早期静注人免疫球蛋白治疗重症手足口病的临床研究

目的探讨早期静脉注射人免疫球蛋白治疗重症手足口病的效果。方法回顾性分析本院2009年6月-2011年3月收治的46例重症手足口病患儿的资料，全部患儿分为研究组24例与对照组22例，两组患儿均给予抗炎、抗病毒、退热、营养支持等常规治疗，研究组24例患儿在常规对症治疗基础上，经家属知情同意，重症早期给予静脉注射人免疫球蛋白2．0～5．0g，分2d给药，注射免疫球蛋白之前给予常规剂量地塞米松（0．2-0．3）mg／（kg·d），对比两组临床效果。结果本组患者均治愈出院，未出现危重病例。研究组体温消退时间、食欲改善时间、精神症状消失时间、平均住院时间明显短于对照组．两组芹够行统计学意义。结论重症手足口病患儿早期静注人免疫球蛋白可促进症状改善、缩短住院时间、增强免，乞玎，防止肺水肿、脑水肿等出现，促进患儿恢复。

Clinical research of early intravenous human immunoglobulin in treatment of severe hand,foot and mouth disease

Objective To explore the efficacy of early intravenous human immunoglobulin in treatment of severe hand,foot and mouth disease.Methods The data of 46 cases of children with severe hand,foot and mouth disease from June 2009 to March 2011 was analyzed retrospectively.All children were divided into study group(24 cases) and control group(22 cases),and two groups of children were given routine treatments of anti-inflammation,antivirus,antifebrile,nutrition support;24 cases of study group were early given intravenous human immunoglobulin 2.0-5.0 g in 2 days and given Dexamethasone at regular dosage of 0.2-0.3 mg/kg per day before injection of immunoglobulin on basis of conventional symptomatic treatment after informed the family members.Clinical effects of two groups were compared.Results All patients were cured and discharged,and no serious disease appeared.The temperature subsided time,appetite improved time,psychiatric symptoms disappeared time and average hospitalization time of the study group were obviously shorter than those of the control group,and the differences of two groups were statistically significant.Conclusion Early intravenous human immunoglobulin in severe hand,foot and mouth disease children can promote improvement of symptoms,shorten hospital stay,enhance immunity,prevent the emergence of pulmonary edema,cerebral edema and promote the recovery of children.
