中医药综合治疗原发性肝癌的疗效及影响生存因素的Cox模型分析

目的:评价中医药综合治疗原发性肝癌的疗效并分析影响生存的预后因素。方法:将352例采用中医药综合治疗(中药配合介入治疗)的原发性肝癌分为中药合莪术油介入组、中药合化疗介入组、中药辨证治疗组等3组进行疗效分析,对预后影响因素行单因素和Cox回归分析。结果:352例患者总体平均生存时间为9.0月,中位生存时间为5.0月,其中中药合莪术油介入组(85例)中位生存期10.25月,平均生存时间13.0月,总计1年、2年、3年、4年、5年生存率为37.76%、11.76%、4.71%、3.52%、2.36%;中药配合化疗介入组(90例)中位生存期9.75月,平均生存时间10.0月,1、2、3、4年生存率为24.44%、6.67%、1.11%、0;中药辨证治疗组(177例)中位生存期4.32月,平均生存时间5.0月,1、2、3、4年生存率为6.78%、2.26%、0.56%、0。单因素分析治疗有意义的预后因素为临床分期、肿瘤大小、静脉癌栓、肝功能Child-Pugh分级、治疗方式、介入方式、莪术油介入、化疗介入、介入次数和中药剂数。多因素分析(Cox模型分析)对生存有显著意义的预后因素为肿瘤大小,肝功能Child-Pugh分级为预后危险因素,介入次数、中药剂数为预后保护因素。结论:中医药辨证治疗肝癌疗效有限,中药配合介入治疗则疗效有显著意义,介入前后辅助中药治疗可进一步提高疗效和生存时间。
