脑脊液细胞学检查联合癌胚抗原免疫细胞化学染色在脑膜癌病早期诊断中的应用

目的:探讨脑脊液细胞学检查联合癌胚抗原(CEA)免疫组化染色在脑膜癌病(Meningeal Carcinomatosis,MC)早期诊断中的应用价值。方法:脑脊液细胞学检查采用Shandon Cytospin4离心沉淀仪玻片离心法,CEA免疫细胞化学采用SP法。结果:33例经脑脊液细胞学检查均发现异常细胞,其中首次脑脊液细胞学检查阳性率为87.88%(29/33),22例结合CEA免疫组化染色,阳性率为81.82%(18/22);结论:脑脊液细胞学检查结合免疫细胞化学染色是诊断脑膜癌的重要手段,且两者互为补充。

Early Diagnosis of patients with Meningeal Carcinomatosis combining Cerebrospinal Fluid Cytology with carcino-embryonic antigen Immunocytochemical stain examination

Objective：To explore the early diagnostic value of cytologic examination of CSF combined with carcino-embryonic antigen immunocytochemistry examination in the patients with meningeal carcinomatosis.Methods：Thermo Electron Corporation Shandon cytospin 4 centrifuge was used in the cytologic examination of CSF,whose function is to produce a monolayer of cells onto a glass slide from CSF and SP staining method was used in the CEA immunocytochemistry.Results：There was a high positive rate in cytologic examination of CSF,malignant cells were found in all of 33 cases.The positive rate of routine cerebrospinal fluid cytologic examination for the first time were 87.88%（29/33）,The positive rate of 22 cases which combined with CEA immunocytochemistry examination was 81.82%（18/22）.Conclusion：Combining cytological with immunocytochemistry examination of CSF is a reliable earlier important diagnosing method for MC.The two methods are Complementary in the early diagnosis.
