地龙对糖尿病肾病大鼠肾脏保护作用的研究

摘    要:目的探讨地龙煎液对糖尿病肾病(DN)大鼠肾脏的保护作用及机制。方法链脲佐菌素(STZ)诱导糖尿病肾病大鼠模型,随机分为糖尿病肾病模型组、地龙低剂量组、地龙高剂量组、ACEI组(阳性对照治疗组)各12只,另以12只正常大鼠作为正常组。地龙低剂量组给予地龙煎液1g.kg-1.d-1、地龙高剂量组3.gkg-1.d-1、ACEI组给予培哚普利4mg.kg-1.d-1灌胃,糖尿病肾病模型组及正常组给予0.9%NaC l溶液灌胃。共观察12周,检测各组12周末24h尿微量白蛋白定量、肾重/体重、肾功能(Sc、rBUN)、肾脏病理变化情况,免疫组织化学检测肾组织Ⅳ型胶原蛋白的表达。结果糖尿病肾病模型组肾重/体重,24h尿微量白蛋白,肾小球、肾小管PAS评分,肾组织Ⅳ型胶原蛋白表达水平均显著高于正常组。地龙高剂量组、地龙低剂量组、ACEI组24h尿微量白蛋白、肾小球、肾小管PAS评分,肾组织Ⅳ型胶原蛋白表达水平较糖尿病肾病模型组显著降低(P〈0.05),其中地龙高剂量组、ACEI组较地龙低剂量组更为显著。结论地龙能显著降低糖尿病肾病大鼠24h尿微量白蛋白水平,减轻肾小球硬化及肾小管损伤的程度,减少肾脏Ⅳ型胶原蛋白的表达,对肾脏有一定的保护作用。

Abstract:Objective To investigate the protective effects of earthworm on experimental diabetic nephropathy.Methods Sprague-Dawley rats injected with streptozotocin(STZ) were randomly divided into diabetic nephropathy group,low-dose therapy group,high-dose therapy group and ACEI group;another 12 normal rats were used as control group.The rats in low-dose therapy group were daily administered earthworm decoction at the dose of 1 g/kg,high-dose therapy group at 3 g/kg and ACEI group with perindopril at 4 mg/kg for 12 weeks.Ratio of kidney weight to body weight,albumin excretion rate(AER) were measured.Renal injury was also assessed by kidney pathology.Expression of extracellular matrix associated protein,collagen Ⅳ,was investigated by immunochemistry.Results Increased AER in diabetic rats was attenuated in low-and high-dose therapy groups and ACEI group(P0.05).Increased pathological injures in diabetic nephropathy rats were also attenuated in low-and high-dose earthworm therapy groups.Earthworm treatment reduced the over expression of collagen Ⅳ in diabetic neprhopathy(P0.05).Conclusion Earthworm can prevent the progression of diabetic nephropathy by reducing the expression of collagen Ⅳ.
