1H—MRS在鼻咽癌放疗后颞叶脑损伤中的应用研究

摘    要:目的探讨氢质子磁共振波谱技术(1H—MRS)对鼻咽癌(NPC)放射性脑损伤的诊断价值,分析颞叶脑组织代谢物的变化规律。方法采用1H—MRS检测正常健康组与NPC放疗后颞叶损伤组的大脑颞叶部内的N-乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱(Cho)的含量及相互间比值。正常健康组为10名脑部无任何病变者,NPC放疗后颞叶损伤组为28例脑颞叶部出现明显病变的患者,使用Philips Achieva超导型磁共振成像仪,选择点分析波谱(PRESS)序列采集,获得正常健康组、放疗后颞叶损伤组的脑颞叶内的波谱图像。测量NAA、Cr、Cho、NAAZCr、Cho/Cr、NAA/Cho值,并进行统计学分析。结果放疗后颞叶损伤组的NAA、Cr、Cho、NAA/Cr、Cho/Cr、NAA/Cho分另4为0.21±0.08、0.27±0.23、0.51±0.11、0.78±3.14、1.92±0.22和0.40±0.10,NAA、NAA/Cr、NAA/Cho值比正常健康组要低,差异有统计学意义(P〈0.05);Cho、Cho/Cr明显上升,差异有统计学意义(P〈0.05)。结论1H—MRs检查对评估NPC放射性脑损伤具有一定的价值。

Abstract:Objective To study the diagnosis value of proton magnetic resonance spectroscopy (1H - MRS ) on the temporal lobe brain tissue with radiation injuries of nasopharyngeal carcinoma ( NPC ) , analyze the metabolites changes of temporal lobe brain tissue. Methods 1H - MRS examina- tion was underwent on healthy group and temporal lobe brain injury after NPC radiotherapy group, analyze the ministry metabolites, such as N - acetyl aspartate ( NAA), creatine ( Cr), choline (Cho) levels and the ratio each other. Healthy brain group include 10 persons, whose brain without any lesions. Temporal lobe injury after NPC radiotherapy group include 28 patients with temporal lobe brain lesions, the two groups were compared, using Philips Achieva superconducting magnetic resonance imaging device, select the point of spectroscopy (PRESS) acquisition sequence, acquire the brain magnetic resonance images in temporal lobe of healthy group, temporal lobe injury after radiotherapy group. Measure the values of NAA, Cr, Cho, NAA/Cr, Cho/Cr, NAA/Cho, and analyze them with statistics. Results The values of NAA, Cr, Cho, NAA/Cr, Cho/Cr, NAA/Cho in temporal lobe injury after radiotherapy group were 0.21 ±0.08, 0.27± 0.23, 0.51 ±0.11,0.78 ±3.14,1.92 ±0.22 and 0.40±0.10, the values of NAA, NAA/Cr, NAA/ Cho was lower than the normal healthy group, there were significant differences between the two groups (P 〈 0.05). Compare with the normal healthy group, the value of Cho, Cho/Cr increased significantly (P 〈 0.05). Conclusions 1H - MRS examination has a certain value to assessment the radiation brain injury after NPC radiotherapy.
