人工真皮支架修复猪Ⅲ度烧伤动物模型的构建

摘    要:目的建立猪Ⅲ度烧伤所致全层皮肤缺损并植入人工真皮支架的动物模型,观察人工真皮支架移植后1、2、3周,支架血管化情况以及人工真皮支架上移植刃厚皮片的可能性。方法造成直径约3 cm的Ⅲ度烧伤创面,48 h后清创后植入胶原-磺化羧甲基壳聚糖/硅橡胶双层人工真皮支架,术后悬吊饲养、隔天换药护理。支架植入后1、2、3周观察创面并植刃厚皮片,植皮2周后观察植皮成活及创面修复。结果①造成约3 cm直径烧伤创面的铜锭最佳直径为2.5 cm;②为防止创面收缩必须应用移植环,手术后必须悬吊饲养;③支架植入后植皮时间最好选择在2周以后;④良好的术后处理是实验成功的重要因素。结论本实验模型稳定、可重复性强,为人工皮肤的研究和生物学意义的验证提供了良好的大型动物模型。

Abstract:Objective To establish a model of full thickness skin burn and wound repair with artificial dermal scaffold in pigs.Angiogenesis of the artificial dermal scaffold were observed in 1 week,2 weeks,and 3 weeks after the transplantations of dermal scaffold and the possibility of blade thickness skin graft on dermal scaffold were investigated. Methods A diameter of 3cm of full thickness burn wound was made in the pig and collagen-sulfonated carboxymethyl chitosan/silicone rubber porous double-layer scaffold was transplanted on the debridement wound after 48h.Pigs were cared by suspension style and the dressings were changed every 2 days.The wounds were observed and blade thickness skin grafts were grafted on it 1 week,2 weeks and 3 weeks after transplantation.The survival rates of grafts and wound repairs were observed 2 weeks after the skin grafts. Results ① A proper diameter of copper column was 2.5 cm for making a diameter of 3 cm of burn wound;② A ring circling wound bed fringe must be applied to prohibit wound contraction,and suspension style must be used to limit pigs' activity;③ Best time for blade thickness skin graft was 2 weeks after dermal scaffolds transplantation;④ Careful post-operative management was essential factor for experiment success. Conclusion This model is stable and reproducible,and it may provide a perfect large animal model for the research and biological validation study of artificial skin.
