充填悬突与龈下菌斑关系的临床研究

摘    要:目的从临床指标及微生物学角度探讨悬突引起牙周炎的机制,揭示悬突对牙周组织的危害性。方法收集49例患者的60个有充填悬突的牙齿(悬突组),以患者同颌无充填体的同名牙作为自身对照组,每个牙记录牙周探诊深度(PD)、龈沟出血指数(SBI);采集龈下菌斑以刚果红染色,测其细菌组成。选择其中10例磨除悬突后重新充填,6个月后复查PD和SBI。结果悬突组的PD、SBI明显高于对照组,其螺旋体、杆菌及球菌较对照组均有显著性差异(P<0.01);重新充填后6个月SBI较充填前有显著性差异(P<0.01)。结论悬突造成牙周组织的破坏,使龈下菌群组成发生变化;临床治疗中要预防悬突的发生。

Abstract:Objective To probe into the mechanism of periodontistis caused by overhanging in the view of clinical parameters and microecology to illuminate the damage of overhanging to periodontal tissues.Methods Sixty teeth with overhanging were diagnosed by examining for PD and SBI of site of teeth with overhanging and the same site of teeth without filling.Subgingival plague were examined.Ten teeth with overhanging were restored,and examined for PD and SBI after six months.Results The differences of PD and SBI levels between the experimental group and the control group were statistically significant (P<0.05).The differences of SBI between the experimental group and the same group after restoration were statistically significant (P<0.01).Conclusion This indicates that overhanging can change the periodontal mircroecolegical environment and destroy the periodontal tissues.Dentists should reinforce the prevation and treatment of overhanging.
