交叉克氏针在儿童有移位肱骨髁上骨折的运用

目的探讨采用交叉克氏针治疗儿童有移位肱骨髁上骨折的注意事项及疗效。方法对需手术治疗的42例有移位肱骨髁上骨折患儿,选择不同肘关节手术入路,切开复位后,分别于肱骨内外髁向肱骨近端行交叉克氏针内固定,3~4周拔除克氏针,行肘关节屈伸功能锻炼。结果根据Dodgt′s疗效评价标准,优30例,良11例,差1例,优良率97.6%。结论切开复位交叉克氏针固定治疗儿童有移位肱骨髁上骨折,操作易掌握,复位理想,固定相对牢靠,可较早行康复锻炼,肘关节功能恢复良好。

Application of crossed Kirschner wire in treatment of supracondylar fractures of humerus with displacement in children

Objective To explore the cautions and effect on internal fixation using crossed Kirschner wire for treating supracondylar fractures of humerus with displacement in children.Methods Forty-two patients with supracondylar fractures of humerus with moving admitted between July 2001 and December 2009 were enrolled in this report.All patients were treated with crossed Kirschner wire to fix fractures after open reduction.Selecting different operational approaches and after open reduction,the crossed Kirschner wires were nailed in from internal and external condyle of the humerus to proximal end of humeral shaft to fix fracture and keeping the wire-cauda outside the skin.The Kirschner wires were got rid 3-4 weeks later,and the exercise of flexion and extention function of elbow joint was performed.Results According to Dodgt′s assessment standard of therapeutic effect,30 cases were excellent,11 cases were good and 1 case was poor.The excellent and good rate was 97.6%.Conclusions The method of fixation with crossed Kirschner wire for treating supracondylar fractures of humerus with displacement in children has the merits of easy operation,good reduction,substantial fixtion,early functional exercise,and good recovery of elbow joint function.
