摘    要:目的比较不同病理模型大鼠脾切除后细菌的吞噬功能.方法75只Wistar大白鼠建立正常对照、肝硬化、激素负荷、免疫抑制4组动物模型,行脾切除术.所有大鼠测定中性粒细胞体外吞噬功能,计算中性粒细胞的吞噬率(PR)和吞噬指数(SI).结果大鼠中性粒细胞对金黄色葡萄球菌、肺炎球菌和大肠杆菌的平均PR分别为26.25%、22.60%和22.57%,平均SI分别为4.72、3.70和3.53,后两种细菌与金黄色葡萄球菌相比均有显著性差异(P<0.05).正常对照大鼠脾切除前后对细菌的平均PR分别为39.21%和30.71%,平均SI分别为5.48和4.11,脾切除前后比较有显著性差异(P<0.05).4组模型大鼠脾切除后对细菌的平均PR分别为30.71%、20.79%、14.46%和13.86%,后三组与正常脾切组比较均有显著性差异(P<0.01),平均SI分别为4.11、3.77、3.49和3.08,免疫抑制组与正常脾切组比较有显著性差异(P<0.05).结论大鼠中性粒细胞对肺炎球菌和大肠杆菌的PR和SI较低.脾切除术后大鼠对细菌的PR和SI均有下降.不同病理状态降低大鼠对细菌的PR和SI,因此临床上对病理性脾切除患者的围手术期处理应有充分的认识和准备.

Abstract:Objective To compare the bacterial phagocytic function in different rat models after splenectomy. Methods Four groups of control, liver cirrohsis, steroid loaded and immune depression were estabished in 75 Wistar rats, and was splenectomy performed. Granulocyte phagocytic test was conducted in all rats, and phagocytic ratio (PR) and swallow index (SI) were counted. Results The average PR of staphylococcus, pneumococcus and colibacillus were 26. 25 % , 22. 60 % and 22. 57 % , and the average SI were 4.72,3.70 and 3.53 respectively, the average PR and SI of last two kinds of bacteria were much lower than at of staphylococcus (P<0.05). The average PR before and after splenectomy in control group was 39. 21% and 30. 71% (P<0. 05), the average SI was 5.48 and 4.11(P<0.05). After splenectomy, the average PR of four rat models were 30.71%,20.79%, 14.46%,13.86% respectively and the value in the last three groups was much lower than that in the control group(P<0.01). The average SI was 4.11,3.77,3.49,3.08 respectively and that in the immune depression group was significantly different from that in the control group (P<0.05). Conclusion PR and SI of pneumococcus and colibacillus are relatively lower. PR and SI decrease after splenctomy. The different pathogenesis lowers PR and SI. So for the clinical patients with pathogenesis, we should pay more attention and get more preparation to their management around splenectomy.
