| 摘 要： | 目的 探讨厄贝沙坦对维持性血液透析（MHD）患者氧化应激和微炎症状态的影响.方法 选择MHD＞半年以上的患者46例,随机分为常规透析组和透析加厄贝沙坦组,同时以20例健康成人作为正常对照组.测定各组治疗前及治疗3月后血浆晚期蛋白质氧化产物（AOPP）、丙二醛（MDA）、超氧化物歧化酶（SOD）和血清C-反应蛋白（CRP）、白细胞介素-6（IL-6）和肿瘤坏死因子-a（TNF-α）的水平,并监测血压及血钾变化.结果 与正常对照组相比较,常规透析组和透析加厄贝沙坦组血浆MDA、AOPP、CRP、IL-6、TNF-α水平均明显升高（P＜0.05）,SOD则明显降低（P＜0.05）.透析加厄贝沙坦组在治疗3月后所测得MDA、AOPP、CRP、IL-6明显低于服药前以及3月后常规透析组（P＜0.05）,而SOD则显著增高（P＜0.05）.常规透析组治疗前后相比,血浆AOPP、MDA、SOD水平有明显差异性（P＜0.05）;常规透析组治疗前后比较,血浆CRP、IL-6、TNF-α水平增加但差异无显著性（P＞0.05）.在整个治疗期间透析加厄贝沙坦组患者血钾正常.结论 血液透析患者存在氧化应激和微炎症状态,厄贝沙坦能有效地改善MHD患者的氧化应激和微炎症状态.

| Abstract: | Objective To investigate the effect of irbesartan on oxidative stress and microinflammatory state in maintenance hemodialysis（MHD）patients.Methods 46 MHD for over half a year were randomly divided into routine HD group（n=23）and irbesartan treatment group（n=23）.20 cases of healthy adults were taken as normal control group at the same time.Plasma high-sensitive C-reactive protein（hs-CRP）,interleukin-6（IL-6）,tumor necrosis factor（TNF-α）,lipid peroxide malondialdehyde（MDA）,advanced oxidative protein products（AOPP）and superoxide dismutase（SOD）were monitored before and after irbesartan treatment for 3months.Meanwhile,blood pressure and potassium levels were monitored among the three groups.Results The levels of AOPP,MDA,CRP,IL-6and TNF-αin MHD patients were higher than those of normal control（P0.05）,but SOD lower（P0.05）.As compared with pre-treatment and routine HD group after 3months treatment,the levels of IL-6,CRP,TNF-α,MDA decreased,SOD increased in irbesartan treatment group.There was no difference in the concentration of CRP,IL-6,TNF-αbetween pre-treatment and after 3months treatment in routine HD group（P0.05）,but there was significant difference in the concentration of AOPP,MDA,SOD between pre-treatment and after 3months treatment in routine HD group（P0.05）.Potassium has been normal in irbesartan treatment group.Conclusion Microinflammation and oxidative stress,existing in MHD patients,can be ameliorated obviously by irbesartan. |
