儿童颞叶癫痫的临床特点及影响其认知功能的因素探讨

目的探讨儿童颞叶癫痫的临床特点及影响其认识功能的因素。方法对71例颞叶癫痫患儿的脑电图、影像学、认识功能等临床资料进行回顾分析。结果脑电图提示单侧颞叶41例,双侧颞叶同步或交替放电20例,额颞叶放电,以颞叶放电为著者8例,颞叶放电传到枕叶2例。头颅影像学检查中29例(40.8%)显示有颞叶结构异常。智力缺陷的发生率为56.3%(40/71)。智力与发作频率、每次发作持续时间、用药种类密切相关。结论颞叶癫痫临床表现多样化,但结合临床、脑电图、影像学结果可以作出诊断。发作频率高、每次发作持续时间长、联合用药种类多的颞叶癫痫患儿智力容易受损。

The clinical characteristics of children with temporal lobe epilepsy and the factors of affecting the cognitive function

Objective To analyze the clinical characteristics of children with temporal lobe epilepsy and to explore the factors that affect intelligence condition.Methods The clinical data of 71 children with temporal lobe epilepsy including EEG,imaging and intelligence condition were retrospective analyzed.Results EEG point out 41 patients with unilateral temporal lobe discharge,20 cases with bilateral temporal lobe synchronous or alternate discharge,8 cases of temporal lobe discharge were significantly on frontotemporal discharge,discharge spread from temporal lobe to occipital lobe in 2 cases.29 cases(40.8%) showed abnormal temporal lobe structure on head imaging.The rate of intelligence defect was 56.3%(40/71).Intelligence condition was closely related to the seizure frequency,duration of each episode and drug types.Conclusion Temporal lobe epilepsy has the diverse clinical manifestations,it can be diagnosed through a combination of clinical,EEG and imaging results.Temporal lobe epilepsy has the characteristics such as high seizure frequency,longer duration of each attack,the more combination of drug type the poorer of intelligence condition on children.
