充血性心力衰竭患者甲状腺素水平和心功能的关系研究

目的探讨充血性心力衰竭与血清甲状腺素水平改变的关系。方法采用化学发光法分别测定充血性心力衰竭患者50例及健康对照组20例血清三碘甲状原氨酸(T3)、四碘甲状原氨酸(T4)、逆-三碘甲状原氨酸(rT3)、促甲状腺素(TSH)。结果充血性心力衰竭患者血清甲状腺素水平与对照组比较显示,心功能Ⅲ级、Ⅳ级患者的T3水平明显低于对照组(t=9.57,P<0.05;t=12.49,P<0.01);心功能Ⅱ级、Ⅲ级、Ⅳ级患者的rT3水平明显高于对照组(t=5.46,P<0.05;t=6.12,P<0.05;t=10.97,P<0.01);T4、TSH指标在各组间比较无统计学意义(P>0.05)。结论血清甲状腺素水平异常与充血性心力衰竭的发病及病情发展有密切关系,监测甲状腺素水平可作为判断心力衰竭严重程度的指标。

The correlation of serum thyroxin and congestive heart failure

Objective To study the correlation of serum thyroxin level with congestive heart failure. Methods A total of 50 patients suffering from CHF and 20 healthy control subjects in the time were enrolled. Serum T3,T4,rT3 and TSH were de- tected by chemolumiescence. Results The study showed that the T3 in Ⅲ and Ⅳ level of heart function was lower than that in control group significantly （t = 9.57,P 〈 0.05;t = 12.49, P 〈 0.01）; the rT3 in Ⅱ ,Ⅲ and IV level of heart function was higher than that in control group significantly （t = 5.46,P 〈 0.05;t = 6.12, P 〈 0.05;t = 10.97, P 〈 0.01）; The levels of T4,TSH were no significantly different in groups（P 〉 0.05 ）. Conclusion Serum thyroxin abnormal level with congestive heart failure and the onset of illness development has close relationship; The level of the thyroid hormones can be use as the indicators to evaluate the extent of CHF.
