宫晓燕辨治孤立性肺结节病机思路

宫晓燕认为孤立性肺结节中医可归为“肺积”范畴。本病多为本虚标实,虚实夹杂之证。本虚为肺气阴两虚、脾气亏虚,实为热毒、气郁、痰凝、血瘀。临证外感六淫雾霾邪毒为致病的基础,强调扶正的同时解毒为要;认为气郁为致病的先导,调畅气机尤为重要,以疏肝解郁、调畅气机为主;同时脾胃健运为本病发展和预后的关键,应重视健脾胃,培土生金,散结祛瘀,贯彻始终。

Gong Xiaoyan's ideas on differentiating the pathogenesis of solitary pulmonary nodule

Professor Gong Xiaoyan maintains that solitary pulmonary nodule can be classified into the category of“pulmonary accumulation”in traditional Chinese medicine.Syndromes of the disease are mostly characterized as origin deficiency and manifestation excess,intermingled deficiency and excess.Origin deficiency can be manifested as deficiency of both lung qi and yin,and spleen qi deficiency,while excess as heat toxin,qi stagnation,phlegm coagulation and blood stasis.In clinical practice,exogenous factors of six climatic exopathogens,haze and pathogenic toxin are considered as the basis of the disease pathogenesis,and it is emphasized that detoxification is the key of treatment while strengthening the body.Qi stagnation is considered to be the leading cause of the disease,and regulating and promoting qi movement is especially important so as to disperse the stagnated liver qi and relieve depression.At the same time,the vitality and smooth transportation of spleen and stomach is the key to the development and prognosis of the disease.Attention should be paid to strengthening spleen and stomach,invigorating metal by reinforcing earth.Dispelling stagnation and removing blood stasis should be implemented throughout the complete course of disease treatment.
