铁缺乏对妊娠期甲状腺功能影响的研究进展

铁缺乏症(ircm deficiency,ID)会增加妊娠期甲状腺功能减退症(甲减)的发生风险。目前认为其发生与甲状腺轴及核受体介导的信号转导通路、甲状腺过氧化物酶及脱碘酶活性、甲状腺自身免疫损伤以及补碘效果等有关。近来研究显示,补铁治疗能够改善甲状腺功能并缓解甲减患者的症状,提示铁剂可能在妊娠期甲减的治疗中扮演重要角色。但目前尚缺乏妊娠期特异的ID诊断标准,并且铁剂的应用剂量和疗程不一,今后仍需开展更多关于铁剂补充对改善妊娠期ID患者甲状腺功能及妊娠结局的研究。

Research progress of the effect of iron deficiency on thyroid function during pregnancy

Iron deficiency(ID)increases the risk of hypothyroidism during pregnancy.At present,it is believed that its occurrence is related to thyroid axis and nuclear receptor pathway,thyroid peroxidase and deiodinase activity,thyroid autoimmune damage and the effect of iodine supplement.Recent studies have shown that iron supplementation can improve thyroid function and relieve symptoms in patients with hypothyroidism,suggesting that iron may play an important roles in the treatment of hypothyroidism during pregnancy.However,at present,there is a lack of pregnancy-specific diagnostic criteria for ID,and the dosage and course of treatment of iron are different.In the future,more studies on the effects of iron supplementation on improving thyroid function and pregnancy outcomes in patients with pregnancy-related ID need to be carried out.
