摘    要:目的 探讨耳背部肿瘤切除后的缺损修复方法及耳后颞蒂皮瓣在修复中的效果.方法 8例耳廓背部肿瘤术后缺损分别用耳后颞蒂皮瓣、游离皮瓣或直接拉拢缝合修复,观察缺损修复效果.结果 本组均愈合,无皮瓣坏死,耳廓外形满意.鳞状细胞癌侵及耳后肌及软骨膜和软骨各1例,予以手术切除受累软骨.其余病例均无软骨膜侵犯,随访1.5~4.0年无复发.结论 对于耳背部缺损应根据大小及部位采取灵活的修复方法,耳后颞蒂皮瓣修复安全可靠.

Abstract:Objective To explore postoperative auricular reconstruction and the efficacy of postauricular flaps in repairing retroauricular defects after tumor excision. Methods Eight patients with operative defect of cancerous or benign skin lesions in the posterior auricular surface underwent repairing surgery by adopting postauricular flaps, free flap graft or primary suture. We observed repairing effect of defected places. Results There was no flap losses, and shape of ears was good. 2 patients had squamous cell carcinoma; one had invaded museulus aurieularis posterior and perichondrium and the other had invaded cartilage. These 2 patients were cured by elective neck dissection. There was no perichondrium encroachment in others patients during the follow-up for 1.5-4.0 years. Conclusion We should have a variety of restorative procedures for retroauricular defects according to the size and place. Postauricular flaps surgery is safe and widely used.
