梁晓春教授治疗经行头痛经验

梁晓春教授认为经行头痛,气血不调是其病机总纲,病位与肝、脾、肾三脏密切相关。肝郁为始发因素,脾虚为重要因素,肾虚为关键因素。血瘀贯穿于病程始终,可见痰湿、寒凝等兼证,且痰、瘀二者之间可互为影响。经行头痛,病因虽复杂,但可简而概之为虚实两端,即气血两虚的"不荣则痛"虚证与气滞血瘀的"不通则痛"实证。临证可分为肝郁气滞、肝血不足、肝郁脾虚、肝郁肾虚和脾肾不足5种证型,诸证型均伴有血瘀。治疗上,以调和气血为要,强调分期施治,即月经前和月经期宜行气活血、通经止痛,月经过后宜益气养血、补虚定痛。各型经行头痛,自有特点,梁师强调临证时详审细辨,谨守病机,灵活施治,用药精准,喜用花类,酌用虫类,善用引经药,疗效显著。

Professor LIANG Xiaochun’s Experience in Treating Headache During Menstruation

Professor Liang Xiaochun thinks that disharmony of Qi and blood are the general principles of pathogenesis of menstrual headache,and the disease location is closely related to the liver,spleen and kidney.Liver depression is the initial factor of menstrual headache,spleen deficiency is an important factor,and kidney deficiency is the key factor.Blood stasis runs through the course of menstrual headache.Phlegm dampness,cold coagulation and other concurrent syndromes can be seen,and phlegm and blood stasis can interact with each other.Although the cause of menstrual headache is complex,it can be simplified and generalized as deficiency and excess,that is,deficiency syndrome of deficiency of both qi and blood,and excess syndrome of qi stagnation and blood stasis.The clinical symptoms can be divided into five types:liver stagnation and Qi stagnation,liver blood deficiency,liver stagnation and spleen deficiency,liver stagnation and kidney deficiency and spleen and kidney deficiency.All the above-mentioned syndrome types were associated with blood stasis.In the treatment,it is important to harmonize Qi and blood,emphasize treatment by stages.That is,before menstruation and menstrual period,it is advisable to carry out activating Qi and blood circulation to relieve pain.And after menstruation,it is advisable to replenish Qi and blood to tonify deficiency and relieve pain.Each type of meridian headache has its own characteristics.Prof.Liang emphasized careful examination and differentiation of clinical symptoms,careful observation of pathogenesis,flexible treatment,accurate medication,like to use flowers,appropriate insects,and good use of meridian guiding drugs,which has a significant effect.
