重型颅脑损伤患者血清高敏C-反应蛋白、D-二聚体的变化及其意义

摘    要:目的探讨重型颅脑损伤(STBI)患者血清高敏C-反应蛋白(hs—CRP)、D-二聚体(D—D)的动态变化及其意义。方法选择2011年12月至2012年12月收治的STBI患者80例,根据入院时GCS评分分为GCS6、7、8分三组,检测患者伤后1、3、7、14天hs-CRP、D—D浓度;选取同期健康志愿者20例作为对照组。结果STBI组伤后1、3、7、14d血清hs—CRP、D—D浓度均高于对照组(P〈0.01);血清hs-CRP、D—D浓度与GCS评分、GOS评分呈显著负相关(P〈0.01);血清hs—CRP、D—D浓度随颅脑CT中线移位程度的增加而显著升高(P〈0.05)。结论STBI患者血清hs—CRP、D—D浓度的动态变化能反映患者的伤情;早期检测血清hs—CRP、D—D可作为评估STBI预后的重要参数。

Abstract:Objective To investigate the dynamic change in the serum contents of high sensitive C-reactive protein (hs-CRP) and D-dimer and its clinical significance in patients with severe traumatic brain injury (sTBI) in order to explore the relationship among the serum content of hs-CRP and D-dimer, GCS, GOS and brain midline shift. Methods The serum contents of hs-CRP and D-dimer were determined 1, 3, 7 and 14 days after the injury in 80 patients with sTBI treated in our hospital from December, 2011 to December 2012 and 20 healthy volunteers serving as control group. The relationships of the GCS, GOS and the midline shift distance on CT images with the serum contents of hs-CRP and D-dimer were analyzed. Results The serum contents of hs-CRP and D-dimer were significantly higher in sTBI group than those in the control group 1, 3, 7 and 14 days after the injury (P〈0.01), serum contents of hs-CRP and D-dimer were significantly negatively correlated with GCS and GOS in the patients with sTBI (P〈O.01). The serum content of hs-CRP was significantly positively related to that of D-dimer (P〈0.01). The serum contents of hs-CRP and D-dimer increased along with the increase in brain midline shift distance. Conclusions The dynamic changes in the serum contents of hs-CRP and D-dimer may reflect the severity of the traumatic brain injury and the patients" injury condition. It is suggested the serum contents of hs-CRP and D-dimer may be used as two important parameters to assess prognoses in the patients sTBI.
