早期肾型糖尿病的Cys C水平及其与尿微量蛋白的相关性研究

摘    要:目的:探讨血清胱抑素C(Cys C)在早期肾型糖尿病中的水平及其与微量蛋白尿(mAlb)的相关性研究。方法:选择128例糖尿病患者做为研究对象,其中单纯糖尿病组患者42例,糖尿病肾病早期者46例,临床糖尿病肾病40例。检测3组血清Cys C、肌酐(Scr)及mAlb水平,并对Cys C及Scr与mAlb相关性进行分析。结果:单纯糖尿病组、糖尿病肾病早期及临床糖尿病肾病血清Cys C水平依次升高,差异有统计学意义(P<0.01);临床糖尿病肾病组Scr水平明显升高,显著高于单纯糖尿病组及早期肾型糖尿病(P<0.01),但后二组间差异无统计学意义(P>0.05);糖尿病组患者血清Cys C、Scr水平与mAlb显著相关(P<0.01),但Cys C相关性更为显著。结论:在糖尿病肾病早期诊断中,Cys C敏感性显著优于Scr,是诊断早期糖尿病肾病的理想检测指标。

Abstract:Objective: To explore the relationship between serum Cystatin C level and microalbuminuria(mAlb) in early diabetic nephropathy.Methods:A total of 128 cases with diabetes were selected,including 42 patients of simple diabetes,46 cases of early diabetic nephropathy and 40 subjects of clinical diabetic nephropathy.The level of serum Cys C,creatinine(SCr) and mAlb were measured in 128 patients and correlation analysis was performed in Cys C,Scr and mAlb.Results: Serum Cys C level were significantly higher in clinical diabetic nephropathy group,followed by early diabetic nephropathy group and simple diabetes group(P<0.01);SCr was higher in clinical diabetic nephropathy group than that of early diabetic nephropathy group and simple diabetes group,but there was no significant different between early diabetic nephropathy group and simple diabetes(P>0.05).Cys C and Scr had a significantly positive correlation with mAlb(P<0.01).Conclusions: In comparison with Scr,Cys C is more sensitive.And it is an ideal detection index for early diagnosis of diabetic nephropathy.
