丙泊酚对ARDS机械通气患者的持续镇静的临床研究

摘    要:目的:探讨丙泊酚对急性呼吸窘迫综合症( ARDS)机械通气患者的镇静效果。方法:对本院2012-10~2014-10期间收治的60例急性呼吸窘迫综合症患者作为研究对象,随机分为研究组和对照组。研究组在其进行机械通气前进行注射丙泊酚镇静诱导治疗,对照组则为咪达唑仑治疗。观察记录两组患者的镇静时间效果,用药时和停药后的呼吸、循环参数变化,肺内动静脉分流率和氧合指数,患者是否出现不良反应。结果:患者注射丙泊酚后,5min内可达到预期效果,起效较快。心压和心率稳定。呼吸频率和血氧饱和度等方面有明显改善,其差异有显著性,具统计学意义(P<0.05)。研究组和对照组比较,研究组的肺内动静脉分流率下降、氧合指数升高,其差异有显著性,具统计学意义(P<0.05)。结论:丙泊酚较之咪达唑仑对于急性呼吸窘迫综合症机械通气患者能够起到良好的镇静效果,见效快、易控制,值得临床推广应用。

CLINICAL STUDIES OF PROTOCOL ON MECHANICAL VENTILATION IN PATIENTS WITH ARDS CONTINUED SEDATION

Abstract:Objective:To investigate the effect of protocol on acute respiratory distress syndrome ( ARDS) in mechanically ventilated patients.Methods:for in the hospital in October 2012 -October 2014 period treated 60 patients with acute respiratory distress syn-drome patients as research subjects were randomly divided into study group and the control group .Research groups in protocol sedation mechanical ventilation before its induction therapy , group therapy , compared with midazolam .The two groups were observed and the effect of sedation time was recorded , after stopping medication and respiratory , circulatory parameters , pulmonary arteriovenous shunt fraction and oxygenation index , whether the patient has adverse reactions .Results:the patients after injection of protocol , within 5min achieved the desired effect , rapid onset .Heart pressure and heart rate were steady .Aspects of respiratory rate and oxygen saturation improved significantly, the difference was statistically significant(P〈0.05).Compared the two groups,pulmonary arteriovenous shunt rate decreased oxygenation index increased in the study group 's, the difference was significant, significant(P〈0.05).Conclusion:Protocol compared with midazolam mechanical ventilation for respiratory distress syndrome in patients with acute sedative effects can play a good role that had quick effect and was easy to control , worthy of clinical application .
