中西医结合治疗慢性乙型重型肝炎122例观察

本文总结了中西医结合治疗122例慢性乙型重型肝炎(早、中、晚期)的经验。全部病例均用基础疗法,部分病例加用肝细胞生长因子(HGF)。中药治疗分为凉血活血解毒扶正组(凉解组)及清热解毒组(清解组)。结果总有效52例,无效70例。血清胆红素、凝血酶原活动度值及就诊时间对预后有重要影响。凉解组与清解组总的病死率及两组病例早期加与不加HGF的病死率均无差异。中期凉解组加与未加HGF的存活率有显著性差异,P<0.01;清解组加与未加HGF生存率无显著性差异,P>0.05:但中期病例凉解组与清解组加HGF的生存率有显著性差异,P<0.01。

Treatment of Severe Chronic Hepatitis B by Combination of TCM and WM Therapy -with an Analysis of 122 Cases

This paper is a summary report of treatment of 122 cases of severe chronic hepatitis bycombination of TCM and WM therapy. Among them, 44 cases were in their early stage of disease,66 in their intermediate stage, and 12 in their advanced stage. They were treated by basic therapywith hepatic cell growth factor (HGF) added to some of them. The patients were divided into twogroups, one receiving treatment of blood-cooling and detoxifying Chinese herbs, the other receivingtreatment of heat-clearing and detoxifying Chinese herbs. The treatment was effective for 52 cases,and ineffective in 70 cases. Values of serum bilirubin (SB) and prothrombin activity (PA) and the timeof initial consultation had marked influence on their prognoses. No difference was found in totalmortality between the two groups, also no difference was found in mortality between the former andlatter group with or without the addition of HGF at the early stage of disease. The survival rate ofthe former group was sinnificantly higher than t of the latter one with HGF added at theirintermediate stage of disease, which were 60% and 14.7% respectively, P<0.01. The conclusion isthat as diagnosis and treatment were based on the Syndrome Differentiation of patients at intermedi-ate stage of disease, Chinese herbs will be effective in many ways such as lowering and eliminatingjaundice, improving symptoms and signs, preventing and curing complications, curing coexistingdiseases, and repairing pathological lesions.
