2007—2021年上海市闵行区流动人口出生缺陷变化趋势

摘    要:目的 分析闵行区流动人口新生儿出生缺陷变化情况,为预防出生缺陷,完善相关政策提供参考依据。方法 通过“上海市出生医学证明信息系统”收集新生儿出生个案信息,计算2007—2021年闵行区流动人口新生儿出生缺陷检出率,采用单因素Poisson回归分析进行趋势分析。结果 2007—2021年闵行区新生儿出生缺陷检出率为12.57‰,其中流动人口检出率为12.31‰,总体呈上升趋势(Wald χ2=168.23,P<0.001),位列前5位的疾病依次为先天性心脏病、多指/趾和并指/趾、唇裂和腭裂、小耳和足先天性变形,其中先天性心脏病检出率呈上升趋势(Wald χ2=145.25,P<0.001),多指/趾和并指/趾、唇裂和腭裂和小耳检出率呈下降趋势(P<0.05)。结论 流动人口中一些严重出生缺陷检出率仍较高,提示流动人口仍是防治重点人群,仍应加强产前检查,避免严重出生缺陷的发生。

Abstract:Objective To analyze the changes of birth defects among migrating population in Minhang District in order to provide relevant basis for birth defects prevention and relevant policies improvement. Methods The data of newborn birth cases were collected through the “Shanghai Birth Medical Certificate Information System”. The detection rates of newborn birth defects among migrating population in Minhang District from 2007 to 2021 were calculated. Single factor Poisson regression analysis was used for trend analysis. Results From 2007 to 2021, the detection rate of newborn birth defects in Minhang District was 12.57 ‰, among which migrating population was 12.31‰ and showed an upward trend(Wald χ2=168.23, P<0.001). The top five diseases were congenital heart disease, polydactyly and syndactyly, cleft lip and palate, congenital absence of(ear)auricle and microtia, congenital deformities of feet. Among the top five diseases, the detection rate of congenital heart disease showed an upward trend(Wald χ2=145.25, P<0.001), and the detection rates of polydactyly and syndactyly, cleft lip and palate, congenital absence of(ear)auricle and microtia showed a downward trend(P<0.05). Conclusion The detection rates of some serious birth defects among the migrating population are still high. It is suggested that the migrating population should still be treated as focus population for birth defects prevention. Thus, the prenatal examination should be strengthened, in order to avoid the occurrence of serious birth defects.
