明胶酶在舌鳞癌发生、演进中的表达和意义

摘    要:【目的】通过检测明胶酶在口腔正常黏膜、白斑、白斑癌变和鳞癌组织中的表达,探讨其在舌鳞癌的发生和演进中的变化和作用。【方法】应用免疫组织化学的方法检测14例口腔正常黏膜、22例白斑、7例白斑癌变和59例舌鳞癌组织中明胶酶(MMP-2和MMP-9)蛋白的表达。【结果】①MMP-2在口腔正常黏膜、白斑、白斑癌变和舌鳞癌组织中阳性率分别为29%、50%、86%和83%;MMP鄄9在口腔正常黏膜、白斑、白斑癌变和舌鳞癌组织中阳性率分别为50%、100%、100%和83%。②正常黏膜组织中MMP-2的阳性率明显低于白斑癌变(P=0.016)和舌鳞癌组织(P=0.000);白斑组织中MMP-2的阳性率明显低于舌鳞癌组织(P=0.001);白斑癌变组织中MMP鄄2的阳性率明显低于伴有淋巴结转移的舌鳞癌组织(P=0.019);舌鳞癌组织中无淋巴结转移组的阳率明显低于伴有淋巴结转移组(P=0.006)。③正常黏膜组织中MMP-9的阳性率明显低于白斑(P=0.021)和白斑癌变组织(P=0.016);白斑和白斑癌变组织中MMP-9的阳性率低于无淋巴结转移的舌鳞癌(P=0.002和0.007);舌鳞癌组织中无淋巴结转移组的阳率明显低于伴有淋巴结转移组(P =0.033)。【结论】明胶酶在舌鳞癌发生和演进过程中表达明显增加,并可能在这一过程中起着重要作用。

Abstract:To investigate the expression and significance of gelatinase in the carcinogenesis, development, and metastasis of tongue squamous cell carcinoma by examining the expression of gelatinase of oral normal mucosa, leukoplakia, leukoplakia cancer and tongue squamous cell carcinoma. The expression of gelatinases MMP-2 (matrix metalloproteinase 2) and MMP-9 (matrix metalloproteinase 9)] were detected in 14 cases of oral normal mucosa, 22 cases of leukoplakia, 7 cases of leukoplakia cancer and 59 cases of tongue squamous cell carcinoma with immunohistochemistry.(1)The positive rates of MMP-2 of oral normal mucosa, leukoplakia, leukoplakia cancer, and tongue squamous cell carcinoma were 29%, 50%, 86%, and 83%, respectively. The positive rates of MMP-9 of oral normal mucosa, leukoplakia, leukoplakia cancer, and tongue squamous cell carcinoma were 50%, 100%, 100%, and 83%, respectively. (2) The positive rate of MMP-2 of oral normal mucosa was significantly lower than that of leukoplakia cancer (P =0.016) and tongue squamous cell carcinoma (P =0.000). The positive rate of MMP-2 of leukoplakia was significantly lower than that of tongue squamous cell carcinoma (P =0.001). The positive rate of MMP-2 of leukoplakia cancer was significantly lower than that of tongue squamous cell carcinoma with lymphoid metastasis (P =0.019). The positive rate of MMP-2 of tonguesquamous cell carcinoma without lymphoid metastasis was significantly lower than that with lymphoid metastasis (P =0.006). (3) The positive rate of MMP-9 of oral normal mucosa was significantly lower than that of leukoplakia (P =0.021) and leukoplakia cancer (P=0.016). The positive rates of MMP-9 of leukoplakia and leukoplakia cancer were significantly higher than that of tongue squamous cell carcinoma without lymphoid metastasis (P =0.002 and 0.007, respectively). The positive rate of MMP-9 of tongue squamous cell carcinoma without lymphoid metastasis was significantly lower than that with lymphoid metastasis (P =0.033).Conclusion] The expression of gelatinase increases significantly in the carcinogenesis, development, and metastasis of oral squamous cell carcinoma. Gelatinase may play an important role in the progression of tongue squamous cell carcinoma.
