结直肠癌合并急性肠梗阻33例临床分析

目的：探讨结直肠癌合并急性肠梗阻时，外科手术方式的选择及相关治疗方法。方法回顾性分析本医院2003年3月~2010年11月期间33例第一诊断为结直肠癌同时合并急性肠梗阻并且最终以手术方式治疗的患者的临床资料。结果33例患者以不同手术方式治疗，一期吻合28例，占84.8%；二期吻合2例，占6.1%；行姑息性造口或捷径吻合术3例，占9.1%。其中，7例（21.2%）术后出现并发症，包括感染性休克2例，切口感染4例，切口裂开1例，经治疗后均好转出院，无死亡病例。结论提高对结直肠癌导致急性肠梗阻的认识，尤其是选择手术方式治疗的，要根据患者的整体情况、肿瘤及肠管的局部情况等综合分析，选择合理的手术方式，同时加强围手术期处理，可以减少并发症，降低死亡率。

Analysis of surgical treatment of 33 colorectal cancer patients with intestinal obstruction

Objective To explore diagnosis and treatment of intestinal obstruction caused by colorectal cancer. Methods The clinical data of 33 patients with intestinal obstruction caused by colorectal cancer were analyzed retrospectively from March 2003 to November 2010. Results There were seven postoperative complications in the thirty three cases （21.2%） who underwent operation,include two cases of incision infection,two case of infective shock,and one case of incisional dehiscence.All cases were discharged from hospital after a favourable turn. Conclusion More attentions should be paid to the intestinal obstruction caused by colorectal cancer.The appropriate operation methods and peri-operative management may decrease the complications and improve the curative effect.
