超声弹性成像比值法对乳腺良恶性肿块的诊断价值

目的探讨超声弹性成像比值法诊断乳腺良、恶性病变的价值及其最佳诊断界点。方法对手术病理结果证实的338个乳腺肿块的超声弹性成像图,利用超声仪器提供的顺应性比值测量方法,比较病灶与同深度乳腺组织的顺应性,从而获得病灶与周围组织的硬度比。用ROC曲线探讨超声弹性成像比值法的最佳诊断界点。结果超声弹性成像比值法对乳腺病灶的最佳诊断界点3.04,其ROC曲线下面积为0.958,其敏感性、特异性及准确性分别为95.0%、89.9%和92.9%。结论超声弹性成像比值法使用简便,有助于对乳腺良恶性肿块的诊断与鉴别。

The value of ultrasonic elastography ratios method for differentiating breast malignancy and benignancy

Objective To explore the best cutoff point of ultrasonic elastography(UE) for differentiating malignant and benign breast lesions using ratios method,and to assess its diagnostic value.Methods 338 surgical excision proved breast lesions’s UE images were reviewed.A receiver operating characteristic(ROC) curve was used to explore the best cutoff point.The area under the ROC curve(Az) and the sensitivity,specificity and accuracy were calculated.Results 3.04 was the best cutoff point of UE ratios method for differentiating malignant and benign breast lesions.The Az,sensitivity,specificity and accuracy were 0.958,95.0%,89.9% and 92.9%.Conclusion The ratios method of UE were convenient in practice and valuable in differentiating breast malignancy and benignancy.
