目的 观察不同水平呼气末正压(PEEP)通气对肺内、外源性急性呼吸窘迫综合征(ARDS)患者的影响。方法 以16例早期肺内源性ARDS(A组)和12例早期肺外源性ARDS(B组)患者为研究对象,调整PEEP水平,分别监测在PEEP通气前和PEEP为5、10、15cmH2O通气后30min时的氧合、呼吸力学和血流动力学的变化。结果 PEEP5cmH2O时与PEEP通气前比较,A、B两组患者的动脉血氧分压(PaO2)/吸入气氧浓度(FiO2)、气道峰压(Ppeak)、气道平台压(Pplat)、平均动脉压(MAP)以及B组患者的呼吸系统顺应性(Crs)差异均无统计学意义(P均〉0.05);A组患者的Crs[PEEP5cmH2O时、PEEP通气前分别为(0.027±0.004)、(0.022±0.005)L/cmH2O]差异有统计学意义(P〈0.05)。PEEP10cmH2O时与PEEP通气前比较,A、B两组患者的PaO2/Fioz[PEEP10cmH2O时、PEEP通气前A组分别为(130.Off=30.6)、(81.6±26.7),B组分别为(137.3±28.9)、(73.6±30.8)]、Crs[PEEP10cmH2O时、PEEP通气前A组分别为(0.032±0.005)、(0.022±0.005)L/cmH2O,B组分别为(0.033±0.005)、(0.022±0.004)L/cmH2O]、MAP[PEEP10cmH2O时、PEEP通气前A组分别为(68.9±8.4)、(79.3±9.2)mEHg,B组分别为(69.0±6.2)、(77.5±8.7)mEHg]以及A组患者的Ppeak[PEEP10cmH2O时、PEEP通气前分别为(33.0±6.O)、(25.0±5.8)mEHg]和PplatFPEEP10cmH20时、PEEP通气前分别为(30.5±5.6)、(22.1±4.8)mEHg]差异均有统计学意义(P〈0.05或P〈0.01),其中B组患者PaO2/FiO2升高幅度、Crs增大幅度均较A组患者更显著(P均G0.05)。PEEP15cmHzO时与PEEP通气前比较,A、B两组患者的PaO2/FiO2[EEP15cmH2O时、PEEP通气前A组分别为(139.8±34.8)、(81.6±26.7),B组分别为(178.7±35.4)、(73.6±30.8)]、Crs[PEEP15cmH2O时、PEEP通气前A组分别为(0.030±0.005)、(0.022±0.005)L/cmH20,B组分别为(0.036±0.007)、(0.022±0.004)L/cmH201、MAP[PEEP15cmH2O时、PEEP通气前A组分别为(66.9±9.1)、(79.3±9.2)mEHg,B组分别为(66.3±5.2)、(77.5±8.7)mEHg]、Ppeak[PEEP15cmH2O时、PEEP通气前A组分别为(40.3±6.7)、(25.0±5.8)cmH2O,B组分别为(35.7±8.5)、(22.2±7.8)cmH2O]和Pplat[PEEP15cmH2O时、PEEP通气前A组分别为(36.9±5.6)、(22.1±4.8)cmH2O,B组分别为(29.2±6.8)、(18.7±5.6)cmH2O]差异均有统计学意义(P均〈0.01),其中B组患者PaO2/FiO2升高幅度、Crs增大幅度均较A组患者更显著(P均〈0.01),A组患者Pplat增高幅度较B组患者更显著(P〈0.01)。A、B两组患者MAP下降幅度比较差异无统计学意义(P〉0.05)。机械通气24h后A组2例出现轻度纵隔气肿。结论 PEEP通气能改善部分ARDS患者的氧合和呼吸系统顺应性,而且对肺外源性ARDS患者的改善可能要优于肺内源性ARDS患者。但高PEEP可能使Ppeak和Pplat显著增高,而且肺内源性ARDS患者Pplat的增高幅度可能要更显著于肺外源性ARDS患者。  相似文献

目的研究慢性阻塞性肺疾病(COPD)患者机械通气时对外源性呼气末正压的生理学反应规律,揭示反应个体化特点。方法随机选取2005年1月至2006年6月由急诊室收入我院呼吸科重症监护室的COPD急性加重期患者15例,仰卧位经口气管插管后接Evita 4呼吸机(德国Draeger公司),采用容量控制通气对受试者序贯给予相当于内源性呼气末正压水平20%、40%、60%、80%、100%、120%的外源性呼气末正压,每次给予外源性呼气末正压后均测量气道阻力、呼吸系统顺应性、气道平台压、总呼气末正压水平。根据施加外源性呼气末正压后患者气道平台压的变化,将15例患者分为正常反应组(11例),反常反应组(4例),对两组间各参数进行比较。结果15例患者当外源性呼气末正压为80%、内源性呼气末正压为100%水平时的气道阻力分别为(18.5±2.0)cmH2O·L^-1·s^-1(1cmH2O=0.098kPa)、(18.0±2.2)cmH2O·L^-1·s^-1,与外源性呼气末正压为0时的气道阻力[(23.0±2.9)cmH2O·L^-1·s^-1]比较差异有统计学意义(t值分别为5.36、6.27,P均〈0.01);当外源性呼气末正压为120%内源性呼气末正压水平时,气道阻力和总呼气末正压水平分别为(17.3±2.1)cmH2O·L^-1·s^-1、(12.7±2.2)cmH2O,与外源性呼气末正压为0时[(23.0±2.9)cmH2O·L^-1·s^-1、(10.0±1.1)cmH2O]比较差异有统计学意义(t值分别为6.79、-3.90,P均〈0.01)。正常反应组患者基础生理学参数(外源性呼气末正压为0时)内源性呼气末正压水平、气道阻力、呼吸系统顺应性、气道平台压分别为(10.0±1.0)cmH2O、(22.8±1.9)cmH2O·L^-1·s^-1、(39±6)ml/cmH2O、(20±4)cmH2O,与反常反应组[(10.0±1.4)cmH2O、(23.1±4.1)cmH2O·L^-1·s^-1、(42±9)ml/cmH2O、(21±3)cmH2O]比较差异无统计学意义(t值分别为0.03、0.10、0.60、0.15,P均〉0.05);正常反应组患者在外源性呼气末正压分别为40%、80%、100%、120%内源性呼气末正压时,△Ppla140,△Pplat80,△Pplat100,△Pplat120分别为(-0.020±0.970)cmH2O、(1.6±1.0)cmH2O、(4.0±2.9)cmH2O、(6.4±3.3)cmH2O,与反常反应组[(-7.500±0.920)cmH2O、(-4.4±1.4)cmH2O、(-3.8±1.9)cmH2O、(-1.6±1.2)cmH2O]比较差异有统计学意义(t值分别为-9.64、-5.90、-3.80、-3.92,P均〈0.01)。结论提示部分无自主呼吸的被动机械通气患者应用外源性呼气末正压是有益的,可以使患者气道平台压显著下降。  相似文献

目的探讨高碳酸血症对急性肺损伤模型的保护作用及可能的机制。方法24只新西兰兔按随机数字表法分为对照组、治疗组、预防组,每组8只。采用脂多糖静脉注射复制肺损伤模型,观察3组兔血流动力学、血气指标的变化;检测肺组织湿/干重(W/D)比、光镜、肺损伤组织学定量评价指标(IQA)来评估肺脏的损伤程度。通过对肺组织中髓过氧化物酶(MPO)、丙二醛(MDA)及血清和支气管肺泡灌洗液(BALF)中白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF—α)浓度及中性粒细胞凋亡率的测定,阐明高碳酸血症对肺损伤保护的可能机制。结果(1)治疗组、预防组在模型形成后平均动脉压、心率、动脉血二氧化碳分压(PaCO2)、动脉血氧分压/吸入气氧浓度(PaO2/FiO2)分别为(79±6)mm Hg(1mmHg=0.133kPa)、(180±10)次/min、(99±13)mmHg、250±26,(80±9)mmHg、(181±12)次/min、(95±11)mmHg、241±56,与对照组[(66±10)mmHg、(139±13)次/min、(31±4)mmHg、182±35]比较差异有统计学意义(t值分别为4.05、26.32、5.36、28.15、12.54、11.07、16.13、12.36。P均分别〈0.05、0.01);(2)治疗组、预防组W/D、NPO、MDA分别为1.98±0.28、1.87±0.30、(6.1±1.6)U/g、(5.8±1.5)U/g、(20±5)mg/L、(19±4)mg/L,与对照组[2.43±0.26、(9.0±1.3)U/g、(36±8)mg/L]比较差异有统计学意义(t值分别为11.07、24.46、2.35、9.63,12.34、25.32,P分别〈0.05、0.01);(3)治疗组、预防组血清和BALF中IL-8、TNF—α、中性粒细胞凋亡率分别为(50±8)ng/ml、(103±49)ng/ml、(94±16)ng/ml、(44±9)ng/ml、(38±9)%、(56±5)%、(49±7)ng/ml、(96±50)ng/ml、(91±14)ng/ml、(39±6)ng/ml、(39±10)%、(55±10)%,与对照组[(91±43)ng/ml、(177±60)ng/ml、(162±15)ng/ml、(67±7)ng/ml、(19±7)%、(43±7)%]比较差异有统计学意义(t值分别为7.12、5.55、7.30、3.93、13.08、8.00,P分别〈0.05、0.01);(4)各组血清及BALF中IL-8、TNF—α与中性粒细胞凋亡率呈负相关(r值分别为-0.73、-0.72、-0.52、-0.64、-0.73、-0.56、-0.57、-0.78、-0.69、-0.75、-0.82、-0.84,P均〈0.05)。结论高碳酸血症对急性肺损伤具有保护作用,对血流动力学无明显影响。  相似文献

【摘要】 目的 探讨容量控制通气(VCV)和压力控制通气(PCV)对DeBakeyⅠ型主动脉夹层手术体外循环(CPB)后气道压力和动脉血气的影响。方法 60例全麻下行DeBakeyⅠ型主动脉夹层手术的患者随机分为2组(A组和B组,每组30例)。所有患者麻醉诱导气管插管后均采用传统的VCV模式。CPB后A组继续采用VCV模式,30 min后转换为PCV模式;B组则先采用PCV模式,30 min后转换为VCV模式。分别在每种通气模式结束后行动脉血气分析,同时记录气道压力、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)及动脉血氧饱和度(SaO2)。结果 与CPB前VCV模式相比,CPB后VCV和PCV模式下PaO2(179.4±100.4和235.7±121.2 mmHg)下降,平均气道压(Pmean)(10.4±2.2和8.7±1.8 cmH2O)和气道峰压(Ppeak)(20.2±3.6和18.3±3.0 cmH2O)升高 (P<0.05);与CPB后VCV模式相比,CPB后PCV模式下Pmean(8.7±1.8 cmH2O)和Ppeak(18.3±3.0 cmH2O)降低(P<0.05),而PaO2(235.7±121.2 mmHg)升高(P<0.05);与CPB前VCV模式相比,CPB后VCV和PCV模式下PaCO2、SaO2均无统计学差异(P>0.05)。结论 DeBakeyⅠ型主动脉夹层手术CPB后,与VCV模式相比,PCV模式下能提高氧合作用,且气道压力低,可防止加重气道损伤。  相似文献

目的建立大鼠离体胰腺灌注技术。方法采用大鼠离体胰腺灌注技术对10只高脂肥胖大鼠胰岛13细胞功能进行研究。结果6只大鼠符合胰腺整体灌注胰腺完整性的评定标准:(1)在整个灌注期间灌注压保持在(70±5)mmHg(1mmHg=0.133kPa)。(2)灌注结束后检测分离的胰腺十二指肠组织块,均存在十二指肠蠕动活性。(3)持续精氨酸刺激较持续高糖刺激胰岛素分泌幅度明显增高[最大胰岛素分泌率:(987±100)μU/min vs (545±50)μU/min,P〈0.05;平均胰岛素分泌率:(544±73)μU/min vs (260±28)μU/min,P〈0.05]。结论成功建立了大鼠胰腺整体灌注技术,此技术可用于大鼠离体胰岛β细胞功能研究。  相似文献

目的观察支气管哮喘(简称哮喘)患者不同气道炎症类型(嗜酸粒细胞炎症与非嗜酸粒细胞炎症)对吸人糖皮质激素(简称激素)治疗的反应性。方法选择近3个月内未接受激素治疗的慢性持续性哮喘患者42例,根据诱导痰嗜酸粒细胞百分比将患者分为嗜酸粒细胞增高组(嗜酸粒细胞〉3%,23例)与非嗜酸粒细胞增高组(嗜酸粒细胞〈3%,19例),进行哮喘症状评分、肺功能检查、诱导痰细胞分类计数。采用酶联免疫荧光法测定痰液嗜酸粒细胞阳离子蛋白(ECP)的浓度。吸人激素治疗1、3个月时进行随访,重复上述哮喘疗效的评价指标。结果嗜酸粒细胞增高组痰嗜酸粒细胞比值、ECP水平基线值分别为0.080(0.063~0.178)、(324±149)μg/L,非嗜酸粒细胞增高组分别为0.017(0.006~0.021)、(152±68)μg/L,两组比较差异有统计学意义(t值分别为4.40、3.33,P均〈0、01)。嗜酸粒细胞增高组第一秒用力呼气容积(FEV1)、FEV1占预计值百分比(FEV1占预计值%)、症状评分的基线值分别为(1.98±0.67)L、(65±20)%、7.0(5.0~10.0)分,非嗜酸粒细胞增高组分别为(2.07±1.05)L、(66±27)%、5.0(2.0~9.0)分,两组比较差异无统计学意义(t值分别为-0.62、-0.09、1.32,P均〉0.05)。吸入激素治疗1、3个月后嗜酸粒细胞增高组的嗜酸粒细胞比值、ECP水平、症状评分、FEV1和FEV1占预计值%分别为[0.019(0.010~0.060)、[0.036(0.006~0.070)、(173±153)μg/L、(173±122)μg/L、3.0(1.0~6.0)分、3.0(1.0~5.0)分、(2.42±0.64)L、(2.43±0.76)L、(77±13)%、(77±18)%,与基线值比较差异有统计学意义(F值分别为6.73、6.71、5.93、7.38、5.78,P均〈0.05)。非嗜酸粒细胞增高组分别为0.013(0.000~0.025)、0.012(0.004~0.031)、(111±50)μg/L、(117±50)μg/L、3.0(0.0~6.0)分、3.0(1.0~7.3)分、(2.22±0.86)L、(2.21±0.24)L、(71±20)%、(65±21)%,与基线值比较,嗜酸粒细胞比值、FEV1和FEV1占预计值%均无统计学意义(F值分别为1.98、0.80、1.37,P均〉0.05),而ECP水平和症状评分比较差异有统计学意义(F值分别为3.78、3.59,P均〈0.05)。多元线性回归分析显示,病情严重度、基线FEV1和痰嗜酸粒细胞百分比与FEV1改善程度相关(r值分别为-0.27、-0.02、0.03,P均〈0.05)。痰嗜酸粒细胞比值的基线值对激素治疗反应的阴性预测值最高(89.5%)。结论对于痰嗜酸粒细胞增高的哮喘患者,吸人激素治疗能够抑制嗜酸粒细胞炎症,改善肺功能和症状评分。非嗜酸粒细胞增高哮喘患者对激素治疗反应性差,FEV1无改善。痰嗜酸粒细胞比值不高是激素治疗反应性差的最佳预测指标。  相似文献

目的探索与评估家庭无创正压机械通气(HNPPV)长期治疗重度慢性阻塞性肺疾病(COPD)稳定期患者的疗效和安全性。方法将40例经住院治疗处于稳定期的重度COPD患者[动脉血二氧化碳分压(PaCO2)≥55mmHg(1mmHg=0.133kPa)]分为常规治疗+HNPPV组(治疗组)20例和常规治疗组(对照组)20例。分别记录观察前(治疗前)、观察2年后的呼吸困难分级评分、辅助呼吸肌评分、心理情绪评分、肺功能、动脉血气、6分钟行走距离(6MWD);采用超声心动图测定肺动脉平均压(mPAP)、病死率及每年住院次数等指标。结果治疗组与对照组的年龄、性别、身高、体重、呼吸困难分级评分、辅助呼吸肌评分、焦虑评分、抑郁评分、动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、6MWD、mPAP、每年住院次数均具有可比性(t值分别为1.08、1.15、1.20、1.09、0.86、0.54、0.00、0.00、0.43、0.96、0.76、0.38、0.26、0.24、0.87,P均〉0.05)。2年后治疗组死亡3例(3/20),对照组死亡8例(8/20),两组比较差异无统计学意义(χ^2=3.27,P〉0.05)。2年后治疗组呼吸困难分级评分、辅助呼吸肌评分、焦虑评分、抑郁评分、PaCO2、PaO2、6MWD、mPAP、每年住院次数分别为(2.4±0.5)分、(2.6±0.6)分、(6.9±2.1)分、(6.1±1.6)分、(49.5±2.2)mmHg、(60.8±4.7)mmHg、(213±45)m、(30.3±2.2)mmHg、(1.4±0.4)次/年,与对照组[(3.9±0.3)分、(4.8±0.4)分、(11.2±2.6)分、(11.6±2.1)分、(61.5±2.3)mmHg、(52.8±2.4)mmHg、(127±23)m、(36.4±2.3)mmHg、(3.9±0.3)次/年]比较差异均有统计学意义(t值分别为9.53、10.83、4.92、7.83、14.07、5.41、6.07、4.81、10.22,P均〈0.01)。结论长期家庭无创正压机械通气对有选择性稳定期重度COPD患者疗效较为肯定。  相似文献

目的分析冠状动脉的病变支数及狭窄程度与周围动脉弹性功能的关系。方法对88例高血压病患者与41例无高血压病临床怀疑冠心病的患者,在冠状动脉造影前后采用美国FDA批准的PULSEMETRIC动脉功能测定仪通过测定肱动脉脉搏图计算出反应血管弹性的参数,包括系统血管顺应性(SVC)、系统血管阻力(SVR)、肱动脉顺应性(BAC)及肱动脉阻力(BAR),分析高血压伴及不伴冠状动脉病变者的动脉弹性的特点。结果(1)高血压组严重冠状动脉病变(冠状动脉病变〉12支以上)发生率[64.7%(57/88)]高于血压正常组[27.1%(11/41),P〈0.05);(2)高血压组的SVC[(0.85±0.10)ml/mmHg(1mmHg=0.133kPa)]、BAC[(0.047±0.011)ml/mmHg]明显低于非高血压组[SVC(1.17±0.11)ml/mmHg,BAC(0.063±0.010)ml/mmHg,均P〈0.05],高血压组的脉压则明显高于非高血压组[(78±20)mmHg比(47±19)ml/mmHg,P〈0.01];(3)高血压组内,动脉弹性有随冠状动脉病变加重而下降的趋势,而动脉弹性功能参数在不同的性别存在不同的变化;(4)当血压水平达到2~3级,SVC有随冠状动脉病变程度加重而降低的趋势。结论在高血压病患者中,无创方法测得的动脉功能参数在一定程度上可以反映其冠状动脉病变程度。  相似文献

背景既往报告异莲心碱是从莲子心中分离的一种双苄基喹啉生物碱单体,具有抗心律失常、Ca^2+拮抗及阻断α受体作用,对高血压左室肥厚有不同程度的改善。高血压左室肥厚心肌肌浆网钙泵(SERCA)活力较正常心肌降低。目的探讨异莲心碱对高血压大鼠左室肥厚及SERCA活力的影响。方法将二肾一夹肾血管性高血压大鼠(RHR)模型,随机分为3组:正常对照组、肾血管性高血压大鼠对照组(未治疗RHR组)和异莲心碱治疗组。在异莲心碱治疗组持续给药10周后,分别测定各组大鼠的血压、左室质量/体质量,以及左室心肌SERCA活力。结果治疗后,异莲心碱治疗组血压(136.4±14.6)mmHg较未治疗RHR组(189.8±4.4)mmHg显著降低(P〈0.01);异莲心碱治疗组左室质量/体质量(2.23±0.43)也较未治疗RHR组(2.93±0.52)显著降低(P〈0.05);异莲心碱治疗组左室心肌SERCA活力[(0.91±0.18)μmol/(gprotein·min)]较未治疗RHR组[(0.61±0.23)μmol/(gprotein·min)]显著升高(P〈0.05),但仍较正常对照组[(1.32±0.18)μmol/(gprotein·min)]低(P〈0.01)。结论异莲心碱能降低RHR的血压,减低RHR的左室质量/体质量,对高血压左室肥厚具有一定的防治作用;其机制可能与异莲心碱能升高RHR肥厚心肌肌浆网SERCA活力,改善心肌细胞内钙超载有关。  相似文献

目的探讨分泌性白细胞蛋白酶抑制物(SLPI)在慢性阻塞性肺疾病(COPD)大鼠支气管肺组织中的表达及转化生长因子β1(TGF-β1)的影响。方法采用熏香烟加气管内注射脂多糖复制大鼠COPD模型,并用TGF-β1单抗干预,测定大鼠的肺功能及病理变化结果,应用酶联免疫法检测支气管肺泡灌洗液(BALF)中SLPI水平,用免疫组织化学法观察支气管肺组织TGF—β1、Smad4、SLPI的表达,用逆转录聚合酶链法检测支气管肺组织中TGF—β1、Smad4和SLPI mRNA的表达。计量资料比较采用单因素方差分析,两样本均数的多重比较采用t检验,等级资料采用秩转换的非参数检验。结果模型组大鼠SLPI阳性系数(1.07)、SLPImRNA吸光度值(0、17±0.01)和BALF中SLPI浓度[(47±4)μg/L]比正常对照组大鼠[(3.86)、(0.84±0.10)和(82±7)μg/L]明显降低;TGF-β1阳性系数(3.91)、TGF-β1 mRNA吸光度值(0.71±0.09)比正常对照组大鼠[(1、12)、(0.15±0.01)]显著增高。经TGF-β1单抗干预后,干预组大鼠SLPI阳性系数(2.69)、SLPI mRNA吸光度值(0.59±0.05)和BALF中SLPI浓度[(69±6)μg/L]比模型组大鼠[(1.07)、(0.17±0.01)和(47±4)μg/L]有明显提高。呼气峰流量[(28±6)ml/s]、第0、3秒用力呼气容积[(4.4±1.3)m1]和第0.3秒用力呼气容秽用力肺活量[(80±10)%]比模型组大鼠[(23±5)ml/s、(3.3±1.4)ml和(62±9)%]有显著改善。结论COPD大鼠支气管肺组织中SLPI明显减少,可能是TGF-β1表达增加所致,其机制可能通过Smads信号转导通路介导。  相似文献

目的 观察不同步分侧肺通气和同步分侧肺通气对单侧急性肺损伤(ALI)犬循环的影响.方法 取健康杂种犬12只,建立盐酸所致单侧肺损伤动物模型,行容积控制通气,将犬按随机数字表法分为不同步分侧肺通气组(NS组)和同步分侧肺通气组(S组).参数:患侧潮气量3.5 ml/kg保持不变,呼气末正压(PEEP)选择15、20、25 cm H2O(1 cm H2O=0.098 kPa);患侧PEEP 10 cm H2O不变,潮气量用随机数字表法选择5、7.5、10 ml/kg.健侧通气参数始终不变,检测不同通气条件下两组犬血流动力学和氧动力学指标.结果 (1)患侧潮气量3.5 ml/kg不变,PEEP为15、20 cm H2O时,两组血流动力学和氧动力学参数差异无统计学意义.当患侧PEEP为25 cm H2O时,NS组心率、体循环平均压(mABP)、心输出量、氧合指数和混合静脉血氧饱和度(SvO2)分别为(98±8)次/min、(84±6)mm Hg(1 mm Hg=0.133 kPa)、(1.10±0.13)L/min、(199±14)mm Hg和(55±6)%,明显低于S组[分别为(124±9)次/min、(103±7)mm Hg、(1.52±0.28)L/min、(221±15)mm Hg和(62±4)%,t值分别为-7.852、-16.561、-15.043、-13.314和-5.653,均P<0.01].(2)患侧PEEP 10 cm H2O不变,潮气量分别为5、7.5 ml/kg时,两组的血流动力学和氧动力学参数比较差异无统计学意义.当患侧潮气量为10 ml/kg时,NS组HR、mABP、心输出量、氧合指数和SvO2均低于S组(均P<0.01).结论 在本实验动物模型中,患侧与健侧所用PEEP水平相差≤20 cm H2O或患侧潮气量≤7.5 ml/kg时,同步和非同步分侧肺通气均能保持循环稳定.若需要更高水平PEEP时,建议选用同步分侧肺通气.  相似文献

Protective effects of hypercapnic acidosis on ventilator-induced lung injury.   总被引:12,自引:0,他引:12  
To investigate whether respiratory acidosis modulates ventilator-induced lung injury (VILI), we perfused (constant flow) 21 isolated sets of normal rabbit lungs, ventilated them for 20 min (pressure controlled ventilation [PCV] = 15 cm H(2)O) (Baseline) with an inspired CO(2) fraction adjusted for the partial pressure of CO(2) in the perfusate (PCO(2) approximately equal to 40 mm Hg), and then randomized them into three groups. Group A (control: n = 7) was ventilated with PCV = 15 cm H(2)O for three consecutive 20-min periods (T1, T2, T3). In Group B (high PCV/normocapnia; n = 7), PCV was given at 20 (T1), 25 (T2), and 30 (T3) cm H(2)O. The targeted PCO(2) was 40 mm Hg in Groups A and B. Group C (high PCV/hypercapnia; n = 7) was ventilated in the same way as Group B, but the targeted PCO(2) was approximately equal to 70 to 100 mm Hg. The changes (from Baseline to T3) in weight gain (Delta WG: g) and in the ultrafiltration coefficient (Delta K(f) = gr/min/ cm H(2)O/100g) and the protein and hemoglobin concentrations in bronchoalveolar lavage fluid (BALF) were used to assess injury. Group B experienced a significantly greater Delta WG (14.85 +/- 5.49 [mean +/- SEM] g) and Delta K(f) (1.40 +/- 0.49 g/min/cm H(2)O/100 g) than did either Group A (Delta WG = 0.70 +/- 0.43; Delta K(f) = 0.01 +/- 0.03) or Group C (Delta WG = 5.27 +/- 2.03 g; Delta K(f) = 0.25 +/- 0.12 g/min/cm H(2)O/ 100 g). BALF protein and hemoglobin concentrations (g/L) were higher in Group B (11.98 +/- 3.78 g/L and 1.82 +/- 0.40 g/L, respectively) than in Group A (2.92 +/- 0.75 g/L and 0.38 +/- 0.15 g/L) or Group C (5.71 +/- 1.88 g/L and 1.19 +/- 0.32 g/L). We conclude that respiratory acidosis decreases the severity of VILI in this model.  相似文献

目的探讨全氟化碳(PFC)雾化吸入对急性呼吸窘迫综合征(ARDS)猪气体交换、呼吸力学和血流动力学的影响。方法16只通过气管内吸入十六烷磺基丁二酸钠(DTG)制作的ARDS模型猪随机分为PFC治疗组和对照组。PFC治疗组在常规机械通气基础上,连接雾化器,以纯氧(氧流速3L/min)作为驱动气流推动雾化PFC治疗2h(7~8ml·kg-1·h-1);对照组在常规机械通气基础上,连接雾化器单独给予纯氧治疗2h(氧流速3L/min),在治疗期间每间隔15min测定动脉血气、血压、心率及平台压、顺应性、呼出潮气量和内源性呼气末正压(PEEPi),观察两组动物气体交换、呼吸力学和血流动力学参数的变化。结果DTG气管内吸入1h后,PFC治疗组动脉血氧分压(PaO2)从(377±55)mmHg(1mmHg=0.133kPa)下降至(56±13)mmHg[吸入气氧浓度(FiO2)100%],对照组PaO2从(383±53)mmHg下降至(49±12)mmHg(FiO2100%);PFC治疗组顺应性从(3.7±1.0)ml/cmH2O下降至(1.5±0.4)ml/cmH2O,对照组顺应性从(3.8±0.7)ml/cmH2O下降至(1.4±0.4)ml/cmH2O。PFC治疗2h后PFC治疗组PaO2上升至(189±133)mmHg,顺应性改善至(4.1±1.4)ml/cmH2O,呼出潮气量增加至(74.5±16.9)ml;对照组PaO2上升至(83±51)mmHg,顺应性改善至(2.8±1.8)ml/cmH2O,呼出潮气量增加至(50.1±4.1)ml;两组比较差异均有统计学意义(P均<0.05)。两组动物pH值、心率、血压、平台压、PEEPi和动脉血二氧化碳分压(PaCO2)比较差异均无统计学意义(P均>0.05)。结论PFC雾化吸入能明显改善ARDS家猪的氧合,提高损伤肺的顺应性,增加呼出潮气量。  相似文献

目的探讨肺泡复张(RM)后再萎陷的机制以及呼气末正压(PEEP)和潮气量(VT)的调节策略。方法健康杂种犬18只,建立油酸所致急性呼吸窘迫综合征(ARDS),行容量控制通气(VCV)、PEEP 16 cm H2O、VT10 m l/kg、通气频率(RR)30次/m in,稳定后作为基础状态(0 m in)。以压力控制通气[气道峰压(PIP)50 cm H2O,PEEP 35 cm H2O,持续60 s]行RM,然后随机分为小VT中等PEEP组(LVMP组,VT10 m l/kg、PEEP 16 cm H2O、RR 30次/m in),小VT低PEEP组(LVLP组,VT10 m l/kg、PEEP 10 cm H2O、RR 30次/m in)和中等VT低PEEP组(MVLP组,VT15 m l/kg、PEEP 10cm H2O、RR 20次/m in)。观察4 h后处死动物,行支气管肺泡灌冼。监测氧合、呼吸力学、血流动力学及肺损伤指标。结果(1)LVMP、LVLP、MVLP组低位拐点(LIP)分别为(16.0±1.3)、(15.8±3.0)、(16.3±1.9)cm H2O。(2)在RM后30、60 m in,LVMP组动脉血氧分压(PaO2)[(371±64)、(365±51)mm Hg]显著高于LVLP组[(243±112)、(240±108)mm Hg]及MVLP组[(242±97)、(232±87)mm Hg,P均<0.05],但直至RM后4 h 3组比较差异无统计学意义;LVLP与MVLP组在RM后各个时间点的PaO2与基础状态比较差异均无统计学意义;MVLP组的通气功能较其他两组显著改善。(3)与基础状态比较,RM后LVMP组平均动脉压(mABP)显著降低,平均肺动脉压(mPAP)显著增加,而其他两组mABP保持稳定,mPAP降低。(4)与基础状态比较,3组PIP和气道平台压(Pp lat)在RM后均显著降低,呼吸系统静态顺应性(Cst)显著改善。在RM后同一时间点比较,MVLP组PIP、Pp lat和Cst均显著好于LVMP组。MVLP组与LVLP组相比,Cst有增加趋势。(5)在相同部位的支气管肺泡灌冼液中,肺损伤指标在各组之间无显著差异。结论与LIP相近的高PEEP有助于防止复张肺泡的再萎陷,但对血流动力学和呼吸力学产生不利影响;早期应用RM能有效“节约”PEEP,并为上调VT提供了较肺泡复张之前更大的空间。  相似文献

STUDY OBJECTIVES: The potential clinical benefits of pressure-controlled ventilation (PCV) over volume-controlled ventilation (VCV) in patients with acute lung injury (ALI) or ARDS still remain debated. We compared PCV with VCV in patients with ALI/ARDS with respect to the following physiologic end points: (1) gas exchange and airway pressures, and (2) CT scan intrapulmonary gas distribution at end-expiration. DESIGN: Prospective, observational study. SETTING: A multidisciplinary ICU in a nonuniversity, acute-care hospital. PATIENTS: Ten patients with ALI or ARDS (9 men and 1 woman; age range, 17 to 80 years). INTERVENTIONS: Sequential ventilation in PCV and VCV with a constant inspiratory/expiratory ratio, tidal volume, respiratory rate, and total positive end-expiratory pressure; measurement of gas exchange and airway pressures; and achievement of CT sections at lung base, hilum, and apex for the quantitative analysis of lung densities and of aerated vs nonaerated zones. RESULTS: PaO(2), PaCO(2), and PaO(2)/fraction of inspired oxygen ratio levels did not differ between PCV and VCV. Peak airway pressure (Ppeak) was significantly lower in PCV compared with VCV (26 +/- 2 cm H(2)O vs 31 +/- 2 cm H(2)O; p < 0.001; mean +/- SEM). The surface areas of the nonaerated zones as well as the total areas at each section level were unchanged in PCV compared with VCV, except at the apex level, where there was a significantly greater nonaerated area in VCV (11 +/- 2 cm(2) vs 9 +/- 2 cm(2); p < 0.05). The total mean CT number of each lung (20 lungs from 10 patients) was similar in the two modes, as were the density values at the basal and apical levels; the hilum mean CT number was - 442 +/- 28 Hounsfield units (HU) in VCV and - 430 +/- 26 HU in PCV (p < 0.005). CONCLUSIONS: These data show that PCV allows the generation of lower Ppeaks through the precise titration of the lung distending pressure, and might be applied to avoid regional overdistension by means of a more homogeneous gas distribution.  相似文献

STUDY OBJECTIVES: To investigate the effects of endotracheal suction in volume-controlled ventilation (VCV) and pressure-controlled ventilation (PCV) with an open suction system (OSS) or a closed suction system (CSS). DESIGN: Randomized comparison. SETTING: Animal research laboratory. PATIENTS: Twelve healthy anesthetized pigs. INTERVENTIONS: The effects of endotracheal suction during VCV and PCV with tidal volume (VT) of 14 mL/kg were compared. A 60-mm inner-diameter endotracheal tube was used. Ten-second suction was performed using OSS and CSS with 12F and 14F catheters connected to - 14 kPa vacuum. MEASUREMENTS AND RESULTS: Thirty minutes after suction in PCV, VT was still decreased by 27% (p < 0.001), compliance (Crs) by 28% (p < 0.001), and PaO(2) by 26% (p < 0.001); PaCO(2) was increased by 42% (p < 0.0001) and venous admixture by 158% (p = 0.003). Suction in VCV affected only Crs (decreased by 23%, p < 0.001) and plateau pressure (increased by 24%, p < 0.001). The initial impairment of gas exchange following suction in VCV was no longer statistically significant after 30 min. CONCLUSIONS: In conclusion, endotracheal suction causes lung collapse leading to impaired gas exchange, an effect that is more severe and persistent in PCV than in VCV.  相似文献

Biologically variable mechanical ventilation (Vbv)-using a computer-controller to mimic the normal variability in spontaneous breathing-improves gas exchange in a model of severe lung injury (Lefevre, G. R., S. E. Kowalski, L. G. Girling, D. B. Thiessen, W. A. C. Mutch. Am. J. Respir. Crit. Care Med. 1996;154:1567-1572). Improved oxygenation with Vbv, in the face of alveolar collapse, is thought to be due to net volume recruitment secondary to the variability or increased noise in the peak inspiratory airway pressures (Ppaw). Biologically variable noise can be modeled as an inverse power law frequency distribution (y approximately 1/f(a)) (West, B. J., M. Shlesinger. Am. Sci. 1990;78:40-45). In a porcine model of atelectasis-right lung collapse with one-lung ventilation-we studied if Vbv (n = 7) better reinflates the collapsed lung compared with conventional monotonously regular control mode ventilation (Vc; n = 7) over a 5-h period. We also investigated the influence of sigh breaths with Vc (Vs; n = 8) with this model. Reinflation of the collapsed lung was significantly enhanced with Vbv-greater Pa(O(2)) (502 +/- 40 mm Hg with Vbv versus 381 +/- 40 mm Hg with Vc at 5 h; and 309 +/- 79 mm Hg with Vs; mean +/- SD), lower Pa(CO(2)) (35 +/- 4 mm Hg versus 48 +/- 8 mm Hg and 50 +/- 8 mm Hg), lower shunt fraction (9.7 +/- 2.7% versus 14.6 +/- 2.0% and 22.9 +/- 6.0%), and higher respiratory system compliance (Crs) (1.15 +/- 0.15 ml/cm H(2)O/kg versus 0.79 +/- 0.19 ml/cm H(2)O/kg and 0.77 +/- 0.13 ml/cm H(2)O/kg)-at lower mean Ppaw (15.7 +/- 1.4 cm H(2)O versus 18.8 +/- 2.3 cm H(2)O and 18.9 +/- 2.8 cm H(2)O). Vbv resulted in an 11% increase in measured tidal volume (VT(m)) over that seen with Vc by 5 h (14.7 +/- 1.2 ml/kg versus 13. 2 ml/kg). The respiratory rate variability programmed for Vbv demonstrated an inverse power law frequency distribution ( y approximately 1/f(a)) with a = 1.6 +/- 0.3. These findings provide strong support for the theoretical model of noisy end-inspiratory pressure better recruiting atelectatic lung. Our results suggest that using natural biologically variable noise has enhanced the performance of a mechanical ventilator in control mode.  相似文献

Increased awareness of pressure-related injury to the alveolar-capillary interface has renewed interest in modes of ventilation that limit alveolar distention such as pressure-controlled ventilation (PCV). We examined respiratory system mechanics and gas exchange during PCV in six dogs. Our data conformed to the predictions of our single-compartment mathematical model of respiratory dynamics during PCV (J Appl Physiol 1989; 67:1081-92). For a fixed pressure (Pset) and inspiratory time fraction (Tl/Ttot) (15 cm H2O and 0.3, respectively), minute ventilation (VE) reached a well-defined plateau as frequency (f) increased from 10 to 50 breaths/min and tidal volume (VT) fell progressively. Concomitantly, the physiologic dead-space fraction (VD/VT) increased from 0.50 +/- 0.04 to 0.85 +/- 0.04, and arterial PCO2 (PaCO2) rose from 39 +/- 4 to 76 +/- 12 mm Hg. At a fixed combination of frequency, applied pressure, and Tl/Ttot (40 breaths/min, 15 cm H2O, and 0.3), VE did not change when we introduced fresh gas continuously from an intratracheal catheter. However, PaCO2 and VD/VT fell progressively as catheter flow increased from zero to 14 L/min (60 +/- 12 to 40 +/- 12 mm Hg and 0.83 +/- 0.03 to 0.25 +/- 0.14 mm Hg, respectively). We conclude that during PCV at a fixed Pset and Tl/Ttot increasing frequency caused VT to fall and VE to reach a plateau. Declining VT was associated with a rise in PaCO2 because of a subsequent fall in alveolar ventilation. A randomized, double-blind, placebo-controlled protocol was used to determine whether milrinone exerts an immediate effect on exercise performance in patients with severe congestive heart failure. In each of 14 patients with New York Heart Association class III or IV congestive heart failure, intravenous milrinone (mean 57 +/- 5 micrograms/kg) and placebo were randomly administered just before maximal progressive upright cycle ergometry. The duration of exercise was significantly longer with milrinone than with placebo treatment (placebo 11.0 +/- 0.6 minutes, milrinone 12.5 +/- 0.9 minutes, p = 0.01). Compared with placebo, milrinone caused a higher peak oxygen uptake (placebo 10.8 +/- 0.6 ml/kg/min, milrinone 12.4 +/- 0.7 ml/kg/min, p = 0.001) and oxygen uptake at the anaerobic threshold (placebo 7.8 +/- 0.4 ml/kg/min, milrinone 9.2 +/- 0.4 ml/kg/min, p = 0.001). At peak exercise intensity, systolic blood pressure (placebo 119 +/- 5 mm Hg, milrinone 131 +/- 5 mm Hg, p = 0.001) and heart rate (placebo 114 +/- 5 beats/min, milrinone 126 +/- 6 beats/min, p = 0.001) were both increased with milrinone. Likewise, at matched submaximal exercise intensities, heart rate (placebo 111 +/- 19 beats/min, milrinone 117 +/- 20 beats/min, p less than 0.05) and systolic blood pressure (placebo 116 +/- 19 mm Hg, milrinone 121 +/- 19 mm Hg, p = 0.04) were higher with milrinone; plasma norepinephrine (placebo 1,692 +/- 208 ng/liter, milrinone 1,320 +/- 216 ng/liter, p = 0.05) and blood lactate concentrations (placebo 2.2 +/- 0.2 mM, milrinone 1.9 +/- 0.2 mM, p less than 0.05) were lower.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献
