醒脑开窍针刺法联合Vitalstim吞咽治疗仪治疗脑卒中吞咽障碍临床研究

目的:观察醒脑开窍针刺法联合Vitalstim吞咽治疗仪治疗脑卒中吞咽障碍的临床疗效。方法:将105例患者随机分为对照组52例和观察组53例。两组均在病情稳定后进行吞咽功能训练。对照组采用Vitalstim吞咽障碍治疗仪进行治疗,每次30 min,刺激同时患者进行吞咽动作,每天1次,每周6次。观察组在对照组基础上加用醒脑开窍针刺法,每天1次,每周6次。两组疗程均为3周。两组患者治疗前后进行洼田饮水试验、标准吞咽功能评估、吞咽困难分级量表和吞咽障碍特异性生存质量量表评分。结果:观察组有效率为92.45%,对照组有效率为78.85%,两组比较差异有统计学意义(χ2=3.968,P=0.046);观察组治疗后洼田饮水试验情况显著优于对照组(P0.05);治疗后观察组洼田饮水试验评分、标准吞咽功能评分、吞咽困难分级量表评分均显著低于对照组(P0.05),吞咽障碍特异性生活质量量表评分显著高于对照组(P0.01)。结论:采用醒脑开窍针刺法联合Vitalstim吞咽障碍治疗仪治疗脑卒中后吞咽障碍,能显著改善吞咽障碍,提高患者生存质量,临床疗效显著。

"Xingnao Kaiqiao" Acupuncture Therapy Combined with Vitalstim Swallowing Therapeutic Apparatus in Treatment of Dysphagia of Stroke

Objective:To observe the clinical efficacy of "Xingnao Kaiqiao" acupuncture combined with Vitalstim swallowing therapeutic apparatus in treatment of dysphagia of stroke.Methods:One hundred and five patients were randomly divided into control group (52 cases) and observation group (53 cases).Both groups experienced swallowing function training in stable condition.The control group was treated with Vitalstim swallowing disorder treatment instrument 30min each time while patients experienced swallowing stimulation and the treatment took place once a day,6 times a week.The observation group was treated with " Xingnao Kaiqiao" acupuncture,1 times a day,6 times a week,in addition to that of the control group.The course of treatment was 3 weeks.Before and after treatment,both groups of patients took the Kubata drinking water test,standard swallowing function assessment,dysphagia grading scale scoring and swallowing disorder-specific quality of life scale (SWAL-QOL) scoring.Results:The effective rate of the observation group was 92.45％ and the effective rate of the control group was 78.85％,with the difference being statistically significant (x2 =3.968,P =0.046).The Kubata drinking water test scoring of observation group after treatment was significantly better than that of the control group (P ＜ 0.05).After treatment,the standard swallowing function scoring and the dysphagia grading scale scoring were significantly lower than that of the control group(P ＜ 0.01).Conclusion:" Xingnao Kaiqiao" acupuncture combined with Vitalstim dysphagia therapeutic apparatus treating for dysphagia after stroke can significantly improve dysphagia and patients' life quality,with significant clinical efficacy.
