钬激光腔内治疗泌尿系结石(附126例报告)   总被引:4,自引:0,他引:4  
目的:探讨泌尿系结石腔内钬激光治疗效果及安全性。方法:总结钬激光结合腔内泌尿外科技术治疗126例泌尿系结石的临床资料。结果:5例肾结石和11例膀胱结石,均一次碎石成功,109例输尿管结石,单次碎石率为96.3%(105/109),复杂结石1例数次行输尿管腔镜下碎石。平均结石排净时间2.8周,平均手术时间30min,平均术后住院2d。术中除1例输尿管穿孔外,无其他并发症。结论:钬激光结合腔内泌尿外科技术治疗泌尿系结石,是一种较新的、安全的、有效的方法。  相似文献

Background

Laser lithotripsy is an established endourological modality. Ho:YAG laser have broadened the indications for ureteroscopic stone managements to include larger stone sizes throughout the whole upper urinary tract. Aim of current work is to assess efficacy and safety of Ho:YAG laser lithotripsy during retrograde ureteroscopic management of ureteral calculi in different locations.

Methods

88 patients were treated with ureteroscopic Ho:YAG laser lithotripsy in our institute. Study endpoint was the number of treatments until the patient was stone-free. Patients were classified according to the location of their stones as Group I (distal ureteric stones, 51 patients) and group II (proximal ureteral stones, 37). Group I patients have larger stones as Group II (10.70 mm vs. 8.24 mm, respectively, P = 0.020).

Results

Overall stone free rate for both groups was 95.8%. The mean number of procedures for proximal calculi was 1.1 ± 0.1 (1–3) and for distal calculi was 1.0 ± 0.0. The initial treatment was more successful in patients with distal ureteral calculi (100% vs. 82.40%, respectively, P = 0.008). No significant difference in the stone free rate was noticed after the second laser procedure for stones smaller versus larger than 10 mm (100% versus 94.1%, P = 0.13). Overall complication rate was 7.9% (Clavien II und IIIb). Overall and grade-adjusted complication rates were not dependant on the stone location. No laser induced complications were noticed.

Conclusions

The use of the Ho:YAG laser appears to be an adequate tool to disintegrate ureteral calculi independent of primary location. Combination of the semirigid and flexible ureteroscopes as well as the appropriate endourologic tools could likely improve the stone clearance rates for proximal calculi regardless of stone-size.

目的:探讨钬激光治疗泌尿系结石的疗效。方法:总结利用Versa Pulse Select钬激光机经输尿管镜或膀胱镜治疗155例泌尿系结石患者的临床资料。结果:8例肾结石和7例膀胱结石均1次碎石成功;140例输尿管结石患者单次手术结石粉碎率为95.7%(134/140),平均结石排净时间2.5周;平均手术时间25min、平均术后住院天数2.5d。术中无输尿管穿孔、撕裂等并发症。结论:钬激光治疗泌尿结石是一种有效的腔道泌尿外科碎石方法。  相似文献

BACKGROUND AND OBJECTIVE: The holmium:YAG (Ho:YAG) laser can be used not only for soft tissue but also for hard tissue such as urinary calculi. The objective of this study was to assess the usefulness of the Ho:YAG laser for endoscopic lithotripsy in patients with urinary tract stone. STUDY DESIGN/MATERIASL AND METHODS: Of 102 procedures performed among 96 patients, 88 were transurethral ureterolithotripsy (TUL), seven were percutaneous nephrolithotripsy, and seven were transurethral cystolithotripsy. Six patients had bilateral stones. The fragments were reduced as much as possible with the Ho:YAG laser. RESULTS: The efficacy rate of the 102 lithotripsy procedures was 93%. With respect to the effect of TUL, the efficacy rates of 40 procedures for the proximal ureter, 18 procedures for the midureter, and 30 procedures for the distal ureter were 85%, 94%, and 100%, respectively. CONCLUSION: The Ho:YAG laser produced a sufficiently strong lithotripsy force on all stones. The results of this study indicate that lithotripsy of urinary tract stones with the Ho:YAG laser can achieve a clinical outcome equivalent to or exceeding that of pulsed dye laser lithotripsy. The Ho:YAG laser is a multipurpose laser and thus is a cost effective and very useful means for endoscopic lithotripsy of urinary tract stones.  相似文献

OBJECTIVE: To determine the efficacy and safety of holmium:YAG laser lithotripsy in children with ureteric calculi. PATIENTS AND METHODS: Between 1999 and 2003, 28 ureteroscopic laser procedures were carried out on 26 children (14 boys and 12 girls, mean age 6.5 years) with ureteric calculi. The mean (range) stone size was 1.21 (0.4-2.2) cm. At ureteroscopy the calculi were in the upper ureter in six (23%), mid-ureter in five (19%) and lower ureter in 15 (58%) patients. A rigid 8 F ureteroscope was used and the Ho:YAG laser energy delivered at 0.6-1.4 J and 6-10 Hz. All patients were evaluated after 3 months with intravenous urography to confirm stone clearance and to exclude ureteric stricture formation. RESULTS: The overall stone-free rate was 92% (24 children) after 28 ureteroscopic procedures. Stones were completely cleared in 83%, 80% and 100% of the procedures in the upper, mid- and lower ureters, respectively. In two children the procedure failed; they were salvaged by ureterolithotomy in one and extracorporeal shockwave lithotripsy in the other. During the procedures, a ureteric perforation was caused by several factors, including a technically difficult procedure and higher laser energy. At the mean (range) follow-up of 18 (3-39) months, low grade vesico-ureteric reflux was detected in two children, and there were no ureteric strictures in any. CONCLUSION: Holmium:YAG laser lithotripsy is an efficient and safe treatment for ureteric calculi in children.  相似文献

目的 探讨输尿管结石ESWL失败后采用腔内钬激光碎石术的临床疗效。方法 自2001年10月至2002年8月,对28例输尿管结石(ESWL失败)行输尿管镜下钬激光碎石术。结果 26例经输尿管镜下钬激光碎石术治愈,治愈率92.8%(26/28);1例结石上移,辅以ESWL治愈;1例因输尿管纤维性扭曲改开放手术。结论 输尿管镜下钬激光碎石术安全、有效、方便,可以作为输尿管结石的首选治疗。  相似文献

INTRODUCTION: Holmium laser lithotripsy represents an additional option in the management of urinary tract calculi. We report the results of a cohort of patients with ureteric and bladder calculi treated with this modality. METHODS: Twenty-three patients underwent holmium laser lithotripsy to treat ureteric or bladder calculi. Power settings of 0.5 J to deliver 2.5-4.0 watts were used for the ureteric calculi, and up to 30 watts for the bladder calculi. RESULTS: The mean ureteric stone size was 7.3 mm (range 4-10 mm). One stone was upper ureteric, seven were mid-ureter and nine were lower-ureter. Eighteen of the 19 patients with ureteric calculi were free of stones 28 days postoperatively. The remaining patient was clear by 12 weeks. All patients with bladder calculi were completely cleared of stones. There were no intraoperative complications. DISCUSSION: Ureteric stone position can limit the use of extracorporeal shock wave lithotripsy due to imaging difficulties. Ureteric lithotripsy overcomes this problem. The holmium laser has proven to be safe and effective in clearing urinary stone burdens of a variety of sizes, sites and compositions in this cohort of patients.  相似文献

目的探讨经尿道气压弹道与钬激光碎石术治疗小儿下尿路结石的安全性和有效性。 方法回顾性分析2016年9月至2017年8月在喀什地区第一人民医院和伽师县人民医院就诊的14例气压弹道碎石术与30例钬激光碎石术治疗的下尿路结石患儿的临床资料。 结果气压弹道组结石清除率达93%,手术时间7~45 min(22±13),术后1例因排尿困难需要尿道外口取石2次、1例因尿潴留需留置尿管。而钬激光组,一次性清石率为97%,手术时间10~50 min(26±14),术后1例排尿困难复查有残留结石,行二次经尿道钬激光碎石术后排尿通畅。两组均无其余严重并发症。两组手术时间、术后留置尿管时间、术后住院天数及并发症的比较差异无统计学意义。 结论气压弹道和钬激光碎石术治疗小儿下尿路结石均疗效好、结石清除率高、无严重并发症。  相似文献

目的探讨输尿管软镜下钬激光碎石术治疗肾结石的疗效。方法采用输尿管软镜下钬激光碎石术治疗肾结石106例,结石大小0.8~1.6cm,平均1.2cm。经输尿管硬镜扩张后留置输尿管扩张鞘,寻及结石以200μm光纤、0.8~1.2J/5~10Hz功率钬激光碎石。结果106例共有结石122枚,结石寻及率96.7%(118/122),单次碎石成功率91.8%(112/122)。手术时间30~60min,平均45min。术后住院1~4d,平均3d。无脓肾、输尿管穿孔等并发症。91例随访3~12个月,平均6个月,无结石复发。结论输尿管软镜下钬激光碎石术治疗肾结石安全、有效,尤其适用于结石体积较小、肾盏无明显积水,以及因解剖特点预期体外震波碎石效果不佳的患者,疗效确切,并发症少。  相似文献

PURPOSE: To study the efficiency and safety of holmium:YAG laser lithotripsy for ureteral stones. PATIENTS AND METHODS: A series of 188 patients with 208 ureteral stones were treated with semirigid ureteroscopy and holmium:YAG laser lithotripsy from January 2003 to December 2005. Of the stones, 116 were lower ureteral, 37 middle ureteral, and 55 upper ureteral. RESULTS: The success rate was 92.7% at the time of ureteroscopy and 96.7% at 3 months. The failures were secondary to retropulsion of the stones (3.3%). There were no perforations and one stricture. Stenting was done in 90% of patients. CONCLUSIONS: The Holmium:YAG laser is an ideal intracorporeal lithotripter for ureteral calculi, with a high success rate and low morbidity.  相似文献

目的 总结输尿管软镜结合钬激光治疗输尿管上段结石的疗效及并发症,探讨手术技巧.方法 38 例输尿管上段结石患者接受输尿管软镜钬激光碎石治疗,先使用8/9.8 F 输尿管硬镜探查患侧输尿管,置入导丝后留置输尿管软镜鞘,引入输尿管软镜,钬激光碎石,并使用套石篮套取出较大的结石碎屑.结果 35 例患者一期顺利碎石,3 例因输尿管狭窄难以置入输尿管软镜鞘,留置输尿管内支架2 周后再次手术成功.手术时间31~56 min,平均42 min.术后7 例患者发热,无其他严重并发症.术后1 个月拔除内支架,2 周后复查KUB,结石清除率92.1%.结论 输尿管软镜结合钬激光是处理输尿管上段结石的有效手段,即便输尿管结石进入肾内,也可一期处理,避免了额外的体外碎石操作.  相似文献

ObjectiveTo evaluate the outcomes of ureteroscopic lithotripsy with pneumatic lithotripter and Holium:Yttrium-Aluminum-Garnet (Ho:YAG) laser in the management of upper third ureteral stones.Materials and methodsPatients who underwent ureteroscopic lithotripsy with pneumatic lithotripter or Ho:YAG laser for upper third ureteral stones were retrospectively reviewed. Patients with urinary tract infection, radiolucent stones, loss of follow-up, concurrent middle or lower third ureteral stones or acute renal failure were excluded. Patient age, stone size and burden (based on KUB or computerized tomography), stone upward migration, double J stent insertion rate, stone free rate and secondary intervention rate for residual stones were compared in both groups.ResultsThere were 158 patients with 178 upper third ureteral stones (135 in pneumatic lithotripsy group and 43 in Ho:YAG laser lithotripsy group) meeting the study criteria. Patients' age, gender, stone laterality, stone size and burden were similar in both groups. The Ho:YAG laser lithotripsy group had better stone free rate, less double J stent insertion rate and less secondary intervention rate as compared with pneumatic lithotripsy (53.4% vs. 40.1%; 72.1% vs. 91.9%; 25% vs. 48.5% respectively, all p < 0.05). In patients with stones larger than 10 mm, Ho:YAG laser lithotripsy had significantly lower upward migration rate, lower double J stent insertion rate, higher stone free rate and less secondary intervention rate.ConclusionsHo:YAG laser lithotripsy is superior to pneumatic lithotripsy in the management of upper third ureteral stones in terms of double J stent insertion rate, stone free rate and secondary intervention rate for stones of all sizes. For stones larger than 10 mm, laser lithotripsy results in less stone upward migration.  相似文献

目的 探讨输尿管镜联合钬激光治疗输尿管上段结石的效果和安全性.方法 回顾性分析205例输尿管上段结石患者行输尿管硬镜钬激光碎石治疗的临床资料,其中男91例,女114例,结石位于单侧188例,双侧17例,炎性息肉引起输尿管腔明显狭窄者12例.结石长径0.8~1.4 cm.结果 192例一次性碎石成功,单次碎石成功率为93.7%(192/205).9例有较大结石碎片(4~6 mm)残留于肾或输尿管内,4例在碎石过程中结石冲入肾内,其中1例较大结石(1.4 cm)移位至肾盂后改行PCNL,其余3例留置双J管改行ESWL,均治愈.19例合并炎性息肉同期行激光烧灼.手术时间15~90min,平均30 min.住院时间2~9 d,平均3.5 d.198例患者在门诊获得随访,随访时间3~24个月,平均6个月,结石均排尽.结论 输尿管镜联合钬激光治疗输尿管上段结石的一种比较理想的腔内碎石技术,其碎石成功率高,并发症发生率低,创伤小,患者术后恢复快,而且可同期处理结石合并炎性息肉和狭窄.  相似文献

目的:比较硬性输尿管镜下国产钬激光与进口钬激光治疗输尿管结石的临床效果。方法:2003年12月~2005年3月应用国产钬激光治疗输尿管结石55例,并通过文献复习与进口钬激光输尿管碎石术的疗效进行比较。结果:国产钬激光治疗输尿管结石55例59枚获得成功,效果满意。2周结石排尽率90%,4周结石排尽率97%。术后并发输尿管狭窄1例,双J管上移1例,无其他并发症发生。与进口钬激光比较,临床效果相当。结论:国产钬激光治疗输尿管结石的疗效满意,且价格便宜,值得临床推广应用。  相似文献

目的:探讨输尿管软镜下钬激光碎石术治疗肾盏结石的疗效。方法:采用输尿管软镜下钬激光碎石术治疗肾盏结石78例,结石大小0.8~1.8cm,平均1.1cm。经留置输尿管扩张鞘或沿导丝直接置入F7,9输尿管软镜抵达肾盂,寻及结石后以200μm光纤、10 W功率钬激光碎石。结果:78例共有结石81枚.其中上、中盏结石53枚,结石寻及率100%,单次碎石成功率96.2%(51/53);下盏结石28枚,结石寻及率96.4%(27/78),单次碎石成功率71.4%(20/28)。无严重并发症发生。结论:输尿管软镜下钬激光碎石术治疗肾盏结石安全、有效.尤其适用于结石体积较小、肾盏无明显积水,以及因解剖特点预期体外冲击波碎石效果不佳的患者。通过软镜能寻及绝大多数肾盏结石,中、上盏结石单次碎石成功率高于下盏结石,与置入钬激光光纤后镜体末端有效弯曲程度减小有关。  相似文献

Use of the holmium:YAG laser for ureterolithotripsy in children   总被引:4,自引:0,他引:4  
OBJECTIVE: To review our experience with rigid ureteroscopy and holmium:YAG laser for treating ureteric calculi in children. PATIENTS AND METHODS: The study included 35 children who were treated with rigid ureteroscopy for ureteric calculi between November 1997 and June 2003 (15 boys and 20 girls; mean age 6.2 years, range 1-14). The mean (range) stone size was 8 (4-15) mm and the duration of anaesthesia 46.6 (15-90) min. The stone was in the distal third of the ureter in 33 children and in the proximal third in two. We used a 7.5/8/10 F rigid ureteroscopes with routine dilatation of the ureteric orifice. For lower ureteric stones, lithotripsy was carried out with holmium:YAG laser in 29 cases, a pneumatic impactor in two and forceps extraction in two. Both stones in the proximal ureter were pushed back into the collecting system. All the ureters were stented using JJ stents in 31 and ureteric catheters in four cases. The mean postoperative follow-up was 12 (2-30) months. RESULTS: Excluding the two stones pushed back, the stone-free rate after a one-stage procedure was 82% (27/33). With repeated procedures in the six (ESWL in two) remaining cases the success rate was 97% (32/33). The ureter was perforated in two patients within the first five in the series. There was no pyelonephritis or gross haematuria after surgery. CONCLUSION: Ureteroscopy and lithotripsy using the holmium:YAG laser is effective and safe for treating ureteric stones in children, in experienced hands. The results would be even better using smaller and flexible ureteroscopes.  相似文献

PURPOSE: To compare the efficacy, safety, and features of pneumatic lithotripsy (PL) with those of laser lithotripsy (LL) and present our clinical experience in the endoscopic management of ureteral calculi. PATIENTS AND METHODS: From August 1994 to February 2000, 285 consecutive patients underwent endoscopic lithotripsy with either the Swiss Lithoclast pneumatic lithotripter (145 patients) or the Ho:YAG laser lithotripter (140 patients) for the treatment of ureteral calculi. RESULTS: In one single session, the overall successful stone fragmentation rate of LL was higher than that of PL (95.7% v 69.7%; P < 0.01). The average time to stone-free status was shorter for LL than for PL (18 days v 31 days; P < 0.01). No major complications were observed in LL, while five ureteral perforations were encountered in PL. CONCLUSIONS: Laser lithotripsy has advantages over PL in high efficiency of stone fragmentation and a low complication rate. Laser lithotripsy is a powerful, effective, and safe treatment modality for ureteral calculi.  相似文献

【摘要】〓目的〓探讨输尿管硬镜下钬激光碎石取石术治疗输尿管上段结石的临床效果及安全性。 方法〓应用输尿管硬镜下钬激光碎石取石术治疗112例输尿管上段结石患者。男63例,女49例,年龄18~87岁(46.7±13.6岁),左侧61例,右侧46例,双侧5例。结石长径0.7~2.2 cm(1.2±0.5 cm)。26例合并输尿管息肉,31例曾行体外冲击波碎石术,所有患侧肾脏均合并不同程度肾积水。 结果〓手术成功率88.4%(99/112),手术时间15~80 min(33.6±16.8 min),术后住院天数3~14天(4.7±2.1天)。所有患者均无严重肉眼血尿、输尿管穿孔、粘膜撕裂、输尿管镜石街形成等并发症。3例患者出现畏寒、高热,其中1例发展为尿源性脓毒血症,予以积极抗感染治疗后,症状逐渐缓解。结论〓输尿管硬镜下钬激光碎石取石术治疗输尿管上段结石是安全,有效的治疗方法。  相似文献

PURPOSE: Among various intracorporeal lithotriptors, Lithoclast (EMS, Switzerland) has become the widely used tool for the treatment of urinary stones. Recently, the holmium:YAG laser has been used with a wide range of potential urological applications, including intracorporeal lithotripsy of urinary calculi. The purpose of the present study is to compare Lithoclast with holmium:YAG laser lithotripsy in ureteral calculi fragmentation. METHODS: Out of 51 patients with ureteral calculi, 26 underwent Lithoclast lithotripsy and 25 holmium:YAG lithotripsy using a 8/9.8F rigid ureteroscope. There were no changes to the ureteroscopes, video monitors, baskets or irrigation devices during the study period. RESULTS: There were no differences in patient age, sex, stone size and location of stones between these groups. The immediate stone-free rates were 96.0% in the holmium:YAG group and 73.1% in the Lithoclast group (P < 0.05). The 3-month stone-free rates were 96.0% and 84.6%, respectively (P = 0.350). The mean operation time and mean period of postoperative hospitalization in the holmium:YAG group (49.8 min and 1.0 days, respectively) were shorter than those of the Lithoclast counterpart (76.9 min and 2.5 days, respectively). Post-treatment complications, such as ureteral perforation, were encountered in only two patients who underwent Lithoclast. CONCLUSIONS: Holmium:YAG lithotripsy was associated with shorter operation time and postoperative hospitalization period. These data also suggest that holmium:YAG lithotripsy was safe and more effective than Lithoclast lithotripsy in the aspect of immediate stone free rate. We believe that holmium:YAG laser is an excellent treatment modality for managing ureteral calculi.  相似文献
