改变是影响患者执行能力的重要因素。  相似文献

Previous studies have demonstrated lower-diffusion anisotropy within white matter in late-onset depression measured by diffusion tensor imaging, which provides information about brain white matter integrity. We have examined whether white matter is abnormal in first-episode remitted geriatric depression by using diffusion tensor imaging. Sixteen remitted geriatric depression patients and 14 well matched healthy controls underwent diffusion tensor-imaging scans of magnetic resonance imaging, which were analyzed by a rigorous voxel-based approach. We found that fractional anisotropy in white matter was lower in patients than in controls at the right superior frontal gyrus, left inferior frontal gyrus, left middle temporal gyrus, right inferior parietal lobule, right middle occipital gyrus, left lingual gyrus, right putamen and right caudate. These results suggested that the white matter integrity of the whole brain was disrupted in first-episode remitted geriatric depression, and that these abnormalities were perhaps involved in the psychopathology and pathophysiology of cognitive impairment in remitted geriatric depression.  相似文献

目的本文研究采用静息态功能磁共振成像(rfMRI)技术描述偏头痛患者静息态脑功能连接改变,为探索偏头痛的发病机制提供影像学资料。方法收集16例偏头痛患者与16例健康对照,采集rfMRI成像,计算低频振幅,找出感兴趣区做功能连接进行统计分析。结果偏头痛患者左侧岛叶、左侧额下回低频振幅显著低于对照组,右侧视觉皮质低频振幅显著高于对照组;以左侧额下回、右侧枕中回为感兴趣区,发现左侧额下回与脑干之间的功能连接增强,与双侧枕叶之间的功能连接减弱;右侧枕中回与双侧楔前叶延伸至扣带回中部区域之间的功能连接增强,与双侧中央前回、双侧缘上回、双侧颞上回及双侧额下回之间的功能连接减弱。结论偏头痛患者无头痛发作时神经元活动强度改变,大脑功能连接异常,这导致大脑整合信息过程改变,并与偏头痛发病相关。  相似文献

End-stage renal disease (ESRD) is associated with vascular and neuronal dysfunction, causing neurovascular coupling (NVC) dysfunction, but how NVC dysfunction acts on the mechanism of cognitive impairment in ESRD patients from local to remote is still poorly understood. We recruited 48 ESRD patients and 35 demographically matched healthy controls to scan resting-state functional MRI and arterial spin labeling, then investigated the four types of NVC between amplitude of low-frequency fluctuation (ALFF), fractional ALFF, regional homogeneity, degree centrality, and cerebral blood perfusion (CBF), and associated functional networks. Our results indicated that ESRD patients showed NVC dysfunction in global gray matter and multiple brain regions due to the mismatch between CBF and neural activity, and associated disrupted functional connectivity (FC) within sensorimotor network (SMN), visual network (VN), default mode network (DMN), salience network (SN), and disrupted FC between them with limbic network (LN), while increased FC between SMN and DMN. Anemia may affect the NVC of middle occipital gyrus and precuneus, and increased pulse pressure may result in disrupted FC with SMN. The NVC dysfunction of the right precuneus, middle frontal gyrus, and parahippocampal gyrus and the FC between the right angular gyrus and the right anterior cingulate gyrus may reflect cognitive impairment in ESRD patients. Our study confirmed that ESRD patients may exist NVC dysfunction and disrupted functional integration in SMN, VN, DMN, SN and LN, serving as one of the mechanisms of cognitive impairment. Anemia and increased pulse pressure may be related risk factors.  相似文献

【摘要】 目的 运用磁共振成像(magnetic resonance imaging,MRI)基于白质地图的扩散张量成像(diffusion tensor imaging,DTI)技术研究皮质下缺血性血管病(subcortical ischemic vascular disease,SIVD)患者联络纤维的超微结构改变,并分析DTI参数与认知功能的关系。 方法 连续入选40例SIVD患者,分为血管性认知障碍组18例,无认知障碍组22例,另设立20例正常老年人为对照组。对三组进行MRI检查并对其白质改变进行评分,对联络纤维进行基于白质地图分析(atlase based analysis,ABA)DTI成像,比较组间部分各向异性指数(fractional anisotrophy,FA)、平均扩散率(mean diffusivity,MD)差异,并对SIVD患者DTI参数与蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)得分进行相关性分析。 结果 无认知障碍组较正常组在右侧矢状层(下纵束/下额枕束)、右侧扣带(海马部)及左侧上额枕束的FA值减低;认知障碍组较正常组存在广泛的联络纤维FA值降低,差异均具有显著性。无认知障碍组与正常组比较,仅左侧扣带海马部及左侧上纵束MD值无显著差异,余联络纤维的MD值均显著增高;认知障碍组较正常组存在广泛的联络纤维MD值增高,差异均具有显著性。与无认知功能障碍组比较,认知功能障碍组在双侧矢状层(下纵束/下额枕束)、双侧扣带扣带回部、海马部、双侧上纵束、双侧上额枕束及双侧钩束的FA值减低,MD值增高,差异具有显著性。SIVD患者双侧矢状层(下纵束/下额枕束)、扣带扣带回部、上纵束、上额枕束及钩束和左侧扣带海马部的FA值与MoCA评分呈正相关,MD值与MoCA评分呈负相关。 结论 SIVD患者联络纤维的FA值减低,MD值增高,且DTI参数与其认知功能密切相关。  相似文献

目的应用静息态功能磁共振成像(rs-f MRI)探讨难治性癫痫患者楔前叶与全脑功能连接的改变及其意义。方法对23例难治性癫痫患者(癫痫组)及23名健康人(对照组)进行rs-f MRI检查,采集数据后采用功能连接的方法,分别计算以左侧楔前叶和右侧楔前叶为感兴趣区与全脑的功能连接,运用双样本t检验的统计学方法发现两组的显著性差异并进行分析。结果癫痫组左侧楔前叶与双侧顶下小叶、双侧岛叶、右侧缘上回、右侧额中回和双侧额下回的功能连接较对照组减弱(均P0.05);癫痫组左侧楔前叶与双侧海马旁回、双侧额上回、左侧后扣带回的功能连接较对照组增强(均P0.05)。癫痫组右侧楔前叶与左侧顶下小叶、右侧缘上回、右侧额中回、右侧额下回的功能连接较对照组减弱(均P0.05);癫痫组右侧楔前叶与双侧楔叶、左侧后扣带回的功能连接较对照组增强(均P0.05)。结论难治性癫痫患者静息状态下楔前叶与全脑的功能连接存在异常,"默认网络"功能的抑制可能是癫痫患者意识及认知等功能损害的重要原因。  相似文献

目的探讨阿尔茨海默病(AD)脑白质葡萄糖代谢异常的意义。方法纳入33例符合美国精神障碍诊断与统计手册-第四版(DSM-IV)AD诊断标准的患者和健康对照20名,进行脑正电子发射断层成像(PET)检查。应用SPM软件对PET图像进行分析。结果①与健康对照相比,AD患者有广泛的白质葡萄糖代谢减低,减低较为明显的区域有右侧额叶皮质下白质、左侧额叶上中回皮质下白质(P<0.001);另外,AD患者左侧额叶内侧回皮质下白质、左侧枕叶楔回皮质下白质葡萄糖代谢增强(P<0.001);②与不伴有精神行为症状(BPS)的AD患者(16例)相比,伴有BPS的AD患者(17例)在左右枕叶中回、右侧枕叶楔回、右侧顶下小叶、左侧颞叶梭形回、左侧额叶内侧回等脑区的皮质下白质葡萄糖代谢增强(P<0.001);而左右额叶中央旁回、右侧额叶上回和中回、左侧颞叶上回等脑区的皮质下白质葡萄糖代谢减低(P<0.001)。结论AD有广泛的白质脑葡萄糖代谢异常,有无BPS的AD白质代谢异常不同。  相似文献

Functional magnetic resonance imaging was used during emotion recognition to identify changes in functional brain activation in 21 first-episode, treatment-naive major depressive disorder patients before and after antidepressant treatment. Following escitalopram oxalate treatment, patients exhibited decreased activation in bilateral precentral gyrus, bilateral middle frontal gyrus, left middle temporal gyrus, bilateral postcentral gyrus, left cingulate and right parahippocampal gyrus, and increased activation in right superior frontal gyrus, bilateral superior parietal lobule and left occipital gyrus during sad facial expression recognition. After antidepressant treatment, patients also exhibited decreased activation in the bilateral middle frontal gyrus, bilateral cingulate and right parahippocampal gyrus, and increased activation in the right inferior frontal gyrus, left fusiform gyrus and right precuneus during happy facial expression recognition. Our experimental findings indicate that the limbic-cortical network might be a key target region for antidepressant treatment in major depressive disorder.  相似文献

ObjectiveEvidence of the brain network involved in cognitive dysfunction has been inconsistent for major depressive disorder (MDD), especially during early stage of MDD. This study seeks to examine abnormal cognition connectivity network (CCN) in MDD within the whole brain.MethodsSixteen patients with MDD and 16 health controls were scanned during resting-state using 3.0 T functional magnetic resonance imaging (fMRI). All patients were first episode without any history of antidepressant treatment. Both the left and right dorsolateral prefrontal cortex (DLPFC) were used as individual seeds to identify CCN by the seed-target correlation analysis. Two sample t test was used to calculate between-group differences in CCN using fisher z-transformed correlation maps.ResultsThe CCN was constructed by bilateral seed DLPFC in two groups separately. Depressed subjects exhibited significantly increased functional connectivity (FC) by left DLPFC in one cluster, overlapping middle frontal gyrus, BA7, BA43, precuneus, BA6, BA40, superior temporal gyrus, BA22, inferior parietal lobule, precentral gyrus, BA4 and cingulate gyrus in left cerebrum. Health controls did not show any cluster with significantly greater FC compared to depressed subjects in left DLPFC network. There was no significant difference of FC in right DLPFC network between depressed subjects and the health controls.ConclusionThere are differences in CCN during early stage of MDD, as identified by increased FCs among part of frontal gyrus, parietal cortex, cingulate cortex, and BA43, BA22, BA4 with left DLPFC. These brain areas might be involved in the underlying mechanisms of cognitive dysfunction in MDD.  相似文献

目的 探讨恢复期老年抑郁症患者全脑白质纤维完整性是否受损,并进一步分析其与认知损害是否关联.方法 对16例恢复期老年抑郁症患者(患者组)和14名健康老年人(对照组)进行汉密尔顿抑郁量表(HAMD)、简易智力状态检查(MMSE)和神经心理学评估后,行全脑弥散张量成像扫描,用SPM2软件处理图像,采用基于体素的分析方法进行组间比较分数各向异性(FA)值.结果 (1)HAMD、MMSE评分和神经心理学测试:患者组HAMD和MMSE评分与正常对照组的差异无统计学意义(P>0.05),但在听觉言语测验[(5.9±2.6)分]、连线测验A[(146±127)s]和B[(264±196)s]成绩均差于正常对照组[分别为(9.3±1.6)分,(73±25)s和(121±35)s;P<0.01~0.05].(2)FA值:患者组低于对照组的脑区主要包括:右侧额上回、左侧额下回、左侧颞中回、右侧顶下回、右侧枕中回、左侧舌回、右侧壳核和右侧尾状核(未校正,P<0.001).(3)患者组右侧额上回FA值与连线B测验成绩呈显著负相关(r=-0.556,P=0.049).结论 恢复期老年抑郁症患者多部位脑白质纤维的完整性受损,这可能是该类患者认知损害的神经病理基础.  相似文献

目的 以静息态功能磁共振的研究方法探索进展期帕金森病(Advanced Parkinson’s disease,aPD)患者丘脑各个亚区与其他脑区之间功能连接的差异,并探讨异常功能连接与神经心理学评分之间的关系。方法 对2018年12月-2021年11月就诊于广东省人民医院的进展期帕金森病患者(aPD组)26例和38名健康对照者(Healthy control,HC)组的临床资料收集、神经心理学评估以及静息态功能磁共振扫描,以5个丘脑亚区作为种子点进行功能连接分析,并采用多元线性回归分析方法分析aPD患者认知功能与丘脑亚区异常功能连接的相关性。结果 aPD组左侧双侧腹前部(Ventral anterior,VA)与左侧中央后回、左侧扣带回中部之间的功能连接显著增强;右侧VA与左侧中央后回、左侧扣带回中部之间的功能连接显著增强。多元回归分析发现,左侧VA与左侧中央后回的功能连接(Functional connectivity,FC)值与色词干扰测试干扰效应(Stroop interfere effects,SIE)呈负相关(β=-0.773,P=0.027),右侧VA与左侧扣带回中部的F...  相似文献

In addition to the role of planning and executing movement, the cerebellum greatly contributes to cognitive process. Numerous studies have reported structural and functional abnormalities in the cerebellum for HIV-infected patients, but little is known about the altered functional connectivity of particular cerebellar subregions and the cerebrum. Therefore, this study aimed to explore the resting-state functional connectivity (rsFC) changes of the cerebellum and further analyze the relationship between the rsFC changes and the neuropsychological evaluation. The experiment involved 26 HIV-infected men with asymptomatic neurocognitive impairment (ANI) and 28 healthy controls (HC). We selected bilateral hemispheric lobule VI and lobule IX as seed regions and mapped the whole-brain rsFC for each subregion. Results revealed that right lobule VI showed significant increased rsFC with the anterior cingulate cortex (ACC) in HIV-infected subjects. In addition, the correlation analysis on HIV-infected subjects illustrated the increased rsFC was negatively correlated with the attention/working memory score. Moreover, significantly increased cerebellar rsFCs were also observed in HIV-infected patients related to right inferior frontal gyrus (IFG) and right superior medial gyrus (SMG) while decreased rsFC was just found between right lobule VI and the left hippocampus (HIP). These findings suggested that, abnormalities of cerebro-cerebellar functional connectivity might be associated with cognitive dysfunction in HIV-infected men, particularly working memory impairment. It could also be the underlying mechanism of ANI, providing further evidence for early injury in the neural substrate of HIV-infected patients.  相似文献

目的基于图论的分析方法了解难治性癫痫患者脑功能网络介数属性变化,探讨介数异常脑区间功能连接改变的意义。方法采集难治性癫痫患者及健康被试RS-f MRI数据后,通过Gretna中Network Analysis模块预处理并计算出各脑区的介数值,经统计检验找出介数值有差异脑区;运用REST软件将上述脑区作为ROI两两之间做功能连接,经统计检验后找出脑区间差异的连接。结果与对照组相比,癫痫组介数值升高的脑区有右侧额下回三角部、右侧嗅皮质;介数值下降的脑区有右侧额上回背外侧、左侧额上回眶部、左侧前扣带回、右侧颞上回、右侧颞下回;与健康被试相比,额下回三角部与颞上回之间的连接降低。结论难治性癫痫患者脑功能网络介数属性及异常脑区间的功能连接改变,引起脑区间信息交流的障碍,可能导致癫痫患者认知功能的损伤。  相似文献

SPM8 DARTEL工具包对3D脑结构成像数据对男性青年反社会人格障碍者和正常组的大脑白质结构进行基于体素的形态学分析(VBM)。结果显示,与正常对照者比较,反社会人格障碍者主要表现为双侧前额叶、右岛叶、中央前回,左右颞上回、右中央后回、右侧顶下小叶、右侧楔前叶、右枕中叶、右海马旁回及双侧扣带回等多个脑区白质体积密度增加,左颞中叶、右小脑的白质体积密度减小。相关分析显示,额内侧回的白质体积密度增加与反社会行为的评分(PDQ)有正相关。提示反社会人格障碍者存在多个脑区的白质形态学明显异常,这些异常可能与其反社会行为有关。  相似文献

目的分析阿尔茨海默病(Alzheimer's disease,AD)脑白质结构改变及与认知功能的关系。方法对37例AD组和32例对照组行简易精神状态量表(mini-mental State examinationn,MMSE)评估和DTI扫描。采用基于全脑体素分析法对两组全脑白质各向异性(fractional anisotropy,FA)图进行比较,采用t检验分析FA值差异,并评估AD组MMSE评分与FA值相关性。结果 AD患者出现FA值下降区域广泛分布在右侧额叶、颞叶、枕叶、丘脑及双侧扣带回、胼胝体、楔前叶、顶叶下回、顶下小叶、缘上回及海马旁回(其中P0.001,未经校正的P值);当使用经FWE校正的P0.05后,AD患者右侧扣带回、左侧胼胝体、颞叶下回及双侧顶叶下回、额叶下回、楔前叶区域FA值较对照组显著下降。AD患者FA值下降与MMSE量表评分呈正相关,(P0.001,未经校正)。结论 AD患者存在特定脑区白质结构改变,并与认知功能损害程度呈正相关。  相似文献

We investigated how resting state (RS) functional connectivity (FC) of the anterior cingulate cortex (ACC) correlates with cognitive rehabilitation in relapsing remitting multiple sclerosis (RRMS) patients. A neuropsychological assessment and RS fMRI at baseline and after 12 weeks were obtained from 20 RRMS patients, who were assigned randomly to undergo treatment (n?=?10) (treatment group-TG), which entailed computer-assisted cognitive rehabilitation of attention/information processing and executive functions for 3 days/week, or not to receive any cognitive rehabilitation (n?=?10) (control group-CG). Voxel-wise changes of ACC RS FC were assessed using SPM8. In both groups, at the two study time points, ACC activity was correlated with the bilateral middle and inferior frontal gyrus, basal ganglia, posterior cingulate cortex, cerebellum, precuneus, middle temporal gyrus, and inferior parietal lobule (IPL). At follow up, compared to baseline, the TG showed an increased FC of the ACC with the right middle frontal gyrus (MFG) and right IPL, while the CG showed a decreased FC of the ACC with the right cerebellum and right inferior temporal gyrus (ITG). A significant “treatment × time” interaction was found for the increased FC of the right IPL and for the decreased FC of the right ITG. In the TG only, significant correlations (p?PASAT performance and RS FC of the ACC with the right MFG (r?=?0.88) and right IPL (r?=?0.76). In MS, cognitive rehabilitation correlates with changes in RS FC of brain regions subserving the trained functions. fMRI might be useful to monitor rehabilitative strategies in MS.  相似文献

We observed the characteristics of white matter fibers and gray matter in multiple sclerosis patients, to identify changes in diffusion tensor imaging fractional anisotropy values following white matter fiber injury. We analyzed the correlation between fractional anisotropy values and changes in whole-brain gray matter volume. The participants included 20 patients with relapsing-remitting multiple sclerosis and 20 healthy volunteers as controls. All subjects underwent head magnetic resonance imaging and diffusion tensor imaging. Our results revealed that fractional anisotropy values decreased and gray matter volumes were reduced in the genu and splenium of corpus callosum, left anterior thalamic radiation, hippocampus, uncinate fasciculus, right corticospinal tract, bilateral cingulate gyri, and inferior longitudinal fasciculus in multiple sclerosis patients. Gray matter volumes were significantly different between the two groups in the right frontal lobe(superior frontal, middle frontal, precentral, and orbital gyri), right parietal lobe(postcentral and inferior parietal gyri), right temporal lobe(caudate nucleus), right occipital lobe(middle occipital gyrus), right insula, right parahippocampal gyrus, and left cingulate gyrus. The voxel sizes of atrophic gray matter positively correlated with fractional anisotropy values in white matter association fibers in the patient group. These findings suggest that white matter fiber bundles are extensively injured in multiple sclerosis patients. The main areas of gray matter atrophy in multiple sclerosis are the frontal lobe, parietal lobe, caudate nucleus, parahippocampal gyrus, and cingulate gyrus. Gray matter atrophy is strongly associated with white matter injury in multiple sclerosis patients, particularly with injury to association fibers.  相似文献

Biased recruitment and sample selection may cause variability in neuroimaging studies. Epidemiologically principled population-based magnetic resonance imaging (MRI) studies of schizophrenia are very rare. We gathered structural MRI data on 154 subjects from the Northern Finland 1966 Birth Cohort, aged 33–35 (100 controls, 54 schizophrenia patients). Regional differences in density of gray matter, white matter, and cerebrospinal fluid (CSF) were identified between groups using nonparametric statistical analysis, and the relationship of the regional differences to duration of illness was explored. Gray matter reductions were found bilaterally in the cerebellum, thalamus, basal ganglia, middle frontal gyrus, inferior frontal gyrus, precentral gyrus, insula, superior temporal gyrus, fusiform gyrus, parahippocampal gyrus, cuneus, and lingual gyrus; in the left posterior cingulate, superior frontal gyrus, transverse temporal gyrus, and precuneus; and in the right postcentral gyrus. Gray matter excesses were observed bilaterally in the basal ganglia, anterior cingulate, and medial orbitofrontal cortices. There were white matter deficits in an extensive network including inter- and intrahemispheric tracts bilaterally in the frontal, temporal, parietal, and occipital lobes, subcortical structures, cerebellum, and brain stem. CSF excesses were found bilaterally in the lateral ventricles, third ventricle, interhemispheric, and left Sylvian fissure. We replicated the previous findings of structural brain abnormalities in schizophrenia on a general population level. Gray and white matter deficits were associated with duration of illness suggesting either that developmental brain deficits relate to an earlier age of onset or that brain abnormalities in schizophrenia are progressive in nature.  相似文献

目的探讨~(18)F-FDG PET显像观察特发性快眼动睡眠期行为障碍(iRBD)患者脑葡萄糖代谢改变和iRBD脑葡萄糖代谢改变与病程间的相关性。方法纳入多导睡眠监测(PSG)确诊的iRBD患者20例(iRBD组)和年龄、性别匹配的健康对照者19例(对照组)。两组均行~(18)F-FDG PET脑显像。基于自动解剖标记模板将大脑划分为90个左右对称的脑区,计算各脑区葡萄糖代谢半定量值。对iRBD组和对照组各脑区葡萄糖代谢半定量值进行独立样本t检验;并对iRBD组脑葡萄糖代谢改变与病程行Pearson相关分析。结果 (1)与对照组比较,iRBD组的双侧背外侧额上回、双侧眶部额上回、双侧眶部额中回、双侧海马、双侧海马旁回、双侧杏仁核、左侧眶部额下回、左侧岛叶、左侧内侧与旁扣带脑回、左侧中央旁小叶、左侧苍白球的葡萄糖代谢半定量值均增高(P0.05);双侧距状裂周围皮质、双侧楔叶、双侧舌回、双侧枕上回、双侧枕中回、双侧枕下回、双侧角回、双侧颞上回、双侧颞中回、右侧颞横回的葡萄糖代谢半定量值均降低(P0.05)。Pearson相关分析结果,iRBD组双侧杏仁核、双侧颞上回、右侧楔叶、右侧枕上回、右侧颞横回、左侧海马、左侧颞中回的葡萄糖代谢半定量值与病程呈正相关(P0.05);而双侧眶部额上回、双侧眶部额中回、左侧中央旁小叶、左侧眶部额下回、左侧内侧和旁扣带回、右侧背外侧额上回、右侧海马旁回的葡萄糖代谢半定量值与病程呈负相关(P0.05)。结论 iRBD患者脑内存在疾病相关的葡萄糖代谢水平改变,有助于客观评估iRBD病情的变化。  相似文献
