恶性血液病并发肺部感染的病原菌构成及耐药性变迁

摘    要:目的:明确恶性血液病并发肺部感染的病原菌构成及耐药性变迁。方法:分析近5年来115例恶性血液病并发肺部感染患者的病原学和药敏学特点。结果:培养出病原菌242株,其中革兰阴性(G-)菌158株,排在前4位分别是铜绿假单胞菌、肺炎克雷伯菌、大肠埃希菌和嗜麦芽寡养单胞菌。铜绿假单胞菌对亚胺培南、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦和阿米卡星的耐药率<30%;肺炎克雷伯菌和大肠埃希菌对亚胺培南、头孢哌酮/舒巴坦和哌拉西林/他唑巴坦的耐药率<30%;头孢哌酮/舒巴坦和复方磺胺甲唑对嗜麦芽寡养单胞菌有一定的抗菌活性。革兰阳性(G+)菌56株,耐药性严重,对万古霉素和替考拉宁敏感率均为100%。近5年来无论是G-菌还是G+菌的耐药率总体呈上升趋势。结论:恶性血液病并发肺部感染患者的主要病原菌为G-菌,呈现出多重耐药性,且耐药率不断增加。

Abstract:OBJECTIVE:To identify the constitution and drug resistances of pathogenic bacteria of malignant hematology with pulmonary infection.METHODS:The etiology and drug sensitivity of 115 cases of malignant hematology with pulmonary infection were analyzed.RESULTS:242 strains of pathogenic bacteria were cultured.There were 158 strains of gram-negative bacteria.Top 4 were Pseudomonas aeruginosa,Klebsiella pneumoniae,Escherichia coli and Stenotrophomonas maltophilia.The drug resistance rates of Pseudomonas aeruginos...
