Hero642镍钛机动根管锉在下颌磨牙C形根管预备中的应用

摘    要:目的] 比较机用 Hero642 镍钛根管锉与手动 K型根管锉 (K型锉 )预备下颌第二磨牙 C形根管的临床效果. 方法] 选取临床上需行下颌第二磨牙牙髓治疗的 C形根管 42例,随机分成两组.实验组 22例,用镍钛机动根管预备;对照组 20例,用手动 K型锉预备根管,根管预备后均用侧压法进行根管充填.记录并比较根管预备时间,通过 X片分析根管预备和根管充填的效果,同时比较两组间根管充填后的反应.结果] Hero642 机动镍钛锉对 C形根管预备的平均时间为 223 s,手动 K型锉为 400 s,两组间有显著差异 (P< 0.01);两组均没有发生器械折断,各有 2例发生根尖轻度偏移;手动 K 型锉组同时发生 2例根尖阻塞和 3例根管内台阶形成;两组间根管充填后疼痛的发生率没有显著差异.结论] 与 K型锉相比,机动 Hero642 镍钛根管锉在下颌第二磨牙 C形根管预备中具有操作简便、省时省力、成形性好、安全性好、效率高、实用性强等特点,是临床上对下颌第二磨牙 C形根管进行预备有效方法.

Abstract:To compare the clinical effect between Hero642 rotary nickel titanium file and stainless K file in preparation of C shaped root canals in the mandibular second molar. Forty two mandibular second molars with C shaped root canal scheduled root canal treatment were divided into two groups based on the random principle. In the experimental group(n=22), C shaped root canals were prepared with Hero 642 rotary Ni Ti files in crown down technique. In the control group(n=20), C shaped root canals were prepared with K Flexofiles by hand. All teeth were obturated with lateral condensation method. The prepared time of each C shaped root canal was recorded and the incidence of EIP (endodontic interappointment pain)was observed. The effect was compared between two groups. The efficacy of preparation and obturation were analyzed by radiograph. The mean prepared time in experiment group (223 s) was significantly less than that in the control group (400 s). No instrument fracture found in both groups. Two slight apical transportations were found in each group. Two apical blockages and 3 apical ledges were found in the K Flexofile group. The incidence of EIP was not significantly different between two groups. Conclusion]Compared with K Flexofile, the Hero642 rotary Ni Ti file can prepare the C shaped root canals effectively, easily,and safely.
