摘    要:目的探讨乙型病毒性肝炎病人血清甲状腺激素异常的机制,评估肝脏病变程度和预后等临床意义。方法采用放射免疫法检测253例不同组别乙型肝炎病人血清T3、T4、rT3、TSH含量并与30例健康体检者对照。结果重型肝炎、肝炎肝硬化、慢性肝炎轻、中、重度病人血清T3、T4、TSH含量显著低于对照组(P〈0.01),rT3显著高于对照组(P〈0.01),统计学有显著差异。除急性肝炎和对照组外,其余各组均存在低T3、T4,高rT3现象。而急性肝炎组T3、T4、TSH稍低于对照组,rT3稍高于对照组,统计学无显著差异(P〉0.05)。结论血清甲状腺激素水平可作为评估乙肝病人病情和预后的另一敏感指标。

The content of serum thyroid hormone in patients with chronic viral hepatitis B and it's clinical significance

Abstract:Objective To investigate the mechanisms of serum thyroid hormone abnormality of patients with viral hepatitis B and the clinical significance on evaluating the disease extent and prognosis. Methods Applying radio-immunity method to detect the T3, T4, rT3, TSH concentration in the patients' serum of different groups and compare them to the 30 health people. Results T3, T4, TSH concentrations of patients with serious hepatitis, hepatitis liver cirrhosis, chronic hepatitis(light, middle, and serious grade) are obviously lower than the control group (P〈0.01), and the rT3 level is higher than the control group (P〈0.01), both of them have significant statistic differences. The phenomenon exists in all the other groups except the acute hepatitis and the control group that the T3, T4 level is low and the rT3 level is high. Whereas in acute hepatitis group the T3, T4, TSH levels are slightly lower than the control group, the rT3 level is slightly higher than the control group, and there is no statistic difference (P〉0.05). Conclusions The serum thyroid hormone level can be regarded as another sensitive index for evaluating the disease extent and prognosis.
