脑白质损伤程度与认知功能障碍的关系

摘    要:目的探讨脑白质损伤程度与认知功能障碍之间的关系。方法根据头颅MRI检查结果随机选取存在不同程度脑白质损伤的患者120例,根据蒙特利尔认知量表(MoCA评价量表)区分认知功能障碍及非认知功能障碍患者,通过Scheltens半定量评分评价脑白质损伤程度,并通过相关分析评价脑白质损伤程度与认知功能障碍之间的关系。结果存在认知功能障碍的患者42例,不存在认知功能障碍的患者78例,其Mo-CA量表评分结果分别为22.44±3.47与28.42±1.13,二者比较差异有统计学意义(P<0.05)。Scheltens半定量评价表分析两组研究者的平均得分分别为8.01±2.50与2.49±0.59,二者比较,差异有统计学意义(P<0.05);相关分析结果表明脑白质损伤组Scheltens半定量结果与MoCA评分结果呈正相关,相关系数为0.869(P<0.05)。结论脑白质损伤是导致认知功能障碍的重要因素,Scheltens半定量评价表可能对脑白质损伤合并认知功能障碍患者具有辅助临床诊断价值。

Abstract:Objective To study the relationship between white matter lesions and cognitive dysfunction.Methods 120 patients with white matter lesions were randomly selected according to their MRI results.All patients were divided into patients with or without cognitive dysfunction according to the results of MoCA scale.Scheltens semi-quantitative evaluation was used to detect the damage degree of white matter.Correlation analysis was used to evaluate the relationship between white matter lesions and cognitive dysfunction.Results There were 42 patients with cognitive dysfunction and 78 patients without cognitive dysfunction.The results of MoCA scale for these two groups were 22.44±3.47 and 28.42±1.13,respectively.Scheltens semi-quantitative evaluation for patients with white matter lesions and patients without white matter lesions were 8.01±2.50 and 2.49±0.59,respectively.There was significant difference between this two groups(P<0.05).The correlation coefficient of Scheltens semi-quantitative results and MoCA scale was 0.869(P<0.05).Conclusions White matter injury could lead to cognitive dysfunction.Scheltens semi-quantitative evaluation has important clinical value for the diagnosis of white matter injury and cognitive dysfunction.
