区域动脉灌注和静脉注射乌司他丁对重症急性胰腺炎的疗效及对机体炎症应激反应的影响

目的：研究区域动脉灌注和静脉注射乌司他丁对重症急性胰腺炎的疗效及对机体炎症应激反应的影响。方法：选择本院收治的100例急性重症胰腺炎患者纳入研究,根据治疗方式不同分为两组,观察组接受乌司他丁联合生长抑素区域动脉内灌注治疗、对照组接受乌司他丁联合生长抑素静脉滴注治疗,比较两组的血淀粉酶及肝肾功能、炎症反应、应激反应。结果：观察组的血淀粉酶、谷丙转氨酶、血肌酐水平均低于对照组,差异有统计意义（P〈0.05）;观察组的肿瘤坏死因子-α（TNF-α）、C反应蛋白（CRP）、白细胞介素-8（IL-8）以及血糖、胰高血糖素水平均低于对照组,差异有统计意义（P〈0.05）。结论：区域动脉灌注乌司他丁有助于改善病情和靶器官功能、缓解炎症和应激反应,是治疗重症急性胰腺炎的理想方法。

Efficacy of regional intra-arterial and intravenous Ulinastatin in treating severe acute pancre- atitis and its influence on inflammatory stress reaction

Objective： To study the efficacy of regional intra-arterial and intravenous Ulinastatin in treating severe acute pancreatitis and its influence on inflammatory stress reaction. Methods： A total of 100 patients with severe acute pancreatitis in our hospital were enrolled and divided into two groups according to different treatment methods. Observation group were trea- ted with ulinastatin and somatostatin regional intra arterial infusion therapy and control group were treated with ulinastatin and somatostatin intravenous drip. Then serum amylase, liver and kidney function, inflammation and stress response were com- pared. Results： Serum amylase, alanine aminotransferase, creatinine levels of observation group were lower than those of con trol group; TNF-α, CRP, IL-8 and blood glucose, glucagon levels of observation group were lower than those of control group. Conclusion： Regional intra-arterial and intravenous Ulinastatin is helpful to improve disease condition and target organ function, reduce inflammation and stress response; it＇s an ideal method in treating severe acute pancreatitis.
