基于球结膜微循环改变验证彭静山教授观眼识病理论

摘    要:目的通过观察气虚血瘀证中风恢复期患者的球结膜微循环形态改变,验证彭静山教授观眼识病理论的客观科学性。方法选取诊断为气虚血瘀证中风恢复期患者70例作为疾病组,70例健康人作为健康组。采用微循环检测仪对两组观察对象的球结膜微循环形态异常区进行形态学分析,并根据田氏定量分析方法计算总积分。记录疾病组患者美国国立卫生研究院卒中量表(National Institute of Health Stroke Scale, NIHSS)评分。所有数据采用SPSS 22.0软件进行统计分析。结果 (1)两组组间球结膜微循环异常频次比较差异有统计学意义(P<0.05);组内两眼间异常频次比较差异无统计学意义(P>0.05)。(2)两组观察对象脾胃区球结膜微循环异常频次分布比较差异有统计学意义(P<0.05)。(3)两组脾胃区球结膜微循环形态积分比较中,形态总积分、微血管数、细静脉管径均较健康组差异有统计学意义(P<0.05),但细动脉管径比较差异无统计学意义(P>0.05)。(4)两组观察对象脾胃区球结膜微循环的异常走形比较差异有统计学意义(P<0.05)。(5)疾病组观察对象的球结膜微循环形态积分与NHISS评分具有正相关关系(P<0.05)。结论气虚血瘀证中风恢复期患者的球结膜微循环改变多表现为脾胃区的形态异常,具体表现是微血管数的减少,细静脉管径的轻度狭窄,血管走形的异常,且球结膜微循环形态学改变与神经功能缺损正相关,充分验证了彭静山教授观眼识病理论的客观科学性。为眼针临床应用提供一定的科学依据。

Abstract:Objective By observing the changes of the bulbar conjunctiva microcirculation in stroke convalescent patients with Qi deficiency and blood stasis, the scientificity of the theory of diagnosis of inspecting eyes by Professor PENG Jingshan was verified. Methods Seventy patients with stroke convalescence of Qi deficiency and blood stasis were selected in the study(the disease group). In the same period, 70 healthy people were recruited as the health group. We observed the bulbar conjunctiva microcirculation of two groups. And we used microcirculation detector for analyzing the abnormal shape and distribution of bulbar conjunctiva microcirculation. We calculated the total and itemized integral according to Tian’s theory. Patients in the disease group were recorded with the National Institute of Health Stroke Scale(NIHSS) score. The date was analyzed by software SPSS 22.0. Results(1)Compared with the health group, the abnormal frequencies of the bulbar conjunctiva microcirculation in the disease group were significantly different(P<0.05). While there was no significant difference between the left eye and the right eye in the same group(P>0.05).(2)There was significantly difference between the two groups in the distribution frequency of the spleen and stomach region of the bulbar conjunctiva microcirculation(P<0.05).(3)In the comparison of morphological integral between disease group and health group,the score of the microvascular number and fine venous diameter were significantly different(P<0.05). While the score of the fine arteriovenous diameter ratio had no significant difference(P>0.05).(4)The degree of vascular curvature was significantly different between the two groups in the spleen and gastric region of the bulbar conjunctiva microcirculation(P<0.05).(5)Correlation analysis showed that the morphological score of bulbar conjunctiva microcirculation of the disease group’s patients was positively correlated with the patient’s NHISS score(P<0.05). Conclusion In the patients with stroke convalescence of Qi deficiency and blood stasis syndrome, the bulbar conjunctiva microcirculation is mostly abnormal in the spleen and stomach region, which is mainly manifested by the decrease in the microvascular number,fine venous diameter and the increase in the degree of vascular curvature. And the morphological changes of the bulbar conjunctiva microcirculation were related to the nerve function. It was fully verified the scientificity of the theory of diagnosis of inspecting eyes by Professor PENG Jingshan. And it would provide the scientific basis for clinical application of eye acupuncture.
