Approximately 36,000 people in the United States will be diagnosed with oral or oropharyngeal cancer in 2010. In more than 90% of the cases of oral cavity and oropharyngeal cancers, histopathologic examination reveals squamous cell carcinoma (SCCA). Despite appropriate initial treatment, oral SCCA recurs in 25% to 48% of cases. Several studies have shown that most cancers recur within about 2 years after the initial treatment. Hence, close follow-up of patients is important for the timely detection of recurrences.This article discusses the types of recurrence and the surveillance and treatment of recurrent oral SCCA.  相似文献

The most frequent malignant tumour in the head and neck region this area is the squamous cell carcinoma of the upper aerodigestive tract. In the Netherlands the incidence of these tumours is low, about 2000 new cases a year. Multidisciplinary treatment of these tumours is mandatory because of the extension of the lesion in the area of other disciplines. The variety of diagnostic and therapeutic possibilities and the supportive care ask for a multidisciplinary approach. The low incidence of head and neck cancer in the Netherlands and the complexity of the treatment of these tumours were the reason to concentrate the care for these patients in multi disciplinary head and neck cancer centres.  相似文献

Chemotherapy may be either an effective alternative to surgery or a valuable adjunct to surgery and/or radiotherapy in the treatment of cancer. The care of medical oncology patients is a multidisciplinary effort. Oral health care providers can expect to be called on to care for patients with cancer undergoing chemotherapy. Early, active participation in developing preventive and therapeutic strategies, in implementing the plan, and in the education and rehabilitation of patients is paramount in addressing quality of life issues. To provide timely and competent care, oral health care providers must understand the disease, its treatment, and the impact the disease and/or its treatment may have on these patients. Oral health care providers should develop and implement preventive and therapeutic strategies with the same ethical, moral, and professional standards of care as may be appropriate in the management of any other patient.  相似文献

The management of a patient with a tumour of the musculoskeletal system has progressed over recent decades. The development of evidence based medicine is a key factor in driving such change. Although now predominantly treated within specialist centres, all orthopaedic surgeons require an understanding of such malignancies and the management options available to the patients affected. We review the literature in three main areas; soft-tissue sarcoma (STS), primary malignant bone tumours and skeletal metastases. We take a critical look at the papers which have helped shape modern practice, and offer discussion into their relevance. We have given a historical perspective to demonstrate key themes in tumour surgery, and given examples of on-going research to provide an understanding of current developments.  相似文献

Childhood neoplasias have become increasingly important in recent years in the ambit of paediatric medicine. This phenomenon has been accompanied by a spectacular improvement in the treatment of childhood cancer, long-term survival rates reaching 90% in the case of some tumours. A corollary of this success is the obligation to provide new and improved medical assistance both as regards the possible prevention of any alterations and, if possible, the avoidance of complications derived from the neoplasm itself and its treatment. Among possible secondary effects are oral manifestations of a chronic or acute nature, which may cause great discomfort, act as foci of systemic infections or have long-term after effects, all of which will depend on the exact moment of the child s development that treatment is undertaken. The incidence and severity of most oral complications is associated with pre-existent factors, such as caries, gingivitis or generally poor hygiene, which strongly affect the beginning, increase and persistence of the same. It is to be decried that a problem in the buccal cavity is allowed to develop, which a simple preventative measure, simple hygiene or dental conservation treatment could prevent or reduce.  相似文献

As the survival rates for pediatric oncology patients continue to increase, more dentists will be faced with providing the necessary dental treatment to these complex dental problems. The dental resolution to the oral presentations of these disease conditions needs to be done in an understanding, supportive environment. The oral cavity and its relationship to both the child's positive self-image and quality of life presents the dental professional with a unique opportunity to enrich the human condition.  相似文献

This study investigated the oral problems occurring in children receiving treatment for malignant disease at a regional oncology center. Forty-three children 2 to 14 years old were followed longitudinally from initial diagnosis for periods ranging from eight to 30 weeks for the development of oral and dental problems. Fifteen children had untreated decay; two required the removal of primary teeth before they began chemotherapy. Three children developed acute dental infections during treatment. Forty (93%) developed oral problems associated with their disease or treatment during the study period. Oral mucosal ulceration was the most frequently encountered problem; it was observed in 28 (65%) patients. In light of the high prevalence of dental and oral problems in these patients, this study emphasizes the need for positive dental involvement, both in pretreatment assessment and in the ongoing care of the pediatric oncology patient.  相似文献

目的:了解高影响因子肿瘤学外文期刊发表的口腔肿瘤学论文情况,为我国口腔肿瘤研究人员的论文投稿提供参考信息。方法:对2002年影响因子在1.000以上的肿瘤学外文期刊近5年(1998~2002年)发表的口腔肿瘤学论文进行检索分析。结果:共有40种影响因子在1.000以上的外文期刊发表了5篇以上口腔肿瘤学论文。结论:我国口腔肿瘤研究人员应深入了解这些期刊及所发表的口腔肿瘤学研究论文的特点,并有针对性地向高影响因子肿瘤学外文期刊投稿。  相似文献

The aim of this study was to determine a functional cut-off point for trismus in head and neck oncology. In total, 89 patients (13 dentate, 30 partially dentate and 46 edentulous) treated for cancer of the oral cavity or oropharynx were asked whether they experienced a limited mouth opening. The mandibular function impairment questionnaire (MFIQ) was filled out and mouth opening was measured. The proportion correctly predicted (proportion true positives+proportion true negatives) was calculated. For cut-off points from 25 to 45 mm, differences in MFIQ scores of the restricted and non-restricted groups were analyzed. A receiver operating curve was constructed.
