我院Ⅰ类切口手术抗菌药物应用情况分析

目的分析医院Ⅰ类切口手术抗菌药物预防性使用情况,加强Ⅰ类切口手术抗菌药物预防使用的控制和管理。方法通过医院信息系统(HIS)统计2018年1-11月江苏省盐城市第一人民医院Ⅰ类切口手术数量和抗菌药物使用率,筛选出冠脉造影等血管介入诊断手术和重点监控Ⅰ类切口手术,统计其抗菌药物使用率,再通过电子信息化病历系统分析抗菌药物使用情况。结果2018年1-11月医院Ⅰ类切口手术中冠脉造影等血管介入诊断手术抗菌药物预防使用率为0,符合不预防使用抗菌药物的规定;原则上不预防使用抗菌药物的Ⅰ类切口手术抗菌药物预防使用率为3.4%,符合小于5%的规定;重点监控Ⅰ类切口手术抗菌药物预防使用率为9.6%,符合≤30%的规定;重点监控Ⅰ类切口手术前0.5~2.0 h内给药率为44.1%,不符合100%的规定;重点监控Ⅰ类切口手术抗菌药物预防使用平均时间为5.09 d,不符合≤24 h的规定;重点监控Ⅰ类切口手术抗菌药物预防使用时间≤24 h占比为9.9%,与规定的100%相差较大。结论该院Ⅰ类切口手术抗菌药物使用基本符合规范,特别是重点监控Ⅰ类切口、原则上不预防使用抗菌药物的Ⅰ类切口及介入诊断手术的使用率均能达到要求,但其他方面还存在一些问题。

Analysis of the application of antibacterial drugs in classⅠincision surgery

Objective To analyze the preventive use of antibacterial drugs in theⅠincision surgery in hospitals,and strengthen the control and management of the preventive use of antimicrobials in theⅠincision surgery.Methods According to the Hospital Information System(HIS),from January to November 2018,the number of ClassⅠincision operations and the use rate of antibacterial drugs in the Yancheng First People’s Hospital,Jiangsu Province,were screened out for coronary angiography and other vascular interventional diagnostic operations and key monitoring of ClassⅠincision operations,made statistics on the use rate of antibacterial drugs,and analyzed the use of antibacterial drugs through the electronic informationized medical record system.Results From January to November 2018,the preventive use rate of antibacterial drugs in coronary angiography and other vascular interventional diagnostic procedures during the first-class incision surgery in the hospital was 0,which was in line with the non-preventive use of antibacterial drugs.In principle,the preventive use rate of antibacterial drugs for classⅠincision surgery that didn’t prevent the use of antibacterial drugs was 3.4%,which met the requirement of less than 5%.The preventive use rate of antibacterial drugs for typeⅠincision surgery was 9.6%,which met the requirement of≤30%.The medication rate within 0.5 to 2.0 hours before the typeⅠincision operation was 44.1%,which didn’t meet the 100%requirement.Focus on monitoring the preventive use of antibacterial drugs for classⅠincision surgery with an average time of 5.09 days,which didn’t meet the requirement of≤24 hours.Focus on monitoring the preventive use of classⅠincision surgery antibacterial drugs for≤24 hours,accounting for 9.9%of the time,which was far from the prescribed 100%.Conclusion The use of antibacterial drugs forⅠincision surgery in this hospital basically meets the norms,especially focusing on monitoringⅠincisions,and in principle not preventing the use of antimicrobials.The utilization rate of interventional diagnostic surgery can meet the requirements,but there are still some problems in other aspects.
