妇科切口感染的流行病学调查与控制

目的：调查妇科口感染流行的原因，以便采取有效的控制措施，尽快控制切口感染流行。方法：采取前瞻性及目标性的病例调查，并对手术室、病房换药室综合情况及环境卫生学调查。结果：2000年2月17日－4月20日妇科115例腹部手术中有22例发生切口感染，罹患率为19．13％。首例病人为金葡菌感染。切口感染与手术房间的分布，刀诊，接台及手术 生之间均无统计学差异。但环境卫生学检测手术医生手、无菌物品及换药室空气均超标。结论：此次妇科切口感染流行多种因素有关。主要为手术、换药过程中消毒隔离措施不严。经加强无菌操作及改变换药方式控制了切口感染的流行。

Epidemologic Studying and Controlling of Incisional Infection in the Department of Gynecology

Objective:To investigate the reason and control an epidemic of incisionial infection in the department of gynecology.Methods:Taking active and prospective surveillance for incisional infection and surveying the hygiene of medical workers hand,operating room dressing change room.Results:Among 115 patients with abdomen operation in the department of gynecology,from Feb 17 to Apr 20 in 2000,22 cases had a abdomen incisional infection.The incisional infection rate was 22.8%.There were no linkage between incisionial infection and the distribution of operation room,emergency treatment,succeed operation,and operators.But bacterial account of the operators hands,sterile instrument and the air of dressing change room revealed all exceed national standard.Conclusions:There are many reasons contribute to this epidemic of incisionial infection,the main reason were focused on disinfection and isolation,this epidemic were stopped by strengthening disinfection and isolation.
