妊娠合并心脏病产妇不同心功能状态对分娩结局的影响

目的 探讨妊娠合并心脏病产妇不同心功能状态对分娩结局的影响.方法 选取在我医院孕检并分娩的妊娠合并心脏病产妇92例作为研究对象,对比产妇和围产儿的结局.结果 Ⅰ～Ⅱ级的剖宫产率为62.32％,显著低于Ⅲ～Ⅳ级的86.96％,差异具有统计学意义(P＜0.05).Ⅰ～Ⅱ级围生儿早产、足月低体重儿、死亡的几率分别为8.70％、1.45％、1.45％,显著低于Ⅲ～Ⅳ级26.07％、13.04％、13.04％,差异具有统计学意义(P＜0.05).Ⅰ～Ⅱ级围产儿Apgar评分、围产儿体重分别为(8.64±1.03)分、(3 198.62±351.49)g,均显著高于Ⅲ～Ⅳ级,差异具有统计学意义(P＜0.05).结论 妊娠合并心脏病产妇心功能状态与分娩结局具有显著相关性,应早期评估妊娠合并心脏病产妇的心功能,早期予以干预治疗.

Birth outcomes of pregnant women with heart disease in different cardiac function grades

Objective To investigate the birth outcomes of pregnant women with heart disease in different cardiac function grades.Methods 92 pregnant women with heart disease were selected as study objects.The maternal and perinatal outcomes were compared.Results The cesarean section rate was 62.32％ in the pregnant women with Ⅰ-Ⅱ cardiac function and was 86.96％ in those Ⅲ-Ⅳ, with a statistical difference (P ＜0.05).The rates of preterm perinatal children, term low birth weight children, and death newborns were 8.70％, 1.45％, and 1.45％ in the pregnant women with Ⅰ-Ⅱ cardiac function and were 26.07％, 13.04％, and 13.04％ in those Ⅲ-Ⅳ , respectively, with statistical differences (P ＜0.05).The perinatal Apgar score and the perinatal weight were (8.64 ± 1.03) and (3 198.62 ± 351.49) g in the pregnant women with Ⅰ-Ⅱ cardiac function and were significantly higher in the pregnant women with Ⅰ-Ⅱ cardiac function than thse Ⅲ-Ⅳ ,with statsticaldifferences (P ＜0.05).Conclusions There are significant correlation between the cardiac function of pregnant women with heart disease and birth outcomes.Pregnancy maternal cardiac function should be early assessed and intervened with early treatment.
