显微血管减压术治疗老年原发性三叉神经痛的疗效分析

目的：评估显微血管减压术（MVD）治疗老年原发性三叉神经痛的近期及远期疗效。方法：总结MVD治疗124例老年原发性三叉神经痛的术后疗效及长期随访结果。结果：术后有效率为93．6％，治愈率为85．5％，并发症发生率为10．5％，术后随时间延长，有效率逐渐降低，迟发性神经系统并发症发生率升高，结论：尽管MVD是治疗老年原发性三叉神经痛的有效方法，但长期随访复发率升高，且神经系统并发症高，对于无明显血管压迫及高龄患者，建议实施三叉神经感觉根部分切断术。

Clinical outcomes after microvascular decompression for trigeminal neuralgia in elderly patients

Objective:To evaluate the immediate postoperative and long term outcomes of microvascular decompression (MVD) in the treatment of trigeminal neuralgia in elderly patients.Methods:Retrospective review of the immediately postoperative and long term outcomes in 124 elderly patients following MVD.Results:Pain contol rate was 93.6% with complete pain free rate of 85.5%. 10.5% of all patients suffered some complications. The pain control rate decreased and the neurosystem complications rate increased in 5 year follow up.Conclusion:MVD is an effective treatment for trigeminal neuralgia in elderly patients,however,the recurrent rate increased in long term follow up and the complications rate of non neurosystem was high. Partitial trigeminal sensory root section is recommended for elderly patients without blood vessel compression.
