我院2008-2010年乙型肝炎抗病毒药物应用分析

目的:了解我院2008-2010年乙型肝炎抗病毒药物使用情况,分析用药趋势。方法:对我院2008-2010年乙肝抗病毒药物的销售金额、用药频度(DDDs)等进行统计、分析。结果:核苷类似物的销售金额逐年增加,干扰素类逐年减少;三年间阿德福韦酯的销售金额、DDDs排名第一或第二位,且日用药金额(DDC)最低;恩替卡韦仅次于阿德福韦酯,DDC值是核苷类似物中最高;拉米夫定的销售金额、DDDs在三年间没有明显变化;替比夫定占抗乙肝病毒药物年销售金额构成比均低于14.3%,DDDs为4种核苷类似物中最低。结论:核苷类似物在乙肝抗病毒药物临床使用中占绝对优势;临床医师更倾向于选择阿德福韦酯用于抗乙肝病毒的治疗;对于长疗程的核苷类似物,除了要关注药物的疗效、监测乙肝病毒耐药性,还要长期随访病人、监测线粒体毒性相关的不良反应。

Analysis on the Appfication of Anfiviral Drugs for Hepatitis B in Hospital from 2008 to 2010

Objective： To investigate the application of antiviral drugs for hepatitis B in our hospital from 2008 to 2010 and to analyze the tendency of drug use. Methods： The total sales amount and DDDs of the antiviral drugs for hepatitis B in our hospital from 2008 to 2010 were analyzed statistically. Results： In those three years, the sales amount of nucleoside analogues increased but interferons decreased annually, adefovir and DDDs ranked the first or the second with the lowest DDC, entecavir ranked next with highest DDC. The sales amount of lamivudine and DDDs did not change significantly. The telbivudine＇ s constituent ratio of sales amount per year of antiviral drugs for hepatitis B was lower than 14.3% with the lowest DDDs among the four kinds of nucleoside analogues. Conclusion： Nucleoside analogues took the dominant place in clinical use among the antiviral drugs for hepatitis B, but clinical physicians preferred to use adefovir for the treatment of hepatitis B virus. For the long course of nucleoside analogues, attention should be paid to not only the drug therapeutic effect but also the drug tolerance, and the adverse reactions related to mitochondria toxicity should be monitored.
