小夹板固定治疗儿童肱骨髁上伸直型骨折60例

手法整复、屈肘位小夹板固定治疗儿童肱骨髁上伸直型骨折,随访统计患肢肘内翻发生率.测量患肘Baumann角,探讨儿童肱骨髁上伸直型骨折并发肘内翻的机制.结果显示:肘内翻发生率为25%.复位后与解除固定时的Baumann角比较,有显著性差异(P＜0.01),肘内翻发生率与复住后Baumann角的大小有关(P＜0.01).提示各种因素造成的骨折远端的内倾是引起肱骨髁上骨折并发肘内翻的重要原因.Baumann角可作为判断骨折端是否发生尺倾的指标.

Small Splint Fixation in Treating 60 Cases of Supracondylar Fracture of Humerus in Children

To investigate the pathogenesis of the supracondylar fracture of humerus complicated cubitus varus in children,manipulative reduction and small splint fixation were used,and the incidence rate of cubitus varus was counted and the angle Baumann of the bad elbow was measured in the follow-up visit.The results showed that the incidence rate of cubitus varus was 25%,there was highly significant differences in angle Baumann between the angle measured after reduction and after dismantling fixation(P<0.01),and the incidence rate of cubitus varus was concerned with the angle Baumann of the bad elbow after reduction(P<0.01).Conclusion:The introversion of distal end of the supracondylar fracture of humerus caused by various kinds of factors is the main pathogenesis of the supracondylar fracture of humerus complicated cubitus varus in children,and angle Baumann can be used for judging the introversion of distal end during the treatment.
