COX-2和VEGF在胃腺癌组织中的表达及相关性研究

摘    要:目的研究环氧化酶2(COX-2)和血管内皮生长因子(VEGF)在胃腺癌组织中的表达及与临床病理特征之间关系。方法采用免疫组织化学方法检测478例胃腺癌组织和49例正常癌旁组织中COX-2和VEGF的表达水平,并分析两者之间的相关性及其与临床病理特征之间的关系。结果胃腺癌组织中COX-2阳性表达率为322/478(67.4%),VEGF的阳性表达率为302/478(63.2%),COX-2和VEGF的表达与患者性别、年龄及胃腺癌的分型、分化程度、肿块直径大小无相关性(P>0.05),与局部浸润深度、TNM分期、淋巴结转移呈正相关(P<0.05),二者在在胃腺癌组织中的表达呈正相关(r=0.329,P<0.01),胃癌组织中COX-2和VEGF阳性表达率明显高于癌旁组织(P<0.01)。结论 COX-2和VEGF在胃腺癌组织中不同程度表达,与局部浸润深度、TNM分期、淋巴结转移呈正相关,提示二者的过表达可能是评估胃癌的生物学行为及患者预后的重要指标。COX-2和VEGF在胃腺癌中高表达且密切相关,提示二者可能存在相互作用机制并共同促进肿瘤的发生发展。

Expression and clinical significance of COX-2 and VEGF in gastric adenocarcinoma

Abstract:Objective To investigate the expression and correlation of cyclooxygenase-2 and VEGF in gastric adenocarcinoma tissue. Methods Cyclooxygenase-2 protein and vascular endothelial growth factor expression in 478 cases of gastric adenocarcinoma tissue and 49 cases of adjacent normal tissue were detected by using immunochemical staining and the relationship between COX-2 and VEGF was analyzed together with the clinicopathologic characteristics. Results The positive rates of COX-2 or VEGF in gastric cancer tissue were 67.4% and 63.2%. The expression of COX-2 and VEGF was associated with the grade of local infiltration ,TNM stage ,lymph node metastasis ( P 〈 0.05 ), while it was not related with age , gender, typing, histological grade, and the size of tumor( P 〉 0.05 ). The expression of COX-2 was closely correlated with the expression of VEGF( r = 0.329, P 〈 0.01 ). The positive rate of COX-2 and VEGF in gastric adenocarcinoma tissue was more than that in adjacent normal tissue( P 〈 0.01 ). Conclusion The overexpression of COX-2 and VEGF associated with the grade of local infiltration,TNM stage,lymph node metastasis means that detection of COX-2 andVEGF protein may be used to assess the malignant biological behavior and prognosis of gastric cancer. The significant relationship between overexpression of COX-2 and VEGF indicates that COX-2 and VEGF can regulate each other and induce the tumorgenesis and development together.
